Diagnostic value of paediatric blood culture bottle allied to MALDI-TOF mass spectrometry in the aetiological assessment of microbial keratitis.

PURPOSE: Infectious keratitis remains a sight-threatening condition, yet its aetiological diagnosis is often hampered by low culture positivity, largely due to limited corneal scraping material. Identifying alternative, high-yield methods is crucial to optimise treatment protocols. This study evaluates the diagnostic performance of paediatric blood culture bottles (PBCB) compared with the conventional multi-sampled method. MALDI-TOF mass spectrometry (MS) was used as the standardised identification tool for isolates recovered using both approaches. METHODS: This prospective study included pat

Microbial keratitis due to Neoscytalidium dimidiatum: Case report & Review of literature.

We present the case of a 68-year-old woman who presented with left eye corneal ulcer. Corneal scrapings and microbiological evaluation revealed fungal corneal infection due to Neoscytalidium dimidiatum. The patient responded well to topical natamycin, voriconazole, atropine, and oral itraconazole. At six-week follow-up, visual acuity improved to 6/9 with a quiet eye and a nebulomacular corneal scar. Accurate identification by appropriate microbiological investigations along with a review of literature on N. dimidiatum keratitis is presented.

From Drug Choice to Drug Delivery: A Viewpoint on Measuring and Improving Owner Adherence in Canine Corneal Ulcers.

Therapeutic success in canine corneal ulcers depends on appropriate antimicrobial selection and reliable at-home drug delivery. However, veterinary data on owner adherence remain limited and are largely self-reported. Evidence from human ophthalmology indicates that dosing frequency, follow-up interval, and hands-on technique training strongly influence treatment outcomes. This viewpoint advocates a shift from drug-centered to delivery-centered management and proposes a testable four-part Clinical Adherence Bundle (CAB-4): scheduling timely rechecks within 14 days, minimizing dosing frequency

Role of cell-associated and secretion-independent virulence factors of Pseudomonas aeruginosa in microbial keratitis and anti-virulence therapeutic strategies.

PURPOSE: Microbial keratitis (MK) caused by Pseudomonas aeruginosa is a rapidly progressive and vision-threatening corneal infection worldwide. Even short delays in diagnosis or inappropriate therapy can lead to irreversible corneal scarring and blindness. This opportunistic pathogen colonizes, invades, and damages ocular tissue by taking advantage of disruptions in the corneal epithelial barrier and using a variety of virulence factors. The aim of this study was to summarize the current understanding of P. aeruginosa’s cell-associated, and secretion system-independent virulence factors involv

Postoperative endophthalmitis is a rare but severe complication of phacoemulsification cataract surgery (PCS). Acinetobacter baumannii is an uncommon pathogen, often associated with multidrug resistance and poor outcomes. In this report, a 73-year-old male who developed endophthalmitis following PCS is introduced, who, despite intensive empirical therapy, experienced no clinical improvement. Following a vitreous sample analysis with polymerase chain reaction, A. baumannii infection was diagnosed. The patient was treated with fortified topical colistin, amikacin, and subconjunctival and intravi

Congenital tarsal kink syndrome.

Congenital horizontal tarsal kink syndrome is a rare but well-defined form of congenital upper eyelid entropion, characterized by a horizontal tarsal fold causing inversion of the lid margin and lashes, with absence of a visible upper eyelid crease. Induced trichiasis may lead to corneal complications and a major risk of amblyopia if diagnosis and management are delayed. We report the case of a 4-week-old infant with right unilateral tarsal kink complicated by a central corneal ulcer and associated microphthalmia. This case was managed by an anterior approach with release of orbicularis adhesi

Successful Medical Management of Pigmented Fungal Keratitis Following Vegetative Trauma.

Fungal keratitis remains a frequent cause of corneal morbidity in tropical regions, particularly after trivial trauma involving plant material. Dematiaceous fungi constitute a smaller subgroup of filamentous organisms but may present with characteristic pigmentation over the corneal lesion. A middle-aged woman with controlled diabetes who developed pain and visual blurring after minor ocular trauma came to the ophthalmology outpatient department. Slit-lamp examination showed a paracentral ulcer with feathery margins and a brownish plaque. Direct microscopy confirmed septate pigmented hyphae. F
