Spontaneous Artificial Corneal Endothelial Graft Reattachment: Case Report.

PURPOSE: EndoArt, a synthetic endothelial substitute for corneal edema, often requires multiple rebubbling procedures because of high detachment rates. This study reports 2 cases of spontaneous EndoArt reattachment, suggesting a potential alternative management approach. METHODS: Two patients with endothelial dysfunction underwent EndoArt implantation. Both experienced partial detachment postoperatively. Although rebubbling was considered, a conservative approach was taken, leading to spontaneous reattachment. Serial anterior segment optical coherence tomography (AS-OCT) monitored graft adher

Cell Counting and Cell Cycle Analysis of Simple Non-Cultured Endothelial Cell Injection (SNEC-I) Therapy: Characterization for Clinical Translation.

Human corneal endothelial cell therapy has recently emerged as a novel solution to treat corneal endothelial diseases. We previously demonstrated the potential of utilizing non-cultured primary corneal endothelial cells (CEnCs) isolated from donor corneas with low endothelial cell density for simple non-cultured endothelial cell injection (SNEC-I) therapy. This study aimed to develop a robust and semi-automated approach for cell counting, characterize the extent of cellular manipulation, and evaluate the translational workflow. To address this, we evaluated manual and automated cell counting a

Multivariate relationships between graft detachment after DMEK and twelve pre/perioperative factors.

This study aimed to identify pre/perioperative risk factors for graft detachment after Descemet membrane endothelial keratoplasty (DMEK). Hierarchical multivariate analysis was conducted on consecutive adults with Fuchs endothelial corneal dystrophy or pseudophakic bullous keratopathy who underwent pseudophakic- or triple-DMEK in 2014-2022 and were followed for 3 months. Significant graft detachment was defined as exceeding 20% of the graft or affecting the graft center. Twelve pre/perioperative factors were included. Of the 170 eyes (125 patients, median age 73 years, 65% women), 15% displaye

Corneal intrastromal silicone injection in severe corneal decompensation as a last resort for pain management: A case series.

PURPOSE: To describe a surgical approach to treating painful bullous keratopathy using an intracorneal silicone bubble in patients with poor visual potential. METHODS: Two eyes of two patients with painful bullous keratopathy are presented in this paper. The surgical technique consists of two steps. First, a corneal stromal pocket is created using the femtosecond laser. Then, an intrastromal bubble of heavy silicone is injected into this corneal pocket. The pocket is then tightly sutured with a nylon suture. Care must be taken to inject an adequate size silicone bubble that is big enough to r

A Comprehensive Review of the Role of Rho-Kinase Inhibitors in Corneal Diseases.

There is growing interest in the application of Rho-associated protein kinase (ROCK) inhibitors (ROCKI) to the treatment of corneal diseases. ROCK is a key regulator of several cellular processes in the cornea, including cytoskeletal organization, cell proliferation, migration, inflammation, and wound healing. ROCKI, such as ripasudil and netarsudil, enhances endothelial cell migration, and promotes repair in conditions characterized by endothelial dysfunction. These agents also exert anti-inflammatory, anti-angiogenic, and anti-fibrotic effects for wound healing. As such, ROCKI demonstrate pr

Aspergillus flavus Keratitis After Penetrating Keratoplasty.

Fungal keratitis is a rare but severe complication following penetrating keratoplasty (PKP). We report the clinical course, rapid deterioration, and management of a case of Aspergillus flavus keratitis occurring three months after PKP. A 69-year-old woman with Fuchs' endothelial dystrophy developed pseudophakic bullous keratopathy following cataract surgery. She underwent an uneventful PKP. At two months postoperatively, the graft remained clear with a visual acuity of 2/10. Three months after keratoplasty, the patient presented with pain, redness, mucopurulent discharge, and decreased vision.
