Visual prognosis in aphakic bullous keratopathy treated by penetrating keratoplasty: a retrospective study of 73 cases.

Seventy-three eyes with limbus to limbus bullous keratopathy following cataract extraction were followed 24 months to determine the long-term visual prognosis following penetrating keratoplasty. All but one eye required anterior vitrectomy. A manual vitrectomy technique was utilized in all cases. Results obtained were equal to or better than other contemporary literature reports utilizing mechanical vitrectomy techniques.

Combined penetrating keratoplasty and mechanical anterior vitrectomy.

Forty patients with aphakic bullous keratopathy undergoing combined penetrating keratoplasty and anterior vitrectomy using mechanical vitreous instruments were studied for a mean length of 36 months. Seventy percent of the grafts remained clear and thirty percent failed during the follow-up period. Fifty-three percent of those with clear grafts (15/28) obtained a visual acuity of 20/70 or better. In comparison to older methods of anterior vitrectomy, we have found that removal of vitreous with mechanical vitreous instruments offers more effective removal of solid and liquid vitreous, decreased

Penetrating keratoplasty: changing indications, 1947 to 1978.

Corneal buttons submitted to the Estelle Doheny Eye Foundation from 1947 through 1978 were studied. According to a study of 710 corneal specimens processed during 31 years, the leading indications for penetrating keratoplasty were aphakic bullous keratopathy, regrafts, keratoconus, and Fuchs' endothelial dystrophy. Intraocular lenses were associated with 22% of all specimens of aphakic bullous keratopathy in 1974, increasing to 68% of all aphakic bullous keratopathy corneas in 1978. Histopathologic appearance of aphakic bullous keratopathy corneas with or without an intraocular lens was simila

Corneal epithelium in penetrating keratoplasty.

We studied corneal epithelium in 66 patients with bullous keratopathy treated with penetrating keratoplasty using McCarey-Kaufman stored donor corneas. Epithelium was evaluated at times of storage, surgery, and postoperative dressing changes. Epithelium was intact in 43 of the donor corneas at storage, and 23 had 5 to 100% (median, 50%) epithelium missing. At the end of the keratoplasty procedure, 16 grafts had epithelium intact, and 50 had 5 to 100% (median, 20%) epithelium missing. Postoperative epithelial healing time ranged from one to 12 days, with a median of two days. Postoperative heal

Bullous keratopathy accompanying essential iris atrophy: response to lowered intraocular pressure.

We present a patient with essential iris atrophy who was normotensive and yet had corneal edema and bullous keratopathy which cleared upon lowering the intraocular pressure. It is proposed that in this patient the endothelial functional status was so compromised that it could not handle the amount of fluid driven into the cornea by a normal intraocular pressure. A more favorable balance between intraocular pressure and endothelial functional status was obtained by lowering the intraocular pressure with acetazolamide and timolol.

Keratoplasty in aphakic eyes with corneal edema: results in 100 cases with 10-year follow-up.

Penetrating keratoplasties were performed in 100 patients with bullous keratopathy and aphakia. In 85% of cases, edema was due to Fuchs' dystrophy. Cases were divided into eyes with good, fair, and poor prognosis, depending on the presence of glaucoma, anterior segment pathology and corneal vascularization. The three combined groups had 78% clear grafts during the first year, but when results were correlated with the prognostic groups, there were 85% of clear grafts in Group I, 78% in Group II, and 60% in Group III. The number of clear grafts dropped prominently in Groups II and III after the

Penetrating keratoplasty in pseudophakic bullous keratopathy.

We studied 25 consecutive patients who underwent penetrating keratoplasty for pseudophakic bullous keratopathy. Follow-up time averaged 20 months. Ten patients required postimplantation surgical procedures because of complications after lens implantation. The time from lens implantation to onset of corneal edema averaged 18 months in patients without postimplantation surgical procedures and five months in patients with postimplantation procedures. The lens implants were removed in 11 patients at keratoplasty. Corneal clouding caused by graft rejection occurred in three patients, while 22 graft

Endothelial cells in dominant cornea guttata.

A family with autosomal dominant cornea guttata in three generations is described. The investigated pedigree consisting of 25 healthy patients and 6 patients with cornea guttata. A few guttata were already present at an early age and very pronounced corneal endothelial guttata appeared in the middle-aged patients. No other ocular pathology was associated with this condition and the endothelial cell counts were not affected by guttata excrescences. The corneal thickness was normal in the affected and non-affected members of the family while bullous keratopathy did not seem to be associated with

Pars plana surgical approach for various anterior segment problems.

Various anterior segment problems can be approached through the pars plana and treated with instrumentation that has been developed for vitreous surgery. Two cases of aphakic bullous keratopathy due to vitreous touch and three cases of subluxated lenses were successfully treated. Of two cases of advanced, persistent hyperplastic primary vitreous, one showed a successful result.

Keratoplasty in Fuchs' dystrophy and bullous keratopathy.

We studied the results in six cases of penetrating keratoplasty in Fuchs' dystrophy and bullous keratopathy. The prognosis has improved considerably in the last decade because of the advances and improvements in the fields of anesthesia, new drugs, better instrumentation, more refined surgical techniques performed under microscopic control, more meticulous selection of the graft, better knowledge of biologic mechanisms, and progress in the prevention, diagnosis, and management of complications.

Indications, complications and prognosis for repeat penetrating keratoplasty.

A review of 47 repeat penetrating keratoplasties in 29 eyes showed the most frequent indications to be Fuchs' dystrophy, bullous keratopathy, and keratoconus. The most common postoperative complications were edema, glaucoma, and vascularization. Ninety other surgical procedures were performed in 26 of the 29 eyes. Final vision was no worse than 20/300 in 26% of the group. It is suggested that repeat grafting should be continued in the absence of contraindications.

Pseudophakic bullous keratopathy. A clinical-pathologic analysis.

Of 226 patients who had an intraocular lens implanted over a three year period, 6.2% developed bullous keratopathy compared with 2.2% of 90 patients who had a standard cataract extraction performed by the same surgeon. An additional 4.0% of the lens implant patients developed peripheral or intermittent corneal edema. The factors related to the higher incidence of this complication in lens implant patients included corneal touch during implantation, prolonged postoperative iritis and, in cases of mild or peripheral corneal edema without bullous keratopathy, recurrent lens dislocation. The most

Management of surgically dislocated intravitreal lens fragments by pars plana vitrectomy.

We examined 26 eyes (19 of them following phacoemulsification) with surgically dislocated intravitreal lens fragments; 24 eyes were subsequently operated on using pars plana vitrectomy for removal of lens fragments. From this select group of patients we found moderate to severe uveitis, glaucoma, and vitreous opacification present in virtually all cases. Less common complications were retinal detachments (six cases) and bullous keratopathy; the latter problem was found most commonly in eyes subjected to anterior segment maneuvers such as the use of lens loops or vitreous irrigation for removal

Endothelial degeneration and posterior collagenous proliferation in aphakic bullous keratopathy.

In series of 39 corneal grafts, aphakic bullous keratopathy comprised 13 (33%) of the cases. By scanning and transmission electron microscopy, endothelial degeneration and posterior collagenous proliferation were consistent features. In all 13 cases, the patchy loss of endothelial cells occurred diffusely over the entire posterior corneal surface. The remaining endothelial cells had become extremely attenuated to increase their surface area. In nine cases, acellular proliferations of collagenous tissue were interposed between the endothelial cells and Descemet's membrane. These posterior colla

Fate of endothelium in a corneal graft.

An 81-year-old white man with bullous keratopathy received a corneal graft from a 13-month-old donor. The postoperative course has been uneventful. The endothelium of the donor cornea was studied on specular microscopy both before grafting and up to 9 months after surgery. There has been no change in the morphologic characteristics of the corneal endothelium nor has there been any decrease in cell density. The donor endothelium seems to persist for at least 9 months unchanged. This case provides a good illustration since it is not too often that a cornea from a 13-month-old donor is used for g

Keratoplasty for bullous keratopathy with intraocular lens.

The presence of a pseudophakos does not appear to introduce any greater hazard when keratoplasty is required because of corneal edema. This, together with the histories obtained from patients requiring such keratoplasty, strongly suggests that the insult to the corneal endothelium takes place at the time of placement of the intraocular lens in the great majority of cases. Of possible further significance is the fact that five of the sixteen patients who underwent keratoplasty had had secondary procedures to stabilize or to reposition the lens following the original lens implantation. The incre

Central thickness in corneal disorders.

In the single individual the central corneal thickness (CCT) shows only small variations. Therefore CCT has been studied in a number of corneal diseases in order to investigate if this dimension might contribute to the diagnosis or to the understanding of the pathogenesis. Normal CCT was found in hereditary dystrophies with the exception of the macular dystrophy of Groenouw (type II), which showed a significantly reduced thickness. Reduced CCT was found in chronic degenerations of leutic, tuberculous or indefinite nature. Marginal degenerations of Fuchs-Terrien type also showed reduced CCT. Th
