Oral amoxicillin-clavulanate: an alternative treatment to refractory Nocardia keratitis-a case report.

BACKGROUND: Bacterial keratitis (bacterial corneal infection) is a serious, vision-threatening disease caused by a wide range of bacterial species. The gram-positive bacterium Nocardia spp. is increasingly recognized in keratitis, yet is particularly challenging to treat due to delayed diagnosis and limited efficacy of current ophthalmic therapeutics. Antimicrobial resistance among clinical Nocardia isolates is also rising, further complicating patient outcomes. Herein, we report a case of Nocardia amikacinitolerans keratitis, resistant to amikacin, ciprofloxacin, moxifloxacin, clarithromycin,

Periocular necrotizing sweet syndrome - a case series and review of literature.

Sweet syndrome (SS), or acute neutrophilic dermatosis, is a dermatologic condition characterized by fever and neutrophilia with tender papular or plaque-like eruptions; diagnosis is confirmed histologically and the disease typically responds to systemic corticosteroids. Ocular associations include conjunctivitis, peripheral ulcerative keratitis, scleritis, uveitis, glaucoma, and retinal detachment, with occasional periocular involvement. A rare clinical variant, necrotizing Sweet's syndrome (NSS), closely mimics necrotizing fasciitis (NF). While NF requires urgent, extensive surgical debrideme

Adjunct rose Bengal photodynamic antimicrobial therapy for refractory Pseudomonas aeruginosa infectious necrotizing scleritis after scleral buckling: a case report.

A 34-year-old man developed rapidly progressive infectious necrotizing scleritis following blunt ocular trauma and subsequent scleral buckle placement for rhegmatogenous retinal detachment. At presentation, best-corrected visual acuity was light perception, with conjunctival lysis, multifocal scleral thinning with uveal exposure, purulent discharge, and marked ocular inflammation. Microbiologic cultures grew Pseudomonas aeruginosa. Despite targeted topical and systemic antimicrobial therapy and surgical debridement with partial buckle removal and tectonic reconstruction, scleral necrosis conti

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Nanomaterials for the Treatment of Contamination by Nosocomial Pathogens in Intensive Care Units.

INTRODUCTION: Posterior subtenon injection of triamcinolone acetonide (PSTA) is commonly done to treat refractory cases of macular edema. Complications may arise from the procedure as well as from the use of the periocular steroid medications. Side effects include subconjunctival hemorrhage, progression of cataract, scleral perforation (resulting in subretinal, subhyaloid, or intravitreal injection of the drug), retinal detachment, ptosis, orbital fat prolapse, orbital abscess, infectious scleritis, ocular hypertension, and scleral abscess. Here we describe a case of inadvertent subretinal tri

Junctional proliferative vitreoretinopathy in rhegmatogenous retinal detachment secondary to viral retinitis in HIV.

INTRODUCTION: Rhegmatogenous retinal detachment (RRD) is a vision-threatening complication of viral retinitis, at times compounded by proliferative vitreoretinopathy (PVR) at the junction of the necrotic and unaffected retina. METHODS: Retrospective review of eyes with 'junctional PVR' in RRD after viral retinitis. We define junctional PVR as presence of stiffness, transverse traction, epiretinal membrane and/or pucker at the junction of normal and necrotic retina. RESULTS: Five eyes of four patients with a mean age at presentation of 46.5 ± 10.96 years were included. Immunosuppression due t

Use of cyclosporine A 0.05% ophthalmic emulsion for a distinctive intrapalpebral phenotype of anterior scleritis.

Chronic scleritis poses a diagnostic and therapeutic challenge with multiple mimickers, underlying autoimmune conditions, diverse presentations and variable course. Treatment for mild forms includes topical corticosteroids and non-steroidal anti-inflammatory drugs (NSAIDs). For long-term control in patients with inadequate responses to steroids and NSAIDs, immunomodulators are employed. While oral cyclosporine has been used for inflammatory ocular conditions, research on topical cyclosporine for scleritis is limited. We present two cases: a man in his early 60s and a women in her late 20s, bot

Infectious scleritis: a review of etiologies, clinical features, and management strategies.

Infectious scleritis is a severe and potentially vision-threatening inflammation of the sclera caused by microbial invasion. Unlike autoimmune scleritis, infectious scleritis is less common with a prevalence of 5%-10% of all cases of scleritis. The initial clinical presentation may often resemble autoimmune scleritis, thus resulting in a delayed diagnosis and poorer outcomes. A high level of suspicion is required in such cases and risk factors such as with a history of trauma or surgery and worsening or refractory to immunosuppressive medications as these may well be infectious. While in estab
