A case of corneal epithelial downgrowth after penetrating keratoplasty treated by intracameral 5-fluorouracil injection and mitomycin C.

BACKGROUND: Corneal epithelial downgrowth is an uncommon, yet potentially sight-threatening complication of intraocular surgery or trauma with poor prognosis. We hereby describe a case of epithelial downgrowth after penetrating keratoplasty which was treated by intracameral 5-fluorouracil injection. CASE DESCRIPTION: In 2019, penetrating keratoplasty was performed for a 69-year-old man with pseudophakic bullous keratopathy. Six months after penetrating keratoplasty, an endothelial opacity suspicious of epithelial downgrowth was noted. Two treatments of intracameral 5-fluorouracil injection we

Silicone Oil Migration into the Graft Interface after Descemet Stripping Automated Endothelial Keratoplasty in a Patient with a Past Surgical History of Silicone Oil Tamponade: A Case Report.

INTRODUCTION: Descemet stripping automated endothelial keratoplasty (DSAEK) is widely accepted as an alternative to penetrating keratoplasty for the treatment of corneal endothelial failure. Surgical outcomes, however, may be compromised in eyes with complex surgical histories, particularly following vitreoretinal procedures. We describe a case of silicone oil deposition at the donor-recipient interface after DSAEK in a patient with prior silicone oil tamponade. CASE PRESENTATION: A 42-year-old man with pseudophakic bullous keratopathy previously underwent traumatic cataract surgery, pars pla

Multiplanar AS-OCT Detection of Clinically Occult Posterior Gas Bubble Dislocation After DSAEK.

Descemet stripping automated endothelial keratoplasty (DSAEK) is a well-established surgical technique for the treatment of endothelial dysfunction, in which intracameral gas tamponade plays a critical role in graft adherence. We report the case of a 67-year-old pseudophakic woman with advanced Fuchs endothelial corneal dystrophy and symptomatic pseudophakic bullous keratopathy in the right eye, who presented with progressive visual deterioration and underwent DSAEK using an 8.25 mm donor graft inserted with a Busin glide and tamponaded with a 25% sulfur hexafluoride (SF6) gas-air mixture. Bleb Compressive Sutures in Descemet Stripping Automated Endothelial Keratoplasty for Eyes with Filtering Blebs Following Trabeculectomy.

Background/Objectives: A disadvantage of Descemet stripping automated endothelial keratoplasty (DSAEK) in eyes with prior glaucoma filtration surgery is the difficulty in maintaining air tamponade during the procedure. Herein, we report the use of bleb compressive sutures in managing air tamponade in the anterior chamber during DSAEK in eyes with blebs following trabeculectomy. Methods: This retrospective case series included 34 eyes of 33 patients that developed bullous keratopathy following trabeculectomy. Bleb compression suturing was performed using a 10-0 nylon suture in eyes with an intr

Association between corneal edema and corneal astigmatism after Descemet membrane endothelial keratoplasty.

PURPOSE: To evaluate changes in corneal astigmatism after DMEK and determine the predictive value of preoperative central corneal thickness (CCT) for postoperative astigmatic change. METHODS: Fifty-five eyes with Fuchs' endothelial dystrophy (FED) or pseudophakic bullous keratopathy (PBK) undergoing DMEK were analyzed. Auto kerato-refractometer (ARKRT)-derived anterior corneal astigmatic change was defined as the primary outcome in 55 eyes. Anterior-segment optical coherence tomography (AS-OCT)-derived anterior, posterior, and total corneal parameters were analyzed as secondary outcomes in 25

Morphological Distinction of Descemet's Membrane Protrusions in Bullous Keratopathy and Guttae in Fuchs Endothelial Corneal Dystrophy.

PURPOSE: To characterize dome-shaped protrusions (DSPs), a previously unrecognized alteration of Descemet's membrane (DM) in bullous keratopathy (BK), and to determine how DSPs differ from classic guttae in Fuchs endothelial corneal dystrophy (FECD). METHODS: DM specimens were collected from 75 eyes with BK during endothelial keratoplasty. A subset of eyes with sufficiently dense protrusions (≥50 per field at ×200 magnification) was selected for detailed analyses. Flat-mounted DMs were examined by phase-contrast microscopy, scanning electron microscopy, and immunofluorescence staining. Quanti

Injectable corneal endothelial cell therapy: recent progress, translational barriers, and future directions.

INTRODUCTION: Corneal endothelial dysfunction, most commonly caused by Fuchs endothelial corneal dystrophy (FECD) or pseudophakic bullous keratopathy (PBK), leads to stromal edema and corneal decompensation when endothelial cell density (ECD) falls below 500-700cells/mm². Standard treatment via corneal transplantation, including Descemet stripping automated endothelial keratoplasty (DSAEK) and Descemet membrane endothelial keratoplasty (DMEK), is limited by the need to standardize corneal endothelial cell culture, donor shortages, immune rejection, and technical complications. This review aims

Oxidative Stress-Induced Age-Related Bullous Keratopathy in Corneal Endothelial Cells With Lipid Droplets and Vacuoles.

PURPOSE: We previously demonstrated that intracellular oxidative stress reduces corneal endothelial cells in Tet-mev-1 mice that genetically overproduce mitochondrial superoxide anions. This study aimed to elucidate the physiological processes underlying oxidative stress-induced corneal endothelial cell loss. METHODS: Primary cultures of mouse corneal endothelial cells were established. Intracellular lipid droplets were detected using Lipi-Green, and autophagosomes in live cells were visualized using DAPGreen. Autophagy was assessed by immunostaining and Western blot analysis using microtubul

Late-onset bullous keratopathy after iris-fixated phakic intraocular lens implantation: a case report.

BACKGROUND: This article presents a case report of bullous keratopathy occurring 12 years after implantation of iris-fixated anterior chamber phakic intraocular lenses (AC pIOL), which eventually required Descemet Membrane Endothelial Keratoplasty (DMEK). This represents a much longer period of endothelial decompensation following implantation than that reported in other studies. CASE PRESENTATION: A 44-year-old woman presented with pain and reduced vision which was diagnosed as uveitis; it was unresponsive to topical steroids. Twelve years earlier, she had iris-fixated AC pIOLs (Myopia Artif

Masked Corneal Opacity in Bullous Keratopathy: The Impact of Epithelial Pathology on Surgical Decision-Making-A Case Report.

Bullous keratopathy may lead to severe corneal opacity and impaired visualization of anterior segment structures, complicating surgical qualification for endothelial keratoplasty (EK). We report the case of a 67-year-old male with pseudophakic bullous keratopathy and Fuchs endothelial dystrophy presenting with clinically complete corneal opacity and visual acuity limited to hand motion. Slit-lamp examination and anterior segment optical coherence tomography demonstrated marked epithelial remodeling with a dense plaque-like surface lesion obscuring deeper corneal structures. Corneal Endothelial Progenitors for Ocular Regeneration: Translating Discovery into Clinical Therapies.

The corneal endothelium is essential for maintaining corneal transparency through active fluid transport and barrier function. Corneal cell loss from disease, ageing or surgical trauma underlies a significant proportion of corneal blindness worldwide, with Fuchs' endothelial corneal dystrophy (FECD) and pseudophakic bullous keratopathy (PBK) representing the dominant clinical indications for corneal transplantation. While Descemet's membrane endothelial keratoplasty (DMEK) has substantially improved surgical outcomes, the procedure remains constrained by global donor tissue shortage. Regenerat
