Presented here are the clinical, histological, histochemical, and ultrastructural findings of an eye with notable ochronotic changes. Ochronotic pigment is seen by light microscopy as amber globules or fiber-like structures in the cornea, conjunctiva, and sclera combined with degenerated collagen. This case report documents a dense and extensive deposition of uveal pigment in corneal endothelial cells of a patient who had undergone 2 previous grafts subsequent to an alkali burn injury. Multiple cardioversion shocks were recently reported to cause disruption of the iris pigment epithelium. Pigment debris was deposited on the anterior lens surface, iris stroma, corneal endothelium, and trabecular meshwork. Similar findings were observed in 3 of 4 patients receiving more than 10 electroconvulsive therapy shocks and in a fifth patient who became attached to an electrified fence.

A case of 'black cornea', occurring 6 years after commencement of a therapy with adrenaline derivative eye drops, is reported. Enucleation was performed because of suspected malignant melanoma. The eye was sent to our laboratory for histopathological examination. The clinical and histopathological findings are described, and the pathogenesis of the 'black cornea' is discussed.

The authors have studied the corneal and lens lesions which appeared following a prolonged treatment by phenothiazines. They examined 186 patients: 147 took phenothiazines of which 35 of them presented anterior segment alterations. We report a case of 'black cornea" secondary to topical epinephrine therapy and present the biochemical analysis of the lesion. The formed pigment is a melanin and not the intermediate substance, adrenochrome, as has been previously reported. Previously undescribed ocular and dermatologic complications of prolonged chlorpromazine therapy have been noted in 70 patients of a series of many thousands receiving similar therapy.
