We report the development of pseudophakic bullous keratopathy in 18 eyes following implantation of Pharmacia Intermedics Ophthalmics model 024 (Hessburg) anterior chamber intraocular lenses. Eight of these intraocular lenses were implanted during uncomplicated intracapsular cataract extractions. Mean onset of corneal edema was 21.4 months after lens implantation. Patients with this type of lens already implanted in their eyes require close follow-up for complications.

Five patients developed corneal edema presumably caused by accidental preoperative ocular exposure to Hibiclens. In all cases, the patients complained of ocular pain after surgery. Conjunctival inflammation and corneal epithelial defects were found in all patients. Seventeen months after penetrating keratoplasty for pseudophakic bullous keratopathy, a patient developed severe pain and a peripheral corneal ulcer that had the characteristic clinical appearance of a Mooren's ulcer. The IOLAB AZAR 91Z flexible anterior chamber intraocular lens may have the highest complication rate of any contemporary intraocular implant. Pseudophakic bullous keratopathy is now the most common reason for penetrating keratoplasty. In previous reports, the type of intraocular lens (IOL) most frequently encountered in these eyes was the iris plane IOL. The authors reviewed 27 cases of IOL removal during penetrating keratoplasty. The occurrence of pseudophakic bullous keratopathy following anterior chamber intraocular lens implantation is well documented. At the time of penetrating keratoplasty and anterior chamber intraocular lens removal, we attempt to reimplant a posterior chamber lens whenever possible because of the complications associated with anterior chamber lenses. We developed a technique to exchange an anterior chamber lens with a posterior chamber lens during penetrating keratoplasty. The intraocular lens loops are inserted in the ciliary sulcus and the optic is sutured to the iris.

In 1986, conjunctival transplantations were performed on 54 patients. Patients with severe corneal ulcers, who did not react adequately to local therapy, formed the largest group who were treated in this way, in particular patients in whom the prognosis for a perforating corneal graft was poor. Other indications are: bullous keratopathy, pterygium, trauma, symblepharon, etc,. In general other forms of therapy are tried first before conjunctival transplantations are performed.

A patient developed an acute onset of pseudophakic bullous keratopathy and a diffuse, gray, subepithelial corneal deposit ten days following surgery during which sodium chondroitin sulfate was used. During the four-and-a-half year period from January 1982 to June 1986, 511 penetrating keratoplasty specimens were submitted to the Pathology Department of The Royal Victorian Eye and Ear Hospital.
