In a clinical series of 350 eyes and a histopathological review of 100 globes, iridocyclitis was associated with scleritis in 42 and 63 per cent and choroiditis in 12 and 62 per cent respectively. A 29-year-old male patient with IgA nephropathy developed scleritis and myasthenia gravis during the follow-up period. No lymphocytic infiltration or atrophy in muscle fibers was observed in muscle biopsy specimens. However, an immunofluorescent study on the same specimens showed deposits of immunoglobulin A in muscular vessels. The development of scleritis and myasthenia gravis in a patient with IgA nephropathy suggests a common immunologic basis for these disorders.

A review of 373 patients with presumed choroidal nevi revealed 19 (5.1%) to have largely or totally amelanotic nevi. All patients but one with amelanotic nevi were white, and all were 48 years of age or older. Each lesion was located posterior to the equator, had a mean diameter of 3.2 mm, and was less than or equal to 1 mm in elevation. A 59-year-old black female with rheumatoid arthritis developed diffuse anterior scleritis with orbital and adnexal involvement. Computerized axial tomograhy (CAT) demonstrated the location and extent of the lesion. Four patients with relapsing polychondritis and eye disease were observed. Diagnosis was made on the basis of clinical symptoms of multifocal chondritis and episcleritis in one case. In the latter patient the disease started with slight visual impairment due to retinal hemorrhage and edema. Despite a relapsing course all patients eventually responded well to topical and systemic corticosteroids. Immunopathologic events are discussed.

We studied 13 patients with Terrien's marginal degeneration of the cornea. Six of these 13 patients had recurrent, disabling, episodic inflammation that was the most dominant feature of the disease. Severe pain and an associated episcleritis or superficial scleritis was present in all the patients during these attacks. These findings suggested that an inflammatory spectrum of Terrien's disease may exist.

Thirty-nine patients took part in a double-blind, between-patient clinical trial to compare clobetasone butyrate, betamethasone phosphate, and placebo eye drops in the treatment of episcleritis. Although from the symptom scores the patients given placebo appeared to do as well as the other patients in the first week of treatment, they did significantly less well after this time. Clobetasone butyrate and betamethasone phosphate eye drops seemed to be equally effective in the treatment of this disease.

Serum samples from 91 uveitis patients and controls were tested by the enzyme-linked immunosorbent assay technique to determine their relative antibody titers to bovine retinal S antigen and to a detergent extract of saline-washed bovine retina, or P antigen. One hundred sixty consecutive patients with the Marfan syndrome were reviewed for ocular, cardiovascular, and skeletal abnormalities, and were graded by severity. The most striking ocular abnormality was enlargement of the globe, presumably caused by scleral stretching. Staphylomata were not a feature of any of the patients seen, nor was keratoconus. Of 92 enucleated eyes with scleral inflammation and 114 eyes from 81 patients with scleritis, 45 (49%) and 20 (18%), respectively, showed evidence of increased intraocular pressure. Damage to the trabecular meshwork of iridocyclitis, overlying corneoscleral inflammation, or peripheral anterior synechiae was the most common cause of increased intraocular pressure. A case of serous retinal detachment and episcleritis in a 26 years old woman is reported. Follow-up studies on 113 patients with various types of primary glomerular diseases were performed for one to 33 months to determine the clinical spectrum of primary glomerulonephritis. Of those studied, six patients exhibited scleritis. All of these six patients with scleritis were identified as having "IgA nephropathy." None of the patients other than those with IgA nephropathy showed scleritis during the study period. It is suggested that some autoimmune mechanisms similar to the manifestation of IgA nephropathy may be involved in the development of scleritis.

A 12-year-old girl developed posterior scleritis and many detachments of the retinal pigment epithelium. Computed axial tomographic findings, reported in posterior scleritis for the first time, disclosed thickening of the posterior sclera.
