Studies of ocular murine cytomegalovirus infection.

The pathogenesis of ocular cytomegalovirus (CMV) infection in mice was studied in detail as a model of ocular involvement of human CMV infection. Smith strain of mouse CMV (MCMV) was inoculated into the anterior chamber of the eye and viral antigen was located by immunofluorescence. When salivary gland passaged MCMV (SG-MCMV) was inoculated into the 12- to 18-day-old ICR/Sic mice, it elicited transient uveitis, retinitis, and scleritis during the early phase of infection followed by spread into the lacrimal glands, extraocular muscles and salivary glands. Mouse embryonic fibroblasts (MEF) pass

Ocular inflammation in autoimmune MRL/Mp mice.

Congenic mice of the MRL/Mp strain spontaneously develop an autoimmune connective tissue disease that shares immunologic and histopathologic features with the human disorders systemic lupus erythematosus, rheumatoid arthritis, and systemic vasculitis. The autoimmune disorder in these mice is markedly accelerated by the recessive gene lpr. Older MRL/Mp-lpr/lpr mice develop significant inflammatory ocular disease, including choroiditis, scleritis, and orbital vasculitis. Animals of both the MRL/Mp-+/+ and MRL/Mp-lpr/lpr substrains develop lacrimal gland inflammatory infiltrates. The MRL/Mp mouse

Changes at the periphery of a lesion in necrotising scleritis: anterior segment fluorescein angiography correlated with electron microscopy.

Anterior segment fluorescein angiography in scleral disease reveals highly characteristic changes in the vasculature of the anterior segment associated with necrotising scleritis. The vaso-obliterative changes discovered in this investigation have been correlated with the histopathology in a patient who had to have tissue replaced. Light and electron microscopy of scleral tissue excised from sites of vascular closure detected by fluorescein angiography peripheral to the scleral defect revealed pathological changes in the absence of inflammatory cell infiltration. These changes included vascula

Lasiodiplodia theobromae panophthalmitis.

Lasiodiplodia theobromae is a rare ocular pathogen. We report a case of panophthalmitis caused by this fungus. The patient was a healthy 62-year-old janitor with no history of ocular trauma in whom keratitis developed. Cultures of corneal scrapings identified the fungus. Despite treatment with appropriate antimycotics the keratitis progressed to infectious scleritis and then spontaneous perforation of the globe, which necessitated enucleation. The ultrasonographic, microbiologic and histologic findings are discussed and the results of in-vitro sensitivity testing presented.

Local treatment of necrotizing scleritis with cyclosporin A.

A report is presented on an 83-year-old female patient with seropositive rheumatoid arthritis who developed a necrotizing scleritis of the right eye. The clinical course could not be influenced by indomethacin, cysteine, and the cytostatic chlorambucil. Local cyclosporin treatment, however, which was initiated after a 4-week therapy-free period, led to considerable improvement within a few days. The disease disappeared after 6 weeks of treatment. There is no recurrence as yet, the follow-up period being 13 months.

Immunology of multisystem ocular disease.

In this review based on Prof. James' 1983 George Nelms Wise Lecture, the authors summarize proven and speculated relationships involved in the immunology of multisystem ocular disease. Extensive tables elucidate the mechanisms underlying such conditions as, systemic lupus erythematosus, polyarteritis nodosa, Sjögren's syndrome, Wegener's granulomatosis, scleritis, sarcoidosis, Behcet's disease, idiopathic retinal vasculitis, sympathetic ophthalmia, Vogt-Koyanagi-Harada syndrome, and dysthyroid eye disease.

Anterior segment fluorescein angiography in the diagnosis of scleral inflammation.

Anterior segment fluorescein angiography was performed on 135 patients with scleritis and episcleritis and on several normal subjects. It was confirmed that the coats of the eye anterior to the extraocular muscles derive their blood supply from perforating ciliary vessels. Characteristic patterns were detected which could distinguish the various forms of scleritis and episcleritis. Fluorescein angiography was found to be particularly useful in the detection of those patients who have early, but not clinically recognisable, necrotizing scleritis. The fluorescein angiographic appearances have be

Lacrimal gland enlargement as one of the ocular manifestations of Wegener's granulomatosis.

Wegener's granulomatosis is characterized by chronic, focal, necrotizing and granulomatous vasculitis. The localized form involves mainly the upper or lower respiratory tracts, or both. In the commoner generalized form, the kidneys are also affected. Literature findings suggest that ocular manifestations are a predominating sign in about 30% of cases. Ocular findings may include conjunctivitis, keratitis and sclerouveitis, pseudotumor of the orbit or proptosis. We report on a patient who showed bilateral episcleritis, keratitis, nasolacrimal duct obstruction and enlargement of the lacrimal gla

Chronic ophthalmic Wegener's granulomatosis.

A case of Wegener's granulomatosis is reported which manifested episcleritis followed by scleritis, and followed in turn by separate episodes of right and left orbital pseudotumor spanning 15 years prior to respiratory tract disease. The unusually protracted progression of symptoms in this case may be explained on the basis of the histopathological findings. Areas of fibrinoid necrosis of connective tissue occurred alone or alternated with areas of nongranulomatous fibrinoid necrosis of blood vessel walls. Granulomatours vasculitis was not observed. This histopathological picture is pathognomi

Scleritis and the spectrum of external inflammatory eye disease.

Scleritis, episcleritis and Mooren's ulcer form a poorly understood spectrum of inflammatory syndromes involving the ocular surface. Their pathogenesis is thought to be a vasculitis and may be related to immune complex deposition, as these syndromes are often associated with systemic diseases which are assumed to be vasculitic in aetiology. Scleritis and Mooren's ulcer are amongst the most severe external inflammatory diseases, often causing extensive damage to the sclera and cornea. They require aggressive investigation and management, often including systemic anti-inflammatory therapy. Syste

Ocular findings in leprosy in the United States.

We examined 55 clinic patients with biopsy-proven Hansen's disease and catalogued the ocular findings. Forty patients had lepromatous disease and 15 had tuberculoid disease. Visual acuity was 20/40 or better in 52 patients. Examination of pupillary size in all cases and after dilation with cocaine and hydroxyamphetamine in 15 cases showed decreased sympathetic innervation to the iris dilator muscle in seven cases. Ocular inflammation was uncommon: there was one case of active interstitial keratitis, three of episcleritis, and four of iritis. Although 41 patients had ocular problems attributabl

Terminal complement component deficiencies and rheumatic disease: development of a rheumatic syndrome and anticomplementary activity in a patient with complete C6 deficiency.

Hereditary deficiencies of early complement components have usually been associated with the development of rheumatic diseases like systemic lupus erythematosus (SLE), while terminal component deficiency is well known to predispose to recurrent neisserial infection. In contrast, only recently have patients been reported with rheumatic disease and hereditary deficiency of a terminal component. The clinical syndrome in these patients has been characterised as 'SLE-like'. We describe here a third patient with complete C6 deficiency and a systemic rheumatic illness characterised by fever, anaemia,

Scleral canal size and optic nerve head drusen.

From projected optic disk photographs we measured the size of the scleral canal in two samples of emmetropic patients: one of patients with unilateral pseudopapilledema and drusen and the other of the general normal population. Measurements on the non-drusen-containing optic disk of patients with unilateral drusen were taken to reflect the scleral canal size of the fellow, affected eye. For both trained (t = 6.642) and untrained (t = 4.274) observers, the average diameters of the non-drusen-containing optic disks of patients with unilateral drusen were significantly smaller than those of the o

Hereditary sclerocornea.

Sclerocornea is a primary anomaly in which scleralization of a peripheral part of the cornea, or the entire corneal tissue, occurs. In the peripheral type of sclerocornea, the affected area is vascularized with regular arcades of superficial scleral vessels. In total sclerocornea, the entire cornea is opaque and vascularized. To our knowledge, 97 cases of all types of sclerocornea have been reported in the world literature, either as a primary anomaly or in association with cornea plana. Peripheral sclerocornea in association with cornea plana was found in nine members of one family, in four o

Acute intraoperative choroidal effusion.

We studied five cases of acute choroidal effusion that occurred during intraocular surgery and one case of a localized choroidal hemorrhage. Acute intraoperative choroidal effusion is a poorly recognized surgical complication and may precede expulsive hemorrhage in many cases. The treatment is immediate closure of the wound, after which the intraocular pressure may rise to 80 mm Hg or more. However, the pressure will return to normal after 15 to 30 minutes and the area of effusion will remain localized. Scleral puncture to drain fluid is not indicated if the wound can be closed before there is

The pathophysiology of proliferative vitreoretinopathy in its management.

Cellular proliferation following retinal reattachment surgery frequently results in contraction and subsequent recurrent detachment of the retina, negating an initial successful reattachment. This process has been called by a variety of names, such as massive vitreous retraction, massive preretinal retraction, and, more recently, proliferative vitreoretinopathy. Although a good start has been made by the Retina Society to classify the various types of proliferative vitreoretinopathy, some modifications in the classification are required. The fundamental problem in the treatment of proliferativ

Ophthalmological complications in the sclerosing bone dysplasias.

The sclerosing bone dysplasias are a group of rare genetic disorders in which overgrowth of the cranio-facial skeleton causes a variety of ophthalmological complications. During the past decade more than 100 South Africans with conditions in this category have been investigated and their ocular manifestations have been appraised. Sclerosteosis was diagnosed in 45 persons. In 70% of the affected adults progressive thickening of the skull led to elevation of intracranial pressure and papilloedema. Proptosis and divergent strabismus was present in 25%, while excessive lacrimation due to bone obst

[Surgical treatment of the sequelae of Basedow's ophthalmopathy].

Functional and cosmetic ophthalmic complications of Graves' disease are exophthalmos, eyelid retraction and oculomotor disturbances. This paper describes and takes over the surgical procedures used when these complications are associated. In severe exophthalmos, we used an orbital decompression of the orbital floor sometimes combined with the medial wall by an anterior approach. In eyelid retraction, we perform a recession of the levator muscle with scleral graft implantation. In incomitant strabismus, surgery is usually limited to recessions of the involved muscles, most of the surgery is dir
