This is a review of 426 Fyoderov Mk I lenses inserted by one surgeon following intracapsular cataract extraction. Important complications in the first year are, macular oedema and retinal detachment with bullous keratopathy increasing over the years and frequently associated with previous problems. It is concluded that the ideal intraocular implant is a single plane lens, independent of iris support, without loops and suitable for insertion after extracapsular cataract extraction through a section in the plane of lens insertion.

In 22 glaucomatous eyes with functioning filtering blebs and adequately controlled intraocular pressures (less than or equal to 22 mm Hg) that underwent clear-cornea cataract extraction, the intraocular pressure remained controlled in 20 (91%). Of 45 eyes with pseudophakic bullous keratopathy, 40 underwent a penetrating corneal transplant. Thirty-six were followed for over six-months post surgically. Preoperative increased intraocular pressure and endothelial dystrophy in the contralateral eye were common findings. The most important factors limiting visual acuity postoperatively were increased intraocular pressure and cystoid macular edema. Of 281 eyes that had had Sato's anterior-posterior radial keratotomy for myopia or myopic astigmatism, 80 (50 patients) underwent follow-up examinations ten to 20 years later. Sixty of the 80 eyes had bullous keratopathy. The onset of this disease occurred when the patient was about 40 years old, regardless of the patient's age at the time of surgery. Seven corneal buttons were obtained from patients with bullous keratopathy who required penetrating keratoplasty. Light and electron microscopy disclosed subepithelial bullae, degenerate or absent Bowman's zone, and thickened Descemet's membrane.

The ultrastructure of Descemet's membrane was studied by transmission electron microscopy in nine corneal buttons from aphakic eyes with bullous keratopathy. We used closed-eye endosurgery to treat 58 patients with anterior segment problems. These included nonsenile cataract, residual lens material, pupillary membranes, vitreocorneal touch, aphakic pupillary block glaucoma, intraocular lens complications, epithelial ingrowth, and total hyphema. A serious problem arises in retinal detachment surgery when intravitreous gas is needed and an iris-supported artificial lens is in place. Using a very small-gauge long needle to cross the anterior chamber just anterior to the lens, monofilament suture can safely be brought across the chamber by threading it through the lumen of the needle and then removing the needle. The process can be repeated in a parallel position to create a mattress suture with no risk of lens-endothelial touch. The suture can be removed easily in the office when the gas has been absorbed. A comparison was firstly done of the oxygen flux through the Bausch and Lomb Plano T lens, the Bausch & Lomb U3 lens, and the Duragel 75 lens with a sensor on the normal eye. The results showed that the Bausch and Lomb Plano T lens was the best for use with the sensor probe. The sensor was then used to obtain a comparison with the normal oxygen flux through a therapeutic lens on keratoplastic eyes and eyes with bullous keratopathy. The results showed that both conditions had a lower oxygen consumption than normal and that eyes with bullous keratopathy predictably had the lowest value.

Thirty-six patients with pseudophakic bullous keratopathy had penetrating keratoplasties with one immediate graft failure and two subsequent rejections. Fifty-four percent of the patients saw 20/40 or better, and 79% saw 20/80 or better. These results are much better than reported for aphakic bullous keratopathy. Endothelial counts in the other eyes showed that 17% of these patients have unexpectedly very low cell counts, which were not appreciated on slitlamp examination and were probably a contributing factor to their corneal decompensation.

Pars plana lensectomy by ultrasonic fragmentation appears to be the treatment of choice for extraction of subluxated and dislocated lenses. The closed system technique allowed extraction of 11/11 subluxated lenses without operative or postoperative complications with an average observation period of 4.5 years. Lens implant surgery has a significant complication rate, although most complications are treatable and occur within the first 2 months after operation. The short-term visual results are comparable with the published results of intracapsular extraction alone. Six cases of bullous keratopathy developed in 138 cases in 7 1/2 years. When endothelial cell density fell below 500/mm2, corneal oedema developed within the year. However, a more recent study showed that endothelial cell loss was much reduced by the closed chamber technique of insertion and by serum coating of the implant.
