Anterior segment fluorescein angiography in the diagnosis of scleral inflammation.

Anterior segment fluorescein angiography was performed on 135 patients with scleritis and episcleritis and on several normal subjects. It was confirmed that the coats of the eye anterior to the extraocular muscles derive their blood supply from perforating ciliary vessels. Characteristic patterns were detected which could distinguish the various forms of scleritis and episcleritis. Fluorescein angiography was found to be particularly useful in the detection of those patients who have early, but not clinically recognisable, necrotizing scleritis. The fluorescein angiographic appearances have be

Lacrimal gland enlargement as one of the ocular manifestations of Wegener's granulomatosis.

Wegener's granulomatosis is characterized by chronic, focal, necrotizing and granulomatous vasculitis. The localized form involves mainly the upper or lower respiratory tracts, or both. In the commoner generalized form, the kidneys are also affected. Literature findings suggest that ocular manifestations are a predominating sign in about 30% of cases. Ocular findings may include conjunctivitis, keratitis and sclerouveitis, pseudotumor of the orbit or proptosis. We report on a patient who showed bilateral episcleritis, keratitis, nasolacrimal duct obstruction and enlargement of the lacrimal gla

Chronic ophthalmic Wegener's granulomatosis.

A case of Wegener's granulomatosis is reported which manifested episcleritis followed by scleritis, and followed in turn by separate episodes of right and left orbital pseudotumor spanning 15 years prior to respiratory tract disease. The unusually protracted progression of symptoms in this case may be explained on the basis of the histopathological findings. Areas of fibrinoid necrosis of connective tissue occurred alone or alternated with areas of nongranulomatous fibrinoid necrosis of blood vessel walls. Granulomatours vasculitis was not observed. This histopathological picture is pathognomi

Scleritis and the spectrum of external inflammatory eye disease.

Scleritis, episcleritis and Mooren's ulcer form a poorly understood spectrum of inflammatory syndromes involving the ocular surface. Their pathogenesis is thought to be a vasculitis and may be related to immune complex deposition, as these syndromes are often associated with systemic diseases which are assumed to be vasculitic in aetiology. Scleritis and Mooren's ulcer are amongst the most severe external inflammatory diseases, often causing extensive damage to the sclera and cornea. They require aggressive investigation and management, often including systemic anti-inflammatory therapy. Syste

Ocular findings in leprosy in the United States.

We examined 55 clinic patients with biopsy-proven Hansen's disease and catalogued the ocular findings. Forty patients had lepromatous disease and 15 had tuberculoid disease. Visual acuity was 20/40 or better in 52 patients. Examination of pupillary size in all cases and after dilation with cocaine and hydroxyamphetamine in 15 cases showed decreased sympathetic innervation to the iris dilator muscle in seven cases. Ocular inflammation was uncommon: there was one case of active interstitial keratitis, three of episcleritis, and four of iritis. Although 41 patients had ocular problems attributabl

Terminal complement component deficiencies and rheumatic disease: development of a rheumatic syndrome and anticomplementary activity in a patient with complete C6 deficiency.

Hereditary deficiencies of early complement components have usually been associated with the development of rheumatic diseases like systemic lupus erythematosus (SLE), while terminal component deficiency is well known to predispose to recurrent neisserial infection. In contrast, only recently have patients been reported with rheumatic disease and hereditary deficiency of a terminal component. The clinical syndrome in these patients has been characterised as 'SLE-like'. We describe here a third patient with complete C6 deficiency and a systemic rheumatic illness characterised by fever, anaemia,

Ophthalmic features of necrobiotic xanthogranuloma with paraproteinemia.

Necrobiotic xanthogranuloma of the skin is associated with paraproteinemia and, often, with plasma proliferative disorders, including multiple myeloma. Other commonly recognized systemic abnormalities include hepatosplenomegaly, a highly increased erythrocyte sedimentation rate, and leukopenia. Fifteen of 16 patients (seven men and nine women with a mean age of 54 years) with this condition had ophthalmic manifestations. Thirteen patients had lesions affecting the skin of the eyelids and periorbital tissue; on casual examination these lesions resembled plane xanthoma. Unlike plane xanthoma, ho

Optic neuritis in inflammatory bowel disease.

It is not unusual for various ocular diseases to be associated with inflammatory bowel disease. Ocular involvement includes the inflammatory entities of keratitis, conjunctivitis, episcleritis, orbital pseudotumor, iritis and neuroretinitis , as well as those states in which the etiology remains undefined, i.e., scleromalacia perforans, peripheral corneal ulceration and furrow, retinal artery occlusion, and optic neuropathy. We report five cases of optic neuropathy associated with ulcerative colitis to emphasize that this may be the sole ocular manifestation of inflammatory bowel disease.

Cogan's syndrome: a treatable cause of neurosensory deafness.

Cogan's syndrome, nonsyphilitic interstitial keratitis associated with vestibuloauditory deficits, can vary in its presentation. This paper describes an unusual case in which deafness developed while the patient was receiving topical therapy with prednisolone and homatropine for mild bilateral iritis and right-sided nodular episcleritis. The 10-year-old boy's hearing improved dramatically with high-dose systemic steroid therapy. The importance of being aware that deafness may occur in children undergoing treatment for "routine" iritis is emphasized.

Adverse reactions to virgin silk sutures in cataract surgery.

During the past 3 years, 12 patients (14 eyes) were encountered with severe reactions to virgin silk sutures used in cataract surgery. These reactions included nodular episcleritis, peripheral corneal ulceration, and wound necrosis with dehiscence, sometimes resulting in endophthalmitis or epithelial downgrowth. Conjunctival and scleral histopathologic studies in four eyes showed acute and chronic inflammation with multinucleated giant cells. Successful treatment included removal of the virgin silk sutures, resuturing with 10-0 nylon sutures, securing the necrotic tissue with cyanoacrylate adh

Melting corneas with collapsing nose.

A 31-year-old white woman with relapsing polychondritis presented with profound, diffuse episcleritis, marked conjunctival chemosis, and corneal-limbal inflammation and melting, which was refractory to corticosteroid treatment. With the initiation of chlorambucil the inflammation subsided, leaving an almost normal slit-lamp picture. The associated findings of nasal cartilage and pinna cartilage inflammation helped to establish the diagnosis of relapsing polychondritis in this patient.

IgA nephropathy: dimeric IgA-secreting cells are present in episcleral infiltrate.

Recent publications have reported a frequent association between IgA nephropathy and episcleritis. In this article, the authors report on the immunohistologic study of an episcleral biopsy obtained in a female patient with Berger's disease and frequent episodes of episcleritis. Numerous dimeric-IgA-secreting cells were demonstrated in the episcleral inflammatory infiltrate. It also should be mentioned that microhematuria often occurred within a few days after the beginning of episcleritis in this patient. These results suggest the participation of ocular mucosal immunity in some cases of IgA n

Fluorometholone acetate: clinical evaluation in the treatment of external ocular inflammation.

Separate, randomized, double-masked clinical studies were conducted to determine the therapeutic efficacy of fluorometholone acetate, a new ophthalmic corticosteroid formulation, in suppressing external ocular inflammation (conjunctivitis, episcleritis, scleritis). Fluorometholone acetate 0.1% was compared to fluorometholone 0.1%, and the acetate formulation was found to be significantly (p = 0.03) more effective. Fluorometholone acetate 0.1% also was compared to prednisolone acetate 1.0%; there was no significant difference (p = 0.49) between these two corticosteroids in their ability to supp

Exogenous ocular candidiasis associated with intravenous heroin abuse.

Seven young men developed disseminated candidiasis within 10 days of a single episode of intravenous heroin abuse. Sequential development of eye and skin lesions was noted in all cases. The bone or costal cartilage was involved in five. Ocular manifestations of candidiasis included episcleritis, chorioretinitis, and endophthalmitis. A presumptive diagnosis of candida chorioretinitis was established rapidly by culture of Candida albicans from involved skin and costal cartilage. Systemic therapy with amphotericin B plus 5-fluorocytosine resulted in cure of the episcleritis, chorioretinitis, oste

[A new etiology of episcleritis: nephropathies with IgA and/or isolated C3 deposits].

Scleritis is a condition of the eye often associated with systemic diseases, allergy and infections. Nomoto et al. have reported the frequency of episcleritis in patients with IgA nephropathy (Berger's disease), a primary glomerulonephritis initially described by Berger in 1968. It is characterized by mesangial deposits of IgA, and is considered one of the most frequent nephritides in France. We report our study of the association of scleritis with glomerulonephritis (GN). Fifty-three GN patients were examined for the presence of scleritis. The exact type of nephropathy in each patient was det

Ocular complications of Wegener's granulomatosis.

140 patients with biopsy-proven Wegener's granulomatosis were examined during a 16-year period at the Mayo Clinic. Forty patients had diverse and explosive ophthalmic involvement, including conjunctivitis, episcleritis, scleritis, corneal ulceration, uveitis, retinal vasculitis, optic neuropathy, orbital mass, orbital cellulitis, and obstruction of the nasolacrimal duct. The multiple ophthalmic and systemic complications in these 40 patients and the importance of establishing the pathologic diagnosis are discussed. Treatment with corticosteroids and immunosuppressive agents has dramatically im
