A child who had had a congenital cataract phacoemulsified at age four months became contact lens intolerant, and at age two years had an implantation of a Worst metal-looped iris-clip intraocular lens. He developed painful pseudophakic bullous keratopathy three years later. Since the child remained with dense deprivation amblyopia and contact lens intolerance, when the IOL was removed an epikeratophakia graft was applied. The pathology of early pseudophakic keratopathy is demonstrated in a corneal button obtained at penetrating keratoplasty for persisting corneal edema and bullous keratopathy 4 months following extracapsular cataract extraction with a posterior chamber lens implant. Histological examination revealed a distinct cell loss and a macrophage reaction that was centered on degenerating endothelial cells in the corneal endothelium.

We studied four patients who, having received postoperative 5-fluorouracil after glaucoma filtering operations, developed serious corneal complications. All four patients had preexisting corneal abnormalities including keratoconjunctivitis sicca, exposure keratopathy, and bullous keratopathy. All of the patients developed epithelial defects in the postoperative period. The complications included bacterial corneal ulceration (two patients), sterile corneal ulceration and corneal perforation (one patient), and a keratinized corneal plaque with underlying sterile stromal infiltrate (one patient).

A rigid one-piece PMMA anterior chamber lens (Scharf type) had to be removed 30 years after implantation because of diffuse bullous keratopathy and painful secondary glaucoma. Scanning electron microscopy showed the IOL to have a crater-like surface, while the haptics, having been in contact with intraocular tissues, were superficially and deeply eroded. Comparison with two identical anterior chamber lenses which had not been implanted revealed that the changes seen in the removed IOL must have been the result of degradation of the PMMA material.

A technique is described for implantation of a posterior chamber intraocular lens during keratoplasty for bullous keratopathy when a large capsulotomy and vitreous prolapse are present. Intramembrane changes occurring in dysfunctional corneal endothelial cell membranes were examined using freeze-fracture and transmission electron microscopy techniques. Three categories of dysfunctional endothelium were examined: aphakic bullous keratopathy, pseudophakic bullous keratopathy and Fuchs' endothelial dystrophy. A retrospective study of 98 91Z anterior chamber intraocular lenses used with intracapsular cataract extraction was performed. Minimum follow-up was 12 months and average follow-up was two years seven months. These patients were part of a group of 113 91Z patients previously reported with minimum six-months follow-up and an average of one year follow-up. We adapted a previously described procedure to quantitate collagen in corneal tissue sections prepared from paraffin-embedded samples. The method entailed staining the deparaffinized tissue sections with Sirius red, eluting the bound dye with NaOH-methanol, and estimating the color in a spectrophotometer as an indication of the collagen content. Patients involved in a clinical trial of cataract surgery who developed pseudophakic bullous keratopathy were studied. We performed a retrospective analysis of the clinical and pathologic diagnoses of 497 corneal buttons that had been submitted to the Estelle Doheny Eye Pathology Laboratory, Los Angeles, during the five-year period 1979 through 1983. The incidence of clinically significant and angiographically documented cystoid macular edema (CME) following penetrating keratoplasty was studied in a consecutive series of 102 eyes over an average follow-up of 30 months.
