Budd-Chiari syndrome and renal failure in Behcet disease. Report of a case and review of the literature.

A case of Behcet syndrome associated with three rare complications is presented. The patient initially presented with asymmetric polyarticular arthritis, subcutaneous nodules, conjunctivitis, and episcleritis. Necrotizing vasculitis and renal failure subsequently evolved. Despite immunosuppressive therapy, Budd-Chiari syndrome with high grade hepatic obstruction developed. A Denver shunt procedure failed to alter the terminal course. At postmortem examination, skin lesions, carditis, hepatic vein thrombosis, gastric ulcerations, and focal proliferative glomerulonephritis were found. The immuno

A probable case of ocular angiostrongyliasis in New Britain, Papua New Guinea.

Unilateral iritis, keratitis and episcleritis presented in a 45-year-old female Melanesian from New Britain, Papua New Guinea. A motile nematode was seen in the anterior vitreous but was not recovered. Inflammation resolved with topical corticosteroid and antibiotic treatment, but visual impairment persisted. She failed to return for repeat serology after discharge, and measurement of serum antibody levels to Angiostrongylus cantonensis in the single specimen obtained, using the direct and indirect immunofluorescent tests, showed a titre of 1:128. Ocular angiostrongyliasis is briefly reviewed

Ocular manifestations of inflammatory bowel disease.

The association between inflammation of the eyes and the intestine is not often recognized by ophthalmologists. We report two patients who developed peripheral corneal ulcers, episcleritis, and scleritis just prior to the onset of Crohn's disease. The severity of the eye disease paralleled that of the intestinal symptoms, and both conditions subsided after treatment with topical steroids, oral prednisone, oral sulfasalazine, and hydrocortisone retention enemas. Inflammatory bowel disease should always be included in the differential diagnosis of scleritis and uveitis, as the patient may be ben

Hypocomplementemic urticarial vasculitis: association with chronic obstructive pulmonary disease.

Since 1973, we have identified and collected follow-up data on 16 patients with hypocomplementemic urticarial vasculitis. Preliminary diagnostic criteria are the presence of typical urticarial skin lesions and low levels of serum complement (all components), plus two of the following: dermal venulitis, arthritis, glomerulo-nephritis, episcleritis or uveitis, recurrent abdominal pain, and C1q precipitin in plasma. Exclusions are systemic lupus erythematosus, mixed cryoglobulinemia, elevated antinuclear antibody titer, hereditary deficiency of a complement component or of C1 esterase inhibitor,

[Wegener's granulomatosis: ocular manifestations, diagnosis and therapy].

As a chronic, focal, necrotizing granulomatous angiitis Wegener's granulomatosis mainly affects the upper respiratory tract, the lungs and kidneys. Various other organs may be affected including te eyes. Ocular manifestation of Wegener's granulomatosis may result from granulomatous sinusitis that has proceeded into the orbit causing nasolacrimal duct obstruction and proptosis, and affecting eye muscles as well as the optic nerve. Another possible picture is presented by focal vasculitis in the form of conjunctivitis, episcleritis, scleritis, uveitis and granulomatous vasculitis of the retina a

Abnormal immune responses in the ocular presentation of Wiskott-Aldrich syndrome.

Wiskott-Aldrich syndrome is an x-linked disorder characterized by a triad of thrombocytopenia, eczema, and recurrent infections. Until recently, survival into the second decade was rare. In the last few years, a better understanding of the syndrome has been achieved, and earlier recognition and newer modes of treatment have improved life. These patients are living longer and ophthalmologic presentation reflects this change. We present three case histories of patients followed for the past 15 years who have had, in association with this syndrome, eczema of the eyelids, episcleritis, marginal ke

[Eye disease in cases of chronic atrophic polychondritis (author's transl)].

Four patients with relapsing polychondritis and eye disease were observed. Diagnosis was made on the basis of clinical symptoms of multifocal chondritis and episcleritis in one case. In the latter patient the disease started with slight visual impairment due to retinal hemorrhage and edema. Despite a relapsing course all patients eventually responded well to topical and systemic corticosteroids. Immunopathologic events are discussed.

Inflammatory Terrien's marginal corneal disease.

We studied 13 patients with Terrien's marginal degeneration of the cornea. Six of these 13 patients had recurrent, disabling, episodic inflammation that was the most dominant feature of the disease. Severe pain and an associated episcleritis or superficial scleritis was present in all the patients during these attacks. These findings suggested that an inflammatory spectrum of Terrien's disease may exist.

Clinical evaluation of clobetasone butyrate eye drops in episcleritis.

Thirty-nine patients took part in a double-blind, between-patient clinical trial to compare clobetasone butyrate, betamethasone phosphate, and placebo eye drops in the treatment of episcleritis. Although from the symptom scores the patients given placebo appeared to do as well as the other patients in the first week of treatment, they did significantly less well after this time. Clobetasone butyrate and betamethasone phosphate eye drops seemed to be equally effective in the treatment of this disease.

Serum antibody levels of uveitis patients to bovine retinal antigens.

Serum samples from 91 uveitis patients and controls were tested by the enzyme-linked immunosorbent assay technique to determine their relative antibody titers to bovine retinal S antigen and to a detergent extract of saline-washed bovine retina, or P antigen. Only those patients whose uveitis fell into the categories of toxoplasmosis, iritis, herpes keratouveitis, pars planitis, sarcoidosis. Behçet's syndrome, and Vogt-Koyamagi-Harada syndrome are presented in this study. Significantly elevated titers (P less than 0.05) of antibody to S antigen were found in the diagnostic categories of iritis

[Relapsing corticodependent posterior pole serous retinal detachment (author's transl)].

A case of serous retinal detachment and episcleritis in a 26 years old woman is reported. The detachment, always, unilateral, disappeared in a few days with corticotherapy, but relapsed eight times in a year when steroids were discontinued (4 times in R.E. and 4 in L.E.). No systemic pathology was found. The detiology probably involved a localized auto immune process. Based on the angiographic findings typical of a central serous chorioretinopathy) and the evolution (similar to Harada's disease) this case supports the hypothesis that relates both diseases as belonging in to the same nosologic

Traumatic episcleritis following phosphorus 32 testing.

Five eyes containing choroidal melanomas were found, on histopathologic examination, to have traumatic episcleritis with foreign-body granulomas and scleral edema underlying the area of tumor. This reaction was clearly correlated with phosphorus 32 testing that had been performed one to four weeks earlier. In all cases, enucleation had been delayed for logistic reasons. Invasion of the sclera or intrascleral canals was found in each case, with extension to the episclera in two of three small melanomas. Although the cases are few, this degree of invasion was greater than that seen in our other

Clinical tear analysis using laser diffraction.

A diagnostic method has been developed to analyze tear specimens from patients with red eyes, using the technique of laser diffraction. After proper tear sample preparation, presence of a diffraction halo produced by the tear specimen indicates presence of an external ocular inflammation and helps rule out such conditions as glaucoma, iritis and episcleritis. Measurement of the size of the halo pattern may also be used to give information as to the concentration of cells in the tear sample.

[Clinico-serologic study of humans with diseases epidemiologically related to halprowiosis (chlamydiosis) of farm animals].

A toxico-allergo-asthenic syndrome, primarily detected as a group disease (16 cases) affecting the workers of a cattle-ranch with the record of halproviosis (chlamydiosis) of agricultural animals, is described. A rise in temperature, sickness, vomiting, headache, loss of appetite, pain in the jaws and the joints, conjunctivitis and episcleritis, frequent urination, weakness, irritability, emaciation appeared to be its most characteristic symptoms. The halprovial nature of this syndrome was revealed on the basis of differential clinico-laboratory studies, serological, epizootic and epidemiologi

Progressive hemifacial atrophy (Parry-Romberg syndrome): report of two cases.

We describe 2 cases of progressive hemifacial atrophy (Parry-Romberg syndrome) with typical facial and ocular involvement. Ocular manifestations in these 2 cases include enophthalmos, due to fat atrophy, decreased corneal sensitivity, episcleritis, and band keratopathy. The slowly progressive changes were documented in one case over a 40 year period. Both cases were unilateral with limitation of the lesions at the midline.

Ocular inflammation and hemorrhage as initial manifestations of uveal malignant melanoma. Incidence and prognosis.

A consecutive series of 450 eyes enucleated because of a malignant melanoma of the choroid or ciliary body was reviewed in an effort to establish the incidence of ocular inflammation or intraocular hemorrhage as the initial clinical manifestation. Thirty-five patients had these clinical signs initially. Twenty-two (4.9%) initially had ocular inflammations. Eight of these had an episcleritis, and the remaining 14 had some form of uveitis, endophthalmitis, or panophthalmitis. Five of those with episcleritis had tumors in the ciliary body, and all six patients who developed panophthalmitis had ne

Potassium iodide sensitivity in four patients with hypocomplementemic vasculitis.

During metabolism studies of radiolabeled proteins in 126 participants four patients were suspected of being sensitive to potassium iodide (Kl) because they repeatedly developed urticaria and other symptoms after Kl administration. Two of the four patients suspected of Kl sensitivity and 10 control patients were orally challenged with Kl to document and characterize Kl sensitivity and to evaluate the possible association(s) of Kl sensitivity with urticaria, hypocomplementemia, and vasculitis. The Kl challenges in the two sensitive patients precipitated urticaria, angioedema, polymyalgias, conj

[Eye a symptoms in relapsing polychondritis (author's transl)].

This is a rare auto-immune illness with necrosis of cartilages. Palpebral oedema, conjunctivitis, episcleritis, iridocyclitis and proptosis belong to this syndrom. But the cataracts often described are iatrogenic, due to the intense corticotherapy required. A report is given of one personal observation and discussion of new therapeutical possibilities.
