The clinicopathologic findings in 17 eyes which were surgically enucleated or removed at autopsy as well as the histopathology of 72 corneal buttons removed from eyes with intraocular lenses are described. In four of five autopsy eyes the intraocular lens was well tolerated; however, in all cases, changes related to the pseudophakos were found. In seven of 12 surgically enucleated eyes, postsurgical bacterial or fungal endophthalmitis had developed. We compared the corneal endothelial morphology between an age-matched population of American and Japanese persons. Endothelial specular photographs were subjected to a computer-assisted morphometry of the area and shape of individual cells. The endothelium in the Japanese population showed a significantly higher cell density when compared with an age-matched American population. We present an initial report on flexible anterior chamber lens implantation in 14 aphakic or pseudophakic bullous keratopathy patients undergoing penetrating keratoplasty. With a minimum follow-up of 12 months, the improvement in visual acuity and graft clarity is encouraging.

Although cataract surgery alone carries a risk of iatrogenic endothelial damage, IOL implantation will further increase the risk of endothelial compromise and resulting bullous keratopathy. PBK is an important consideration in the differential diagnosis of corneal disease in the pseudophakic eye and with proper treatment visual loss due to PBK is usually reversible. While seen most frequently with iris clip implants, all pseudophakes are susceptible to PBK and should be carefully observed for this disease entity.

Fifty-three patients with pseudophakic bullous keratopathy (PBK) were treated with: Corneal transplant, Removal of the original intraocular lens (IOL), Replacement with a sutured-in posterior chamber IOL. Vitrectomy was performed in all but two cases. Eighty-eight per cent of the corneal transplants were clear and the vision was 20/200 or better in seventy-five per cent. Visual results are equal to other reported series where over one-third of the original IOLs were removed at the time of keratoplasty and were not replaced. Vitreous samples were obtained from 41 eyes undergoing vitrectomy, and radioimmunoassays were performed to measure concentrations of the prostaglandins PGE2, PGF2 alpha, prostacyclin, and thromboxane. A child with a history of a monocular congenital cataract, cataract surgery and intraocular lens implantation at age three developed pseudophakic bullous keratopathy which persisted following intraocular lens removal and vitrectomy two years later. At age nine she presented to the Cornea Service at Wills Eye Hospital with painful bullous keratopathy and underwent a penetrating keratoplasty and anterior vitrectomy. The case is presented to bring attention to potential long-term corneal complications of pediatric cataract surgery with intraocular lens implantation.

Pseudophakic bullous keratopathy (PBK) has become the leading indication for penetrating keratoplasty. In our initial fifty patients having keratoplasty for PBK there was gradual loss of clear grafts in patients with retained intraocular lenses (IOLs). Specular microscopy was performed on donor tissue, and periodically postoperatively, for 130 grafts for PBK. We present the pathologic and scanning electron microscopic findings from 44 semiflexible, all polymethylmethacrylate, anterior chamber intraocular lenses (IOLs). Seventy-six corneal transplants performed by one surgeon for pseudophakic bullous keratopathy (PBK) were studied. The mean follow-up was 23 months (range 2-76). Of 41 eyes with lenses left in situ, the mean acuity was 20/45; 60.9% of the eyes saw 20/40 or better. In contrast, the mean acuity for 18 eyes with lens removal and 17 eyes with intraocular lens exchange was 20/52 and 20/62, respectively; 42.8% saw 20/40 or better. Two male patients had corneal endotheliopathy of unknown origin. In both patients it was characterized by continuously progressing bullous keratopathy preceded by a line of keratic precipitates resembling a rejection line. The disease was progressive and did not respond to intensive corticosteroid therapy. No virus could be isolated from the aqueous humor, and the aqueous-antibody titers to herpes simplex, varicella zoster, and measles viruses were below detection level.

The fellow normal-appearing eyes of 23 patients with unilateral aphakic bullous keratopathy were examined by wide-field specular microscopy and pachymetry. All patients had: pseudophakic or aphakic bullous keratopathy in one eye; unoperated fellow eye on no medication; no other ocular disease; and no endothelial guttata seen by slitlamp biomicroscopy or specular microscopy. A 33-year-old white woman was referred for treatment of a painful left eye in 1964. She underwent an 11 mm corneal transplant for bullous keratopathy and did well until 1982 when a transplant was performed on the right eye for similar problems. Color specular microscopy, a noninvasive, in vivo microscopic technique, was utilized to study the corneal epithelium in 17 patients including eight with keratoconus, seven with bullous keratopathy, and two with Fuchs' corneal dystrophy.
