Peripheral corneal ulcers, conjunctival ulcers, and scleritis after cataract surgery.

After undergoing uneventful cataract extractions, four patients developed an inflammatory reaction that included scleritis and peripheral corneal infiltrates and ulcers. Two of these patients also developed conjunctival ulcers. The inflammatory reaction in all four patients responded to topical corticosteroids.

Bronchocentric granulomatosis with eye involvement.

Bronchocentric granulomatosis is a recently defined entity within the "pulmonary angitis and granulomatosis" group of lung diseases. Unlike the other granulomatoses, extrapulmonary disease has not been encountered with bronchocentric granulomatosis. Therefore, we report a case demonstrating apparent association with scleritis, an uncommon eye disorder often accompanying underlying systemic disease. This introduces a potential source of diagnostic confusion with Wegener's granulomatosis, which can resemble bronchocentric granulomatosis clinically and radiographically, and which has a known asso

Uveal effusion attending scleritis posterior. A case report with A-scan and B-scan echograms.

A report is given of a patient with exudative ciliochoroidal and retinal detachment associated with posterior scleritis. The history and clinical findings were similar to earlier described cases and initially led to suspicion of a malignant melanoma. Ultrasonic examination performed by combination of standardized A-scan and contact B-scan technique allowed the diagnosis to be established and the regression of this condition to be followed. There was complete recovery after treatment with atrophine and topical steroids.

The complement fixing ability of putative circulating immune complexes in rheumatoid arthritis and its relationship to extra-articular disease.

The hypothesis that the pathogenicity of putative circulating immune complexes (CIC) in rheumatoid arthritis (RA) is related to their ability to fix complement was investigated. Three assays for CIC were employed; (a) the 125I C1q binding assay (C1q BA), (b) the C1q solid phase assay (C1q SP) and (c) the Raji cell assay (RCA). Evidence for hypercatabolism of complement was obtained by using a highly sensitive quantitative assay for C3d (a breakdown product of C3) by rocket immunoelectrophoresis. One hundred and fifty-two patients with classical or definite RA were studied; 54 had clinical evid

A probable case of ocular angiostrongyliasis in New Britain, Papua New Guinea.

Unilateral iritis, keratitis and episcleritis presented in a 45-year-old female Melanesian from New Britain, Papua New Guinea. A motile nematode was seen in the anterior vitreous but was not recovered. Inflammation resolved with topical corticosteroid and antibiotic treatment, but visual impairment persisted. She failed to return for repeat serology after discharge, and measurement of serum antibody levels to Angiostrongylus cantonensis in the single specimen obtained, using the direct and indirect immunofluorescent tests, showed a titre of 1:128. Ocular angiostrongyliasis is briefly reviewed

Ocular manifestations of inflammatory bowel disease.

The association between inflammation of the eyes and the intestine is not often recognized by ophthalmologists. We report two patients who developed peripheral corneal ulcers, episcleritis, and scleritis just prior to the onset of Crohn's disease. The severity of the eye disease paralleled that of the intestinal symptoms, and both conditions subsided after treatment with topical steroids, oral prednisone, oral sulfasalazine, and hydrocortisone retention enemas. Inflammatory bowel disease should always be included in the differential diagnosis of scleritis and uveitis, as the patient may be ben

Hypocomplementemic urticarial vasculitis: association with chronic obstructive pulmonary disease.

Since 1973, we have identified and collected follow-up data on 16 patients with hypocomplementemic urticarial vasculitis. Preliminary diagnostic criteria are the presence of typical urticarial skin lesions and low levels of serum complement (all components), plus two of the following: dermal venulitis, arthritis, glomerulo-nephritis, episcleritis or uveitis, recurrent abdominal pain, and C1q precipitin in plasma. Exclusions are systemic lupus erythematosus, mixed cryoglobulinemia, elevated antinuclear antibody titer, hereditary deficiency of a complement component or of C1 esterase inhibitor,

[Wegener's granulomatosis: ocular manifestations, diagnosis and therapy].

As a chronic, focal, necrotizing granulomatous angiitis Wegener's granulomatosis mainly affects the upper respiratory tract, the lungs and kidneys. Various other organs may be affected including te eyes. Ocular manifestation of Wegener's granulomatosis may result from granulomatous sinusitis that has proceeded into the orbit causing nasolacrimal duct obstruction and proptosis, and affecting eye muscles as well as the optic nerve. Another possible picture is presented by focal vasculitis in the form of conjunctivitis, episcleritis, scleritis, uveitis and granulomatous vasculitis of the retina a

Abnormal immune responses in the ocular presentation of Wiskott-Aldrich syndrome.

Wiskott-Aldrich syndrome is an x-linked disorder characterized by a triad of thrombocytopenia, eczema, and recurrent infections. Until recently, survival into the second decade was rare. In the last few years, a better understanding of the syndrome has been achieved, and earlier recognition and newer modes of treatment have improved life. These patients are living longer and ophthalmologic presentation reflects this change. We present three case histories of patients followed for the past 15 years who have had, in association with this syndrome, eczema of the eyelids, episcleritis, marginal ke

Scleral grafting in the management of ligneous conjunctivitis.

Ligneous conjunctivitis is a rare, usually bilateral, chronic membranous conjunctivitis, which begins acutely in childhood and persists for many years. In view of the poor results obtained with other forms of therapy in the past, the resection of the involved conjunctiva and tarsus with the grafting of donor sclera is offered as another alternative in the management of this distressing condition.

Uveal malignant melanoma associated with ocular and oculodermal melanocytosis.

Seventeen patients with ocular or oculodermal melanocytosis were identified among 1,250 Caucasian patients with uveal malignant melanoma. The uveal melanomas in these seventeen patients were similar in size, cell type, and tendency to metastasize to those occurring in reported populations without ocular or oculodermal melanocytosis. Clinical and histopathologic study of these 17 cases indicated that all of the affected eyes had episcleral and choroidal melanocytosis and that other ocular and periocular tissues (sclera, iris, conjunctiva, angle structures, and optic disc) were involved less com

Epibulbar lesions simulating extraocular extension of uveal melanomas.

Twelve patients who were referred to the Oncology Service of Wills Eye Hospital, Philadelphia, with a pigmented epibulbar lesion that clinically resembled malignant melanoma of the choroid with extraocular extension were found to have other ophthalmic entities. The most common conditions that simulated extraocular extension of uveal melanoma were conjunctival malignant melanomas, which accounted for four of the 12 pseudomelanomas. Three of the 12 patients had staphylomas or scleral ectasis. Two of the 12 had subconjunctival hematomas. Senile scleral plaques, a presumed cellular blue nevus, and

Ultrastructural studies of corneal collagen during the development of experimental scleritis in rabbits.

An active Arthus reaction induced in the rabbit cornea leads to scleral disease resembling that in humans. Collagen damage was located in the corneal stroma at the site of the immune reaction. In the electron microscope, some regions of corneal stroma, without loss of lamellar integrity, demonstrated the apparent swelling of individual, scattered collagen fibrils such that a helical arrangement of component microfibrils was evident. In areas showing more extensive damage, fibril fragmentation and solubilization was associated with the aggregation of fibrous banded material. These observations

Pseudomonas aeruginosa scleritis.

In two patients Pseudomonas aeruginosa scleral infection led to ocular perforation. In one patient, a scleral abscess was identified anteriorly. A scleral perforation occurred at a more posterior focus, but the eye was salvaged with minimal residual visual function. In the other patient, perforation at the corneoscleral limbus occurred after initial corneal improvement with antibiotic therapy; histopathologic examination of the enucleated globe disclosed an abscess extending from the corneoscleral limbus to the equator superiorly.

Ocular changes in limited forms of Wegener's granulomatosis.

Eight patients who had limited forms of Wegener's granulomatosis are described, with details of their pathology. Ocular pathological data were available for 2 of them. The condition carries a serious ocular risk; useful vision was lost in 6 out of 16 eyes (37%). An indolent but slowly progressive marginal keratitis and scleritis was a prominent feature in 4 patients and was helpful in suggesting the diagnosis. Limited froms of Wegener's granulomatosis carry a better prognosis and response to treatment than the classical disease.

Combined detachments in Wegener's granulomatosis.

We report a case with limited Wegener's granulomatosis in which the eye findings were the initial and only evidence of the disease when the patient was first seen. The patient was unusual in that he developed bilateral combined choroidal and retinal detachments and severe necrotising scleritis, which led to bilateral globe perforations and loss of sight. We recommend a trial of cytotoxic agents in progressive ocular inflammatory diseases unresponsive to other medications before there is irreversible loss of vision.
