Recurrent Episcleritis Revealing Pulmonary Tuberculosis in an Adolescent: A Case Report and Literature Review.

INTRODUCTION: Tuberculosis (TB) can affect the eye through direct infection or immune mechanisms. While ocular TB most often presents as uveitis or scleritis, episcleritis is an uncommon manifestation and can be mistaken for an idiopathic or autoimmune disease. CASE PRESENTATIONS: A 17-year-old female patient presented at our hospital with recurrent episcleritis. The first two flare-ups were mild and responded well to conservative treatment; therefore, no further investigation was pursued at the time. However, during the third episode, routine baseline workup revealed a mildly elevated erythr

Atypical Cogan's Syndrome Is Common among Japanese Patients: A Case Report and Literature Review in Japan.

We report the case of a 55-year-old Japanese woman who presented with fever, headache, vertigo, sensorineural hearing loss, and scleritis, leading to a diagnosis of Cogan's syndrome (CS). Her hearing acuity recovered to previous levels after high-dose glucocorticoid treatment, despite seven weeks having passed since the onset. We reviewed 30 adult cases of Japanese CS using PubMed and Ichushi-Web. We identified several clinical features of Japanese patients with CS, including an older age of onset, less interstitial keratitis, more frequent fever and headache, elevated inflammatory marker leve

Case Report: Early-onset VEXAS syndrome with recurrent pulmonary inflammation and myelodysplasia: a diagnostic and therapeutic challenge.

BACKGROUND: VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a newly recognized adult-onset autoinflammatory disorder caused by somatic mutations in the UBA1 gene. It typically presents in older males with systemic inflammation, hematologic abnormalities, and a relapsing, treatment-refractory course. Reports in younger patients remain rare, and pulmonary involvement is often misdiagnosed as infection. CASE PRESENTATION: We present a 30-year-old man with a two-year history of recurrent scleritis, fever, cough, auricular chondritis, rash, and transfusion-dependent ma

Crizotinib-associated nodular scleritis in an adolescent.

We report the case of a 17-year-old girl with history of anaplastic large-cell lymphoma on crizotinib therapy who presented with pain and blurred vision in the left eye. Examination demonstrated nodular non-necrotizing anterior scleritis in the left eye. The patient initially improved with oral glucocorticoid treatment until she developed nodular non-necrotizing anterior scleritis in the right eye during taper of this medication. Discontinuation of crizotinib allowed the patient to taper off glucocorticoid treatment, with subsequent resolution of scleritis.
