Subretinal Aspects of the Optoretinographic Response.

PURPOSE: Optoretinography (ORG) detects stimulus-evoked, nanometer-scale changes in the optical path length of photoreceptors, hypothesized to reflect osmotic water shifts into and out of the outer segments (OS), as well as electrostatic effects of opsin photoisomerization. The aim of this study was to measure parallel changes in the subretinal space (SRS), which may reflect changes in the SRS volume. The results of these experiments could affect interpretations of the photoreceptor ORG, which make testable hypotheses about the SRS volume. Moreover, because water movement in the outer retina i

Posterior Migration of a Descemet Membrane Endothelial Keratoplasty Graft Associated With Macular Proliferative Changes.

A 79-year-old male underwent descemet membrane endothelial keratoplasty (DMEK) for bullous keratopathy. Intraoperatively, the intraocular lens (IOL) became unstable during graft manipulation, and the graft was lost from the anterior chamber. Postoperative examination failed to identify the graft. Two weeks later, optical coherence tomography (OCT) showed macular proliferative changes without hemorrhage or peripheral ischemia. Subsequent pars plana vitrectomy revealed the DMEK graft in the vitreous cavity, associated with fibrovascular proliferation. The graft was successfully removed. This cas

Interaction of an Iris Implantation Cyst with Pseudophakic Bullous Keratopathy: A Case Report.

Background and Clinical Significance: Currently, the only effective treatment for cataracts is surgery. The most commonly employed method is phacoemulsification, a well-established procedure that uses ultrasound energy to fragment the lens, allowing for easier removal. Potential postoperative complications range from mild to severe corneal edema (pseudophakic bullous keratopathy), which may be caused by intraoperative endothelial cell damage, to the rare formation of iris cysts. Case Presentation: In this paper, we report the case of a patient who underwent cataract surgery in both eyes, where

Cultured Human Corneal Endothelial Cell Injection (Vyznova) in Eyes with Extremely Low Intraocular Pressure After Glaucoma Surgery: Case Reports.

PURPOSE: Eyes with extremely low intraocular pressure (IOP) less than 4 mm Hg after glaucoma surgery represent a major challenge for endothelial keratoplasty, as inadequate anterior chamber support often leads to graft detachment and early graft failure. Cultivated human corneal endothelial cell (cHCEC) injection has recently emerged as a graft-free therapeutic alternative; however, its efficacy in eyes with extremely low IOP has not been well established. METHODS: This case series describes the clinical outcomes of cHCEC therapy using Vyznova in 2 patients with bullous keratopathy (BK) and p

Regression of Recalcitrant Corneal Neovascularization and Graft Revival With Systemic Bevacizumab: A Case Report.

PURPOSE: Corneal neovascularization is a sight-threatening pathology arising from various conditions and compromises keratoplasty survival. Treating neovascularization is critical for graft outcomes; however, both medical and surgical interventions often fail in severe cases. We report the first known case of regression of corneal neovascularisation associated with systemic bevacizumab accompanied by unexpected recovery of a previously failed Descemet Membrane Endothelial Keratoplasty (DMEK). METHODS: A 64-year-old woman presented with right eye bullous keratopathy and deep stromal neovascula
