A non-surgical method for subretinal delivery by trans-scleral microneedle injection.

Novel therapeutics have emerged for treating neurodegenerative eye diseases but are limited by non-optimal methods of ocular administration. Subretinal injection is the preferred method of delivery for retinal gene and stem-cell therapies, but its invasive and complex surgical procedure is a major limiting factor in clinical investigations and practice. Here, we engineered a novel trans-scleral injection technique to safely administer to the subretinal space in a simple, non-surgical, and minimally invasive procedure. Subretinal injection using this technique in rodents and rabbits took <1 min

Immunomodulatory Therapy Discontinuation for Reasons Other Than Efficacy in the Treatment of Ocular Inflammation.

PURPOSE: Immunomodulatory therapy (IMT) is commonly used in the treatment of chronic ocular inflammation. While there is substantial data on their efficacy, comparatively little data is available on their relative tolerability and safety. Here, we compare the discontinuation rates for reasons other than efficacy for both conventional IMT (cIMT) - methotrexate, mycophenolate, and azathioprine - and biologic IMT (bIMT) - adalimumab and infliximab - in the treatment of ocular inflammation. METHODS: A retrospective cohort study was performed for patients treated with IMT for ocular inflammation b

Efficacy of mini-scleral lenses in visual rehabilitation for corneal disorders: a prospective study.

BACKGROUND: Corneal irregularities are a significant cause of visual impairment, impacting patients' quality of life. Conventional treatment with spectacles and/or contact lenses fail to provide adequate rehabilitation in these cases. This study aimed to evaluate the efficacy and patient satisfaction with use of mini-scleral devices (MSDs) in patients with various corneal disorders including keratoconus, corneal scarring, post-corneal transplant, and post-refractive surgery. METHODS: This analytical, observational, prospective study included 40 eyes of 32 patients with corneal surface irregul

Microindentation reveals softening of the equatorial and anterior sclera during early myopia development in tree shrew eyes.

Myopia has reached epidemic levels worldwide and significantly increases the risk for blinding diseases such as glaucoma, making it a pressing global health concern. Myopia is commonly associated with biomechanical weakening and remodeling of the sclera, resulting in an excessively elongated eye relative to its optical system. The exact regions of scleral remodeling and tissue softening remain unclear. The purpose of this study was to establish a microindentation testing approach for spatial mapping of scleral stiffness and localization of softened regions. Microindentation tests were performe

0.01% atropine combined with carteolol hydrochloride can inhibit the scleral remodeling in guinea pigs with form-deprivation myopia.

Myopia is the most widespread refractive error caused by an increase in the axial length (AL) of the eyeball, and is also a major risk factor for other blinding eye diseases, seriously endangering human health and quality of life. Investigating practical methods to control the progression of myopia is therefore crucial. The only medication that has been shown to effectively delay the progression of myopia over an extended period of time is atropine. Although they have more adverse effects, atropine eye drops with a high concentration work best to correct myopia. Atropine at low concentrations

Trojan Horse-Inspired Biomimetic Lipoprotein Nanocarrier for Noninvasive Anti-VEGF Therapy of Ocular Fundus Neovascularization.

Pathological neovascularization is a major cause of vision loss in various ischemic and inflammatory ocular fundus diseases, with current anti-VEGF therapies requiring invasive and frequent intravitreal injections. In this study, a Trojan horse-inspired biomimetic lipoprotein nanocarrier (A-PA-αCPP) is developed for noninvasive anti-VEGF therapy via topical administration. Utilizing phosphatidic acid (PA), specifically 1,2-dioleoyl-sn-glycero-3-phosphate (DOPA), as the sole phospholipid, the nanocarrier enables electrostatic tethering of aflibercept, with a cell-penetrating peptide (CPP)-funct

Efficacy of posterior scleral reinforcement in the management of myopic traction maculopathy: a single-arm retrospective study.

BACKGROUND: Myopic traction maculopathy (MTM) is a condition characterized by retinal complications due to excessive axial elongation in myopic eyes. Posterior scleral reinforcement (PSR) stabilizes the posterior pole by mechanically supporting the thinned sclera, limiting further axial elongation, and reducing macular traction. This study evaluates the efficacy of PSR in patients diagnosed with MTM. METHODS: This study included patients with myopic traction maculopathy (MTM) who underwent posterior scleral reinforcement (PSR) between September 2020 and February 2023. Inclusion criteria were

Idiopathic Sclerochoroidal Calcification With a Concurrent COL11A2 Variant: A Case Report.

Sclerochoroidal calcification (SCC) is a rare ophthalmic finding often discovered during routine evaluation, characterized by calcium deposition within the sclera and choroid. We report a patient with bilateral SCC and a concurrent heterozygous COL11A2 variant. The patient reported gradual bilateral vision changes over several months. A fundus examination revealed yellow-white placoid lesions superior to the macula in both eyes, consistent with SCC. Multimodal imaging, including fundus autofluorescence (FAF), optical coherence tomography, and B-scan ultrasonography, confirmed the presence of c

Critical role of lipid metabolism in axial myopia development.

The global prevalence of myopia is becoming increasingly severe, with epidemiological models predicting that by 2050, approximately 50% of the world's population will be affected by myopia, and about 10% will suffer from high myopia. The incidence of high myopia is projected to increase fivefold, making it the leading cause of irreversible vision impairment. Myopia often leads to various complications and has been associated with other ocular diseases, including early-onset cataracts, age-related macular degeneration, and primary open angle glaucoma. As a result, the control and management of

Extraocular muscle-myokine-scleral remodeling axis: IL-6 trans-signaling as a novel mechanism in myopia development.

PURPOSE: Myokines modulate biological processes in adjacent and distant tissues. Given the crucial role of extraocular muscles (EOMs) during prolonged near work, this study investigated whether contracting EOMs secrete IL-6 and other myokines to influence scleral remodeling in myopia, and identified downstream targets via bioinformatic analyses. METHODS: Tear IL-6 and soluble IL-6 receptor (sIL-6R) concentrations were measured before and after 2 h of near work. An electrical stimulation model was established to mimic sustained EOM contraction. To further assess the role of IL-6 trans-signalin

Ophthalmic complications of radiotherapy.

Radiotherapy is a vital tool in the management of benign and malignant conditions affecting the eye, orbit, and surrounding tissues. It may be used alone or in combination with surgery or systemic therapies. While conventional external beam radiotherapy (EBRT) is still indicated for selected orbital and periocular conditions, intraocular tumours are now predominantly managed with plaque brachytherapy, proton therapy, or stereotactic radiosurgery. Despite advances in radiotherapy precision, adjacent normal structures are still often exposed to radiation, resulting in a variety of ophthalmic com

Dual-functionalized mesoporous silica nanoparticles for topical axitinib delivery to the posterior eye segment.

Topical drug delivery to the posterior eye segment remains a significant challenge due to ocular anatomical barriers, particularly in diseases such as wet age-related macular degeneration (AMD), where treatment typically relies on frequent intravitreal (IVT) injections of anti-angiogenic agents. In this study, we present a non-invasive eye drop formulation of axitinib (AXT), a potent vascular endothelial growth factor receptor (VEGFR) inhibitor, encapsulated within 25-nm dual-functionalized mesoporous silica nanoparticles (AXT@dual-MSNs) engineered for efficient retinal delivery. The nanoparti

Predicting Actual Lens Position to Generate a Novel Intraocular Lens Power Calculation Formula for Marfan Syndrome.

PURPOSE: To develop a novel intraocular lens (IOL) power calculation formula by predicting the actual lens position (ALP) in patients with Marfan syndrome who have ectopia lentis undergoing IOL implantation. METHODS: Patients with Marfan syndrome undergoing in-the-bag IOL implantation or scleral-fixated IOL (SF-IOL) implantation were prospectively recruited. ALP was calculated by adding anterior chamber depth to half of the IOL thickness, measured using the IOLMaster 700 (Carl Zeiss Meditec). A generalized linear model was developed to predict the ALP and integrated into effective lens positi

The Role of Decorin in the Ocular Surface: Structure, Function, and Therapeutic Potential.

The ocular surface, comprising the cornea, conjunctiva, and sclera, relies on a complex extracellular matrix (ECM) for structural support and functional integrity. Decorin (DCN), a small leucine-rich proteoglycan, a key ECM component involved in collagen fibrillogenesis, growth factor modulation, and cellular regulation, is essential for maintaining corneal transparency, wound healing, and preventing pathological fibrosis. DCN interacts with various target molecules, including transforming growth factor-beta (TGF-β), vascular endothelial growth factor (VEGF), and epidermal growth factor recept
