Clinical presentations, complications, and outcomes of benoxinate topical ophthalmic anesthetic abuse: a retrospective cohort study.

BACKGROUND: Topical anesthetic abuse is a concerning ophthalmic morbidity that may lead to irreversible visual loss. The primary goal of this study was to compare the clinical presentations, complications, healing duration and visual acuity changes between topical ophthalmic anesthetic users and non-users after superficial corneal trauma. The secondary goal was to correlate different variables related to the use of topical ophthalmic anesthetics with the final clinical outcomes. METHODS: This retrospective cohort study reviewed files of patients with a positive history of superficial corneal

ABO blood group associations with corneal and organ failure diagnoses on the Ecuadorian transplant waiting list: A 2017-2022 observational cohort study.

BACKGROUND: ABO antigens are broadly expressed on endothelial and epithelial glycoproteins and modulate innate immune responses, complement activation, tissue immunogenicity, and endothelial-derived haemostatic factors. Their differential distribution across the specific diagnoses that lead to end-stage organ failure has been little explored in Latin American populations, where a high prevalence of group O provides a distinct genetic background to examine these associations. We report an exploratory analysis in national transplant waiting list of Ecuador. METHODS: We conducted a retrospective

Smartphone-Based Portable Slit Lamp in Anterior Segment Diseases: A Narrative Review of Clinical Assessment and Integration with Artificial Intelligence.

Smartphone-based portable slit lamps are rapidly evolving from simple add-ons into practical, low-cost front-line tools for anterior segment care beyond traditional clinical settings. By integrating high-performance smartphone cameras with compact optical attachments (e.g., slit-light converters, macro lenses, blue filters) and dedicated applications, these devices deliver slit-lamp-style imaging and video capture to environments where conventional biomicroscopes are inaccessible. Accumulating clinical evidence-most notably for the Smart Eye Camera (SEC) and iSpector MINI HE 010-21-confirms th

Automated dual-camera DMD system for stereo optical sectioning imaging.

We present a compact, automated, stereo-optical-sectioning imaging system for clinical assessment of the anterior segment, using a digital micromirror device (DMD) as a spatial light modulator. The platform synchronizes two CMOS cameras with the DMD, which projects programmable slit beams to produce depth-resolved cross-sectional views of the anterior eye. By adjusting the micromirror activation time, the system can also extend the dynamic range of the acquired images, accommodating both dim (e.g., the cornea) and highly reflective regions (i.e., the iris). In a pilot study, the prototype inst

Rapid Pterygium Progression in a Child on Growth Hormone Therapy: A Case Report and Literature Review.

Pterygium is rare in children, and the effects of growth hormone (GH) therapy on its progression have not been previously described. Because GH and GH-releasing hormone receptor pathways influence cellular proliferation and inflammation, GH supplementation may contribute to accelerated pterygium growth in susceptible patients. This case highlights a rapidly progressive pterygium in an adolescent receiving GH therapy. A 13-year-old male presented with blurred vision and a rapidly enlarging nasal pterygium in the left eye. A small red nodule had first been noted in this patient at six years of a

Bridging the Gap: A Case of Unique Anterior Segment Optical Coherence Tomography Presentation of Retrocorneal Membrane and Fibrous Bridging After Birth-Related Descemet's Membrane Detachment.

Descemet's membrane detachment (DMD) is an uncommon corneal pathology that typically occurs following intraocular surgeries or direct ocular trauma. Congenital or birth trauma-related DMD is rare and often remains undiagnosed in asymptomatic individuals. Longstanding cases may develop fibrous bridging or retrocorneal membrane formation, making diagnosis challenging without advanced imaging. We report the case of a 32-year-old asymptomatic male who presented for a routine ophthalmic evaluation. Slit-lamp examination of the right eye revealed a localized inferotemporal corneal opacity with subtl

A bioactivity-enhanced thermosensitive amnion-derived hydrogel with sustained IGF-1 release: A multitargeted and efficient strategy for corneal injury repair.

Corneal alkali burn (CAB) remains difficult to treat because rapid tear clearance limits topical drug exposure, while the injured cornea develops a complex microenvironment involving inflammation, neovascularization, fibrosis, epithelial barrier loss, and impaired nerve repair. Here, we developed a human amniotic membrane-derived thermosensitive hydrogel (hAMgel) and loaded it with insulin-like growth factor-1 (IGF-1) to create a bioactive sustained-release system (IGF1@hAMgel) for CAB repair. The hAMgel transformed from a flowable precursor into a semi-transparent film on the ocular surface w

Repurposing verteporfin and hyaluronic acid gel for ocular surface treatment to prevent corneal scarring.

There is an unmet need for point-of-care therapies to prevent scarring and promote corneal clarity after injury, which is essential for maintaining vision. Verteporfin, an inhibitor of Yes-associated protein (YAP), has been shown to prevent fibrosis in several organs. Visudyne (VP) is an FDA-approved liposomal formulation of verteporfin used to treat abnormal blood vessels in the eye. Here, we showed that VP reduces myofibroblast formation in corneal stromal fibroblasts. To prolong the residence time of verteporfin on the ocular surface, the cohesive viscoelastic ProVisc® hyaluronic acid (HA)

Phototherapeutic Keratectomy for Concrete-Induced Corneal Chemical Injury: A Case Report.

Chemical injuries to the cornea, particularly those caused by alkaline substances like concrete, can lead to severe ocular damage and vision impairment. To the best of our knowledge, there are no published reports on the use of phototherapeutic keratectomy (PTK) specifically for concrete-induced corneal injuries. This case report presents the first documented successful management of a concrete-induced corneal chemical injury using PTK. A 69-year-old male patient presented with bilateral ocular injuries following exposure to concrete after a machine explosion at work. The right eye exhibited s

Enhanced Corneal Wound Healing and Neovascularization Suppression via AME-SupraGel: A Biocompatible Hydrogel with Sustained Release and Stability.

Amniotic membrane extract (AME) is widely recognized for its anti-inflammatory, antifibrotic, and antiangiogenic properties, making it a valuable option for corneal wound healing. However, short retention time and rapid degradation of AME limit its clinical efficacy. In this study, we developed a novel AME-SupraGel that combines the therapeutic benefits of AME with the sustained-release capabilities of a supramolecular peptide-based hydrogel. The design of AME-SupraGel addresses the limitations of the AME solution by enhancing retention on the ocular surface and providing prolonged therapeutic

A deep dive into radiation keratopathy; Going beyond the current frontierss.

Radiotherapy is one of the conventional treatments for head and neck malignancies. Despite the implementation of protective measures to minimize the detrimental impact on healthy tissues surrounding the radiation site, radiation keratopathy remains a prevalent complication. We aimed to establish a mouse model of radiation keratopathy to characterize the pathophysiology of the disease and enable future identification of potential treatments. Thirty-six mice were divided equally into six groups. One eye of each mouse was irradiated with 5, 10, 15, 20, 25, and 30 Gy and the other eye used as a co

Long-term ocular complications after kidney transplantation.

To assess long-term ocular complications and identify factors affecting these complications in pre-emptive and dialysis patients who have undergone kidney transplantation. We included 548 patients who had a complete ophthalmologic examination at least one year after kidney transplantation. The patients were divided into two groups: those who received dialysis before kidney transplantation (group 1) and those who did not (group 2). We recorded the presence of diabetes mellitus (DM) and hypertension (HT), which could contribute to concomitant retinopathy. Data collected included best corrected v

Schnyder Corneal Dystrophy in an Adolescent: A Case Report With Multimodal Imaging.

Schnyder corneal dystrophy (SCD) is a rare, autosomal dominant, bilateral corneal dystrophy characterized by progressive deposition of cholesterol and phospholipids within the central corneal stroma. The condition is associated with pathogenic variants in the UBIAD1 gene, responsible for lipid metabolism. We report the case of a 17-year-old female presenting with progressive bilateral visual deterioration. Comprehensive ophthalmological examination, including slit-lamp biomicroscopy, in vivo confocal microscopy, and anterior segment optical coherence tomography (AS-OCT), was performed. The exa

Retrolental Opacity: Atypical Manifestation of Lens-Induced Uveitis.

The crystalline lens can cause intraocular inflammation when its capsule is ruptured or allows leakage of lens proteins into the anterior chamber, a condition known as lens-induced uveitis (LIU). LIU is a rare form of uveitis and can be classified based on the integrity of the lens capsule. When the capsule is ruptured, the inflammation is called phacoantigenic or phacoanaphylactic uveitis, whereas if the capsule remains intact but leaks proteins, it is termed phacolytic uveitis. Common causes include ocular trauma, previous surgery, or spontaneous rupture in hypermature cataracts. Initial tre

Ipsilateral, Nonrotational Autokeratoplasty (INRA) Enabling Cataract Surgery in Eyes With Severe Ocular Surface Disease and Corneal Opacity.

PURPOSE: In patients with severe ocular surface disease (OSD) and cornea and cataract-related vision impairment, where the risk of allograft rejection after allogeneic corneal transplantation is unacceptably high and cataract surgery impossible because of corneal opacity, cataract extraction may still offer meaningful improvement in vision-related quality of life. We describe ipsilateral, nonrotational autokeratoplasty (INRA) as option to perform open-sky cataract surgery after removal of the central cornea and prevent immune responses by placing back the original host cornea.
