In Vitro Characterization of Insulin-Loaded Soft Contact Lenses and Their Effect on Corneal Epithelial Cell Viability and Permeability.

Background/Objectives: Corneal epithelial defects and ulcers remain a significant clinical challenge, often leading to vision impairment and requiring prolonged treatment. In this context, topical insulin has recently gained attention in ophthalmic research. However, conventional eye drops suffer from short residence time and poor bioavailability. To overcome these limitations, the present study evaluates, for the first time in vitro, multiple commercially available soft contact lenses as sustained insulin delivery platforms, analyzing how protein loading influences the essential physicochemic

Successful management of pediatric Pythium insidiosum keratitis - A case report highlighting the role of targeted antimicrobial therapy and mechanical debridement.

Human pythiosis keratitis is a rare but potentially sight-threatening infection. We report the case of an 8-year-old boy who developed a nonhealing corneal ulcer after finger nail trauma that did not respond to broad-spectrum antibiotics. Initial cultures identified Staphylococcus warneri and Streptococcus mitis. Worsening clinical signs prompted repeat investigations, which confirmed Pythium insidiosum through polymerase chain reaction. Despite intensive topical (natamycin and voriconazole) and systemic antifungals (voriconazole), the ulcer progressed with biofilm formation. Manual debridemen

A case of severe bilateral corneal ulcer associated with sintilimab therapy: case report and literature review.

BACKGROUND: Immune checkpoint inhibitors, including PD-1 inhibitors such as sintilimab, have revolutionized cancer treatment but may induce immune-related adverse events (irAEs). Ocular irAEs, particularly severe corneal complications, are increasingly recognized but remain underreported, especially with sintilimab. METHODS: We report a case of a patient in their 60 s with gastric adenocarcinoma who developed bilateral severe corneal ulcers following sintilimab therapy. A comprehensive review of literature on PD-1/PD-L1 inhibitor-associated corneal lesions was conducted to contextualize this

Morphological Changes in Fungal Elements and Antifungal Drug Susceptibility in an Ex Vivo Model of Fungal Keratitis.

PURPOSE: Fungal keratitis is a serious eye infection that can cause irreversible vision loss. One of the causative fungal pathogens, Fusarium solani (Neocosmospora solani), shows limited susceptibility to antifungal drugs. Furthermore, differences between in vitro and clinical outcomes are encountered in some cases, complicating treatment. In this study, we compared in vitro and ex vivo antifungal susceptibility using a porcine corneal model of F. solani keratitis. METHODS: An ex vivo fungal keratitis model was developed using porcine corneas, which were inoculated with F. solani and assigned

Rare case of Colletotrichum coccodes causing chronic fungal keratitis in an elderly female: Diagnostic delay and antifungal resistance.

UNLABELLED: Fungal keratitis is a vision-threatening corneal infection, most commonly caused by Fusarium and Aspergillus. We report a rare case of keratitis caused by Colletotrichum coccodes in an elderly patient with a chronic corneal ulcer refractory to fluconazole. Initial corneal scrapings were negative, delaying diagnosis. Repeat deep biopsy, fungal culture and pan-fungal polymerase chain reaction confirmed C. coccodes. The patient was managed with intensive topical amphotericin B and voriconazole, alongside a glycerol-preserved corneal patch graft to provide tectonic support. CONTRIBUTI

Corynebacterium bovis Keratitis Mimicking Acanthamoeba Infection Following Ocular Trauma.

Corneal isolates of Corynebacterium species are frequently dismissed as contaminants, yet they are capable of causing severe, vision-threatening keratitis. A 45-year-old man with no history of contact lens use developed acute left eye pain and visual loss after a stone fragment struck his eye during work, with immediate contamination by garden hose water. Slit-lamp examination revealed a ring-shaped stromal infiltrate with visual acuity reduced to light perception, a presentation consistent with Acanthamoeba keratitis. Empiric therapy with fortified topical vancomycin and ceftazidime, antifung

Traumatic canalicular laceration and complex eyelid injury in a newborn delivered by forceps.

We present a neonatal case of complex upper eyelid lacerations with canalicular injury following a forceps-assisted vaginal delivery. Mother's insistence on vaginal delivery despite obstetric advice of caesarean section led to obstructed labour requiring forceps delivery. The neonate presented with four full-thickness lacerations of the right upper eyelid, including injury to the upper canaliculus. Immediate surgical repair with canalicular stenting was performed. A corneal ulcer developed secondary to stent-induced mechanical trauma. Early stent removal with topical fortified antibiotics led
