Exudative retinal detachment and oedema of the posterior retina are well-recognized clinical entities for which a cause is rarely found. Scleritis may cause both these clinical syndromes by a spread of inflammation from the sclera into the uveal tract. Although treatment may result in a rapid resolution of scleritis, retinal detachment and macular oedema persist and prove extremely resistant, even to high doses of systemic steroids. Recovery may be incomplete and may occur only after weeks or months of treatment.

A new treatment of a case of scleromalacia perforans, complicating rheumatoid arthritis is reported. To save the eye fascia lata was transplanted, preoperatively prednisolone and d-penicillamine was started, and this therapy was continued for one year thereafter. The effect was favorable, as pains vanished, and the grafted fascia remained whole. Cure of scleritis of the other eye hitherto unresponsive to local treatment was attained. Advantages of the combination of both immunosuppressive and surgical therapy is emphasized.

A case of unilateral paralimbal scleromalacia (so-called spontaneous scleral intercalary perforation) is presented. The otherwise healthy patient was folowed over a period of more than 10 years. Two years before the condition was diagnosed, the patient had an attack of scleritis in the same eye. The disease was complicated by keratitis and resulted in an eye with light perception only. Scleral tissue from the defect was examined histologically.

Thirty-four cases of uveitis, 26 anterior and 8 posterior, showed elevated intraocular pressure in 25 cases during treatment with local steroids as well as systemic steroids. The patients were treated with drugs to reduce the IOP, but this was not accomplished until the local steroids were stopped. Two patients had episcleritis and a secondary open angle glaucoma. Anterior uveitis was absent in three of the four eyes. The low facility of outflow suggests that increased episcleral venous pressure was not the mechanism of the glaucoma. The response of the intraocular pressure and, in one case, the outflow facility to steroid therapy suggests that inflammation of angle structures was the pathogenesis of the raised intraocular pressure.

Seventy-one patients with glaucoma needing surgical correction had either a peripheral iridectomy with a thermal sclerostomy or a trabeculectomy utilizing a modification of Watson's technique in which the scleral flap was closed tightly with sutures. The present study reveals that pupillary abnormalities are common after keratoplasty for keratoconus and that, in addition to the fixed dilated pupils which we have found in 7.8 per cent. of eyes, varying degrees of partially dilated pupil frequently occur after operation. Tension analysis along the retinal surface demonstrates the advantage of meridional buckles over limbal-parallel buckles. A cylindrical sponge generates tensional forces in the retina along the long axis of the buckle and perpendicular to it. Five cases of aortic incompetence and nodular seropositive rheumatoid arthritis are presented. Four cases underwent aortic valve replacement. Two of these had granulomatous involvement of the aortic cusps similar to subcutaneous rheumatoid nodules, and another showed a nonspecific fibrosis. Ocular complications occur in about 50% of cases of ophthalmic zoster. They include inflammatory reactions of the eyelid, conjunctivitis, scleritis, keratitis, iridocyclitis, secondary glaucoma, optic neuritis, internal ophthalmoplegia, ocular motor palsies and exophthalmos. Very dangerous complications are a concomitant facial paralysis and a neuroparalytic keratitis. Then a tarsorrhaphy should be done in time. An ophthalmologist should be consulted, when the side of the tip of the nose presents vesicles (Hutchinson's rule).

Choroidal folds were defined as folds in Bruch's membrane and the anterior choroid. A clinical, histopathological and experimental investigation was performed.Eleven cases with choroidal folds were described.
