Corneal thickness was measured on awakening in both eyes of ten subjects who slept with a silicone elastomer lens in one eye only. The mean corneal edema level with the silicone lens after eight hours of sleep was 2.0 +/- 2.0% and was significantly lower (p less than 0.01) than the level in the eye that did not wear a lens (3.6 +/- 2.1%). This indicates that the oxygen level supplied to the anterior cornea during sleep with silicone elastomer lenses is higher than without a lens. Because of their many advantages, posterior chamber (PC) intraocular lenses (IOLs) were sutured to the posterior iris (in the absence of a lens capsule) in combination with penetrating keratoplasty for aphakic and pseudophakic corneal edema in 53 cases. A controlled, double-masked, randomized experiment was conducted on 9 human subjects, to determine whether there is an intersubject difference in the corneal swelling response to anoxia. Thirty-three patients who had corneal damage from the original corneal surgery and incomplete vitrectomy and who had a vitreous incarceration were divided into two groups. In one group, Nd:YAG laser therapy was performed without using a viscoelastic material in the anterior chamber. Topical application of the H2-histamine receptor agonist, dimaprit (S-[4-N,N-dimethylaminopropyl]isothiourea), produced eosinophil chemotaxis into the anterior segment of rabbit eyes only when an H2-antagonist was co-administered. Nordimaprit (S-[4-N,N-dimethylaminoethyl]isothiourea), a structural homologue of dimaprit that lacked activity at histamine receptors, produced eosinophil chemotaxis whether or not an H2-antagonist was co-administered. Onset of eosinophil chemotaxis began after 2 or more days of treatment, and was accompanied by corneal edema, opacification, and ocular inflammation.

A 38-year-old man previously had a cutaneous malignant melanoma excised from his right shoulder. Chandler's syndrome is a particularly rare form of unilateral secondary glaucoma, in which--besides other symptoms--corneal epithelial edema occurs even when intraocular pressure rises slightly to above 20 mmHg. For 3 years now the authors have had a patient under observation whose condition corresponds to Chandler's syndrome. A remarkable feature of this case is that corneal edema occurred at physiological pressure levels. The symptoms and course are described; a brief differential diagnostic review is given and therapeutic possibilities are pointed out.

Among 149 consecutive infants with ophthalmia neonatorum in Nairobi, Neisseria gonorrhoeae was recovered from 43%, Chlamydia trachomatis from 13%, and both microorganisms from 4%. Fifty-three patients with pseudophakic bullous keratopathy (PBK) were treated with: Corneal transplant, Removal of the original intraocular lens (IOL), Replacement with a sutured-in posterior chamber IOL. Vitrectomy was performed in all but two cases. Eighty-eight per cent of the corneal transplants were clear and the vision was 20/200 or better in seventy-five per cent. Visual results are equal to other reported series where over one-third of the original IOLs were removed at the time of keratoplasty and were not replaced. Silicone oil can dissolve about seven times more oxygen that can water. Seven patients who had a mean preoperative intraocular pressure of 40 mm Hg (range, 34 to 52 mm Hg) underwent combined keratoplasty and trabeculectomy. This resulted in an average postoperative intraocular pressure of 23.7 mm Hg. Five of seven eyes had well controlled intraocular pressures throughout the postoperative period (mean follow-up, 16.1 months). This approach can achieve a satisfactory result while avoiding some of the complications of other forms of glaucoma therapy.
