Scanning electron microscopy of intraocular lens and endothelial cell interaction.

Scanning electron microscopic evaluation of two intraocular lenses removed at keratoplasty for pseudophakie bullous keratopathy demonstrated apparently viable endothelial cells adherent to their surfaces. These presumably adhered to the lens implants at the time of their insertion and confirm this as a mechanism for endothelial damage and subsequent corneal edema from intraocular lens implantation.

Optometric management of Fuchs's endothelial dystrophy.

Fuchs's endothelial dystrophy interferes with removal of fluid from the cornea by the endothelium. It progresses through stages of corneal guttata, corneal edema, bullous keratopathy, vascularization, and scarring, and complications such as glaucoma or infection. The optometrist can detect this disorder and manage its early stages as well as provide counseling and proper referral. This paper includes a case study and a discussion of the course and treatment of this disease.

[Thin soft hydrogel lenses of high and low water (author's transl)].

The authors relate the results obtained with very thin soft hydrogel lenses (0,10 mm thickness) of high and low water content: the Vergo-Softcon THX of 70% water-content and the Bauch and Lomb Plano-T of 38%. The most frequent indications encountered are bullous keratopathy, epithelial defects and keratitis sicca. The results are positive in 78,2% of cases with 70% w/c soft hydrogel lenses. Results are given for cases of direct fitting of thin lenses, as well as for cases of fitting after failures of classic soft hydrogel lenses and in cases of exchanges of 40% lenses for 70% lenses. The 70% w

Bilateral versus unilateral cataract extraction: advantages and complications.

Cataract extraction was performed on 734 patients. These were followed up postoperatively for up to 1 year. In 448 cases the extraction was performed in both eyes during 1 operating session (896 eyes) and in 1 eye in 286 cases, a total of 1182 cataract extractions. Preoperatively more than 75% of the eyes in both groups had perception of light only. Postoperatively more than 85% of eyes in both groups achieved a visual acuity of 6/30 or better. The postoperative visual performance of an eye was not influenced by the mode of operation, bilateral or unilateral. The overall incidence of complicat

The phacoemulsification procedure. III. Corneal complications.

Persistent corneal edema is one of the complications of phacoemulsification. Five patients underwent this procedure 6 to 8 months before keratoplasty was performed. In addition to bullous keratopathy, two corneas had corneal scarring due to probe overheating or corneal vascularization of the anterior chamber. Electron microscopy of corneal specimens showed that four cases had Fuch's dystrophy without warts and one had guttata. Endothelial cell destruction varied from lessions of small size (15 to 50 mu) to large abrasions. Cases with severe edema and vitreous adhesion showed retrocorneal membr

Keratoprosthesis: a 12-year follow-up.

A retrospective study of our total experience with implantation of four types of keratoprosthesis in 125 cases was reviewed. Diagnoses in the cases treated included chemical burns, derangement of the anterior segment from disease or injury, aphakic bullous keratopathy, ocular pemphigoid, Stevens-Johnson syndrome, anterior cleavage syndrome, Mooren ulcer, and blast injuries. The visual results in some cases have been gratifying and in others heartrending. Thirty percent of the patients attained 20/15 to 20/40 visual acuity, but at the end of 12 years, only 13% still had this visual acuity. Twen

Intraocular lens surgery on a resident's service.

Twenty-three patients had intraocular lens surgery performed by eight separate ophthalmology residents over an eighteen month period of time. Adequate technical results were achieved in 19 of the 23 cases. Three patients had visual complications directly related to the intraocular lens surgery: two with bullous keratopathy and one with cystoid macular edema. By restricting surgical indications to older patients with good vision in the fellow eye and using general anesthesia in conjunction with the operating microscope, osmotic agents, intracapsular surgery, and close operative supervision, it

Bullous keratopathy (Fuchs' endothelial dystrophy) treated systemically with 4-trans-amino-cyclohexano-carboxylic acid.

Twenty patients with bullous keratopathy (Fuchs' endothelial dystrophy) were treated systemically with the antifibrinolytic drug tranexamic acid. The effect was evaluated by slit-lamp biomicroscopy, measurement of central corneal thickness and determination of visual acuity. The patients subjective complaints were also registered. The duration of the treatment varied from 3 to 16 months. In most cases the treatment was given over several periods with intervening free intervals. In all cases the central corneal thickness decreased and slit-lamp biomicroscopy revealed an improvement. The visual

Use of the Bausch & Lomb Soflens Plano T contact lens as a bandage.

Twenty patients with corneal disease were treated with a recently released soft lens specifically designed for use as a bandage. Patients wore their lenses continuously 24 hr a day. The results were similar to those of previous studies conducted with other lens designs. Improvement was frequent (10 of 12 cases) for bullous keratopathy and corneal graft, burn, or neuroparalytic lesion; limited (1 of 4 cases) for dry-eye syndrome; and absent (4 cases) for irregular cornea and vernal keratoconjunctivitis. No difficulties were found with the concurrent use of medication.

Corneal opacification occurring after phacoemulsification and phacofragmentation.

Four patients developed progressive corneal opacification five to eight months after ultrasonic cataract extraction. Transmission electron microscopy was performed in all patients. Scanning electron microscopy, performed in only one patient, showed radiating folds of Descemet's membrane with pits on its posterior surface as well as focal traumatic disruption probably caused by probe contact. Bullous keratopathy was present in one patient. Two other patients had folds of Descemet's membrane and a thin retrocorneal fibrous layer. All four tumor specimens showed secondary epithelial and stromal c

Corneal oedema and endothelial cell loss after iris-clip lens implantation.

Lens implantation as a form of replacement surgery has some clear advantages over other forms of aphakic correction. The most feared complication of this type of operation is bullous keratopathy. A short-term study was carried out in two comparable groups of patients to assess corneal oedema and endothelial cell loss after simple cataract extraction and after iris-clip lens implantation following intracapsular extraction as a primary procedure. All patients were randomly chosen to have one or other procedure. The results indicate that lens implantation causes greater postoperative corneal oede

New aspects of contact lenses in ophthalmology.

The introduction of the hydrophilic contact lens has been a significant advance in ophthalmology for the correction of ametropia, as well as for the therapy of corneal disease. The number of potential contact lens candidates has been greatly expanded by the introduction of both spin-cast and lathe-cut lenses composed of a variety of individual hydrophilic polymers. Myopia, hyperopia, presbyopia, aphakia and moderate astigmatism can be corrected with a reasonable degree of success with the present lenses. Even in keratoconus hydrophilic lenses offer a nonsurgical alternative, especially when co

Bullous keratopathy following lens extraction in eyes with acute glaucoma due to intumescent cataract.

Nine cases are presented, six of them in detail, which suggest that there is a high risk of bullous keratopathy developing when the lens is removed for the treatment of acute glaucoma due to an intumescent cataract. In some cases the keratopathy is due to detachment of Descemet's membrane from the cornea. Acute glaucoma due to an intumescent cataract should be treated medically or by peripheral iridectomy. The lens should be removed only after the intraocular pressure has been normal and the cornea free of edema for at least 6 weeks.

Hereditary corneal dystrophy in the Manx cat: a preliminary report.

A progressive, apparently inherited corneal dystrophy is described in an inbred line of Manx cats. Initial changes in the cornea are seen at four months of age and characterized by anterior stromal edema. Progressive worsening of the condition produces severe bullous keratopathy with eventual breakdown of both epithelium and stroma. Light microscopic and ultrastructural studies in the advanced disease state revealed marked edema of the corneal stroma, disintegration of collagen material, and the formation of epithelial bullae. Ultrastructural evidence shows a normal endothelium to be present.

Aphakic bullous keratopathy treated with prosthokeratoplasty. An analysis of 34 consecutive cases.

Thirty-four eyes with advanced aphakic bullous keratopathy were treated with the Cardona keratoprosthesis as a primary procedure. There were three extrusions (9%), two of which were associated with infections; two other conditions were worsened (6%) as a result of a late permanent vitreous hemorrhage or possibly an infection. Twenty-eight eyes (82%) showed a notable improvement during follow-up periods that ranged from 6 to 72 months. The advantages and limitations of this technique are compared to those of penetrating keratoplasty.

Changing indications for keratoplasty.

We conducted a clinical and pathologic review of 1,057 recipient corneal buttons on file at the Wilmer Institute, to assess the incidence of corneal conditions treated with keratoplasty, and how these have changed (1941-1973). Regrafting was the most frequent indication for keratoplasty in the last ten years in this series. Some corneal diseases, such as inflammatory conditions, decreased in relative importance as an indication for keratoplasty, while others such as aphakic bullous keratopathy (including vitreous touch) and Fuchs' endothelial dystrophy increased.

[Therapeutic problems in postoperative keratopathy (author's transl)].

Keratopathy following intraocular surgery is usually a neglegible transitory lesion which does not require special treatment. An intractible bullous keratopathy frequently develops in cases of endothelial dystrophy. Other causative factors leading to postoperative keratopathy are excessive operative traumatisation of the cornea, vitreocorneal adhesions, athalamia, detachment of Descemet's membrane or epithelial down-growth. For conservative treatment of postoperative keratopathies topical application of corticosteroids, acetylcholine, vitamine-A-ointments, hyperosmotic agents, soft contact len

[Metaherpetic keratitis clinical and virological findings (author's transl)].

2,277 specimens from 901 eyes were cultured for herpes simplex virus (HSV). 161 of 391 herpes-diseased eyes yielded HSV. The clinico-virological correlation led to a simple diagnostic and therapeutic scheme which is applicable by the ophthalmologist in his office without virological confirmation: 1. Superficial viral herpes (dendritic keratitis and allied disorders), HSV-isolating rate 96%. 2. Stromal herpes (disciform edema, different types of interstitial herpetic keratitis), only sporadical findings of HSV in the lacrimal fluid. The rate of virus-recovery increases, however, when an interst

Alterations in corneal morphology following thermokeratoplasty.

Fifteen corneal buttons were removed at keratoplasty from patients with keratoconus who had previously undergone thermokeratoplasty (TKP). The buttons were studied with light microscopy, in an attempt to analyze the type of changes produced by TKP. The principal histologic findings in patients with retarded epithelial healing (8 of 15) included epithelial thinning, bullous keratopathy, thickening of the epithelial basement membrane, and frequent destruction of Bowman's membrane. More severely affected patients showed asceptic stromal necrosis and fibrinous iritis with hypopyon. A poor visual r

[Special indications for the use of soft contact lenses as a drug-release-system (author's transl)].

Research has been performed, both experimentally and clinically, to establish the value of the association of soft contact lenses and some types of eye drops. The use of soft contact lenses with eye drops may be useful in some special cases: a) more prolonged and more sustained effect compared with the usual way of administration of eye drops (especially antiglaucomatous substances, antimetabolites, mydriatics); b) possibility of reducing the concentration to avoid local discomfort or systemic side-effects, without loss of their effectiveness on the eye conditions to be treated.
