Surgically Induced Scleral Necrosis: A Case Report.

Surgically induced necrotizing scleritis (SINS) is a scleral ulceration that can occur days to years after various ocular surgeries. It is an uncommon complication that may lead to scleral perforation. We report the case of a 45-year-old diabetic male patient who underwent vitreoretinal surgery on his left eye for the treatment of a complicated diabetic retinal detachment. He presented three months after the procedure with extensive scleromalacia pre-perforans. This case highlights the risk of SINS following ocular surgery. Therefore, patients at high risk of developing SINS should be identifi

Ocular Ultrasound as a Key to Diagnosing Uveitis-Masked Syndromes: Tips and Tricks.

Background and Objectives: Uveitis-masked syndromes or masquerade syndromes (UMSs) are a group of ocular conditions with several systemic underlying causes, malignant or nonmalignant, that mimic the inflammatory status of the uvea. They are often difficult to detect and diagnose with traditional techniques, such as ophthalmic exams. Ocular B (bidimensional)-ultrasound (OBU) is a non-invasive, repeatable, rapid ultrasound method effective in indirect signs that lead back to systemic diseases. It is comparable in effectiveness with other imaging tools. The cause of UMSs can often be serious, and

Posterior scleritis misdiagnosed as central serous chorioretinopathy: Case report.

This case report describes a 39-year-old male presenting with serous retinal detachment (SRD) who was initially misdiagnosed with acute central serous chorioretinopathy (CSC). CSC is typically a self-limiting condition, but recurrence occurs in approximately 30-50 % of patients depending on risk factors and disease chronicity. He received antibiotics for suspected infection, but his symptoms persisted without corticosteroid use. After referral to our hospital, posterior scleritis was confirmed through inflammation signs, multimodal imaging (showing optic disc edema, extending SRD, thickened ey

Ocular presentation of ischaemic optic neuropathy and recurrent episcleritis secondary to relapsing polychondritis.

Relapsing polychondritis is a rare autoimmune-driven cause of cartilaginous inflammation with multi-systemic impact and disease. We present an acute presentation of optic neuropathy on an eight-month background of wandering oligoarthritis, auricular chondritis and recurrent episcleritis. Prompt courses of oral steroids and ongoing immunosuppression with cyclophosphamide, methotrexate and anti-TNF treatment have ensured a swift recovery in ocular and systemic presenting signs and symptoms. This case highlights the clinical workup, multidisciplinary nature and patient factors that led to a promp

The sequential therapy for non-infectious scleritis: A case report.

Scleritis results from an inflammatory response in the sclera and can be associated with a variety of infectious and non-infectious ocular and systemic diseases. It can cause vision loss, uveitis, high intraocular pressure and other complications, resulting in poor prognosis. There are no reports of non-infectious scleritis in children. Herein, we describe a case of sequential tocilizumab treatment on tofacitinib in a child with non-infectious scleritis. Extensive examinations ruled out the cause of infection, and the previous conventional glucocorticoid treatment was effective, but the diseas
