Exogenous ocular candidiasis associated with intravenous heroin abuse.

Seven young men developed disseminated candidiasis within 10 days of a single episode of intravenous heroin abuse. Sequential development of eye and skin lesions was noted in all cases. The bone or costal cartilage was involved in five. Ocular manifestations of candidiasis included episcleritis, chorioretinitis, and endophthalmitis. A presumptive diagnosis of candida chorioretinitis was established rapidly by culture of Candida albicans from involved skin and costal cartilage. Systemic therapy with amphotericin B plus 5-fluorocytosine resulted in cure of the episcleritis, chorioretinitis, oste

Experimental Salmonella-associated conjunctivitis in cats.

Cats were infected experimentally with Salmonella typhimurium via the conjunctiva. Clinical signs consisted of lacrimation, conjunctivitis, blepharospasm, prominent nictitating membrane and scleral injection. These signs were accompanied by an absolute neutrophilia and conjunctival smears indicative of moderate to severe suppurative inflammation. Ocular signs disappeared by day 6 postinfection. Salmonella typhimurium was cultured intermittently from the inoculated conjunctivae and rectal swabs through day 7. At necropsy, mesenteric and cervical lymph nodes were enlarged. Histopathological find

Orbital myositis simulating infectious cellulitis: report of two cases.

Two cases of orbital myositis simulating infectious cellulitis were reported, in which the diagnosis was made by CT scans, biopsy and laboratory findings of thyroid function. In both cases, ophthalmoscopy revealed partial occlusion of the central retinal vein and choroidal folds due to posterior scleritis. Histopathology of a biopsy specimen revealed infiltration of lymphocytes, eosinophils and plasma cells and perivasculitis which suggested an allergic origin of this disease.

Choroidal osteoma after intraocular inflammation.

A 12-year-old girl had blurred vision and pain behind her left eye for three months. An examination disclosed focal choroiditis and optic nerve edema. Her visual acuity was 20/30. Two similar previous episodes had been diagnosed as scleritis and uveitis and the patient was still on a regimen of 5 mg of prednisone per day and eyedrops containing prednisolone and phenylephrine. A pink choroidal mass superotemporal to the optic disk was negative for calcium by computed tomography and ultrasonography. The corticosteroids were discontinued. Three months later, the patient's visual acuity had improv

The treatment of necrotizing scleritis with an autogenous periosteal graft.

We used autogenous periosteum to reinforce scleral defects in two patients with necrotizing scleritis. This technique produced good anatomical results and maintained useful vision in each case. Autogenous periosteum offers advantages over other tissues as a tectonic graft for necrotizing scleritis, since it is easy to harvest and handle, it is strong and vascular, and is not subject to immunologic rejection.

[A new etiology of episcleritis: nephropathies with IgA and/or isolated C3 deposits].

Scleritis is a condition of the eye often associated with systemic diseases, allergy and infections. Nomoto et al. have reported the frequency of episcleritis in patients with IgA nephropathy (Berger's disease), a primary glomerulonephritis initially described by Berger in 1968. It is characterized by mesangial deposits of IgA, and is considered one of the most frequent nephritides in France. We report our study of the association of scleritis with glomerulonephritis (GN). Fifty-three GN patients were examined for the presence of scleritis. The exact type of nephropathy in each patient was det

Ocular complications of Wegener's granulomatosis.

140 patients with biopsy-proven Wegener's granulomatosis were examined during a 16-year period at the Mayo Clinic. Forty patients had diverse and explosive ophthalmic involvement, including conjunctivitis, episcleritis, scleritis, corneal ulceration, uveitis, retinal vasculitis, optic neuropathy, orbital mass, orbital cellulitis, and obstruction of the nasolacrimal duct. The multiple ophthalmic and systemic complications in these 40 patients and the importance of establishing the pathologic diagnosis are discussed. Treatment with corticosteroids and immunosuppressive agents has dramatically im

Budd-Chiari syndrome and renal failure in Behcet disease. Report of a case and review of the literature.

A case of Behcet syndrome associated with three rare complications is presented. The patient initially presented with asymmetric polyarticular arthritis, subcutaneous nodules, conjunctivitis, and episcleritis. Necrotizing vasculitis and renal failure subsequently evolved. Despite immunosuppressive therapy, Budd-Chiari syndrome with high grade hepatic obstruction developed. A Denver shunt procedure failed to alter the terminal course. At postmortem examination, skin lesions, carditis, hepatic vein thrombosis, gastric ulcerations, and focal proliferative glomerulonephritis were found. The immuno

Metastatic carcinoma masquerading as scleritis.

A 36-year-old white man abruptly developed painless visual loss in his left eye. Fundoscopic evaluation revealed a shallow left superotemporal retinal detachment with an underlying yellowish choroidal infiltrate. Fluorescein angiography disclosed indistinct choroidal leakage, and both B-scan ultrasonography and computed tomography demonstrated superotemporal thickening of the choroid and the sclera. The patient was diagnosed clinically as having a scleritis and an associated exudative retinal detachment. He was placed on a several months' course of systemic prednisone, and despite this regimen

Secondary vascular glaucoma.

Secondary vascular glaucoma is increased intraocular pressure due to venous obstruction in or outside the eye. Its main aetiological features are lesions of the vortex veins and the anterior ciliary veins and orbital and cavernous sinus obstruction; increase in orbital volume, tenonitis/scleritis posterior and idiopathically elevated episcleral venous pressure are described. In 8 out of 148 patients with exophthalmos of various aetiology an abnormal difference in intraocular pressure between the eyes was found, but only 3 of them (2%) had glaucoma; this frequency equals the glaucoma frequency

Conjunctival and episcleral injection in drug abuse.

A young man presented with severe conjunctival and episcleral injection in both eyes without other manifestations. On the second visit, persistent questioning revealed the intravenous use of an over-the-counter preparation with a stimulant effect. Unusual external ocular signs may be an indication of drug abuse.

Lacrimal gland and fossa masses: evaluation by computed tomography and A-mode echography.

Although masses of the lacrimal gland and fossa are accurately localized by CT, their appearances are often similar making differentiation between various pathological entities difficult. We examined 32 patients with CT and/or standardized A-mode echography (SAE) to determine if SAE added significant information to that supplied by CT alone. Other than five cases of dermoid cysts, which were fat containing, and one case of lacrimal gland pseudotumor, which had associated uveal scleral thickening and enhancement, there was no specific CT pattern that allowed a definite diagnosis. SAE reflectivi

Complications of herpes zoster ophthalmicus.

Of 86 patients with herpes zoster ophthalmicus seen at the Mayo Clinic, Rochester, Minn, from 1975 to 1980, 61 had some form of ocular involvement. Corneal disease was seen in 47, uveitis in 37, postherpetic neuralgia in 15, scleritis in three, and ocular motor palsies in three. No case of optic nerve or retinal involvement was found. Of serious concern were four patients with neurologic complications, including two with contralateral hemiplegia and two with segmental cerebral arteritis. Because the neurologic complications occur several months after the episode of herpes zoster ophthalmicus,

A survey on eye involvement among leprosy patients in The Netherlands.

A survey of 121 leprosy patients in the Netherlands showed eye involvement in 20% of the patients, rising to 27% if madarosis and the adnexae are included. Iris atrophy was the main lesion encountered in BL and LL patients. One typical lepromatous fundus lesion was seen. No bilateral blindness occurred. Two eyes had a visual acuity of 3/60 due to intractable scleritis and acute iritis in LL with ENL. Seven eyes of 5 patients had a visual acuity of 0.2 to 3/60, mainly due to iris atrophy in lepromatous leprosy. Prevalence of blindness in our series was low as compared with the literature.

Bilateral endogenous necrotizing scleritis due to Aspergillus oryzae.

A case of bilateral necrotizing scleritis due to Aspergillus oryzae is reported. The patient was a former addict of intravenous narcotics treated five years previously for meningitis due to the same organism. A seeding focus in the thoracic spine was eventually found. The patient responded well to combined local and systemic therapy with amphotericin B, flucytosine, and natamycin. This represents, to the best of our knowledge, both the first reported case of ocular disease due to this species of Aspergillus and of isolated scleral, nonintraocular involvement in endogenous oculomycosis.
