Clinical Challenges in Managing Bilateral Acute Iris Transillumination: Insights From a Case Triggered by Fluoroquinolone Therapy.

Bilateral acute iris transillumination (BAIT) is a rare ocular condition characterized by acute pigment dispersion from the iris pigment epithelium (IPE), resulting in diffuse iris transillumination, atonic mydriatic pupils, and intraocular hypertension. BAIT is often associated with severe photophobia and requires extended medical management. Differentiating BAIT from related conditions such as bilateral acute depigmentation of the iris (BADI) is critical, as BAIT presents with a more severe clinical course and lasting complications. Trigger factors include respiratory infections and fluoroqu

Nanothin Descemet stripping automated endothelial keratoplasty performed on a lacerated cornea after globe rupture: a case report.

BACKGROUND: When performing corneal endothelial transplantation, the presence of low intraocular pressure, abnormal host corneal shape, and septal defects between the anterior and posterior chamber may cause poor graft adhesion. We report our experience with Descemet stripping automated endothelial keratoplasty (DSAEK) using a nanothin graft in a case of bullous keratopathy (BK) in a lacerated cornea after globe rupture-a situation that encompasses these risk factors. CASE PRESENTATION: An 81-year-old man was admitted to our department after he had ruptured his right eyeball following a fall.

Intracellular dark endothelial spots: A new imaging biomarker for the development of bullous keratopathy after cataract surgery.

PURPOSE: Intracellular dark endothelial spots (IDES) on specular microscopy indicate corneal endothelial stress, and can be an imaging biomarker for subsequent endothelial cell density (ECD) loss after endothelial keratoplasty. This study evaluated the risk factors for the development of bullous keratopathy (BK) by comparing low ECD patients with or without IDES before cataract surgery. METHODS: This retrospective study included 106 consecutive patients who underwent cataract surgery with a preoperative ECD of less than 1000 cells/mm2. The patients were divided into 2 groups according to pres

Descemet's Stripping Automated Endothelial Keratoplasty (DSAEK) Management of Bullous Keratopathy After PreserFlo MicroShunt Device: A Case Report and Literature Review.

Bullous keratopathy is a known complication following glaucoma filtration surgeries, and Descemet's stripping automated endothelial keratoplasty (DSAEK) has been reported as an effective management approach in several cases. However, management of bullous keratopathy in eyes with minimally invasive bleb surgery (MIBS) devices presents unique challenges. We report the first successful case of DSAEK in a patient with bullous keratopathy following PreserFlo MicroShunt (PMS) (Santen Pharmaceutical Co., Ltd., Osaka, Japan) implantation, with additional complications of prior vitrectomy for malignan

Endoilluminator-assisted descemet membrane endothelial keratoplasty (E-DMEK) for enhanced trans-corneal illumination in eyes with chronic pseudophakic bullous keratopathy (PBK).

Descemet membrane endothelial keratoplasty (DMEK) is the most desirable corneal transplant surgery as it is associated with the least chances of failure due to endothelial rejection and has the fastest visual recovery among corneal transplants. This technically challenging surgery involves several critical steps such as graft unfolding, positioning and correct orientation. Severe pre-existing corneal oedema limits visibility and prolongs the surgical duration, adversely affecting the success. We present two cases of severe corneal oedema, poor anterior chamber visualisation, wherein we perform
