Four cases of bacterial endophthalmitis occurred after more than 2,800 closed vitrectomies. Despite vigorous antibiotic therapy, all four eyes were lost. The long term results of grafting for Fuchs' endothelial dystrophy are reported in 25 patients with 33 penetrating keratoplasties, with a mean observation period of 50 months, 33% of the cases showed a visual acuity of 0.5 or better, while 42% had a visual acuity less than 0.1. Human donor corneas were used for penetrating keratoplasty in one eye of each of 12 rhesus monkeys. In six animals, a 9.5-mm cornea was sutured into a 9.0-mm recipient bed by means of interrupted 10-0 nylon sutures. Six other animals received a 6.5-mm cornea in a 6.0-mm bed. Corneal edema developed in seven aphakic eyes of six patients (three men and three women ranging in age from 60 to 88 years) who used daily- or extended-wear hydrophilic and silicone contact lenses. Anesthetic complications in 500 consecutive operations for congenital heart disease were studied and their contribution to mortality and morbidity were assessed. The great majority of the cases were young and sick children in ASA Class III, IV, or V with complex congenital heart disease. Anesthetic mortality was zero and hospital mortality was 6.3%. Seven patients presenting with acute corneal stromal edema without prior surgery, trauma, ocular disease or known exposure to noxious agents are reported. A high water content lens (Scanlens 75, +14.5 dioptres) and a low water content lens (Bausch and Lomb Soflens H3, +14.5 dioptres) were compared with respect to corneal thickness response after 6 hours of wear in a provocative test on 12 aphakic eyes. 282 cataract patients were operated and a Worst Medallion 2-loop pseudophakos was implanted. The material consisted of 154 males, 128 females, mean age 58.8 years, range 3-88 years. 200 had senile, 35 pre-senile, 31 traumatic and 16 some other type of cataract. 247 were intracapsular, 35 extracapsular extractions. 87.6% of all cases had a vision of greater than or equal to 0.5; if 8 cases with non-implant dependent complications are excluded, the rate is 90.1%. In a 79-year-old woman with temporal arteritis, an attack of corneal edema coincided with an ischemic attack of the anterior eye segment. Specular microscopy 17 months after the acute attack demonstrated a 72% endothelial cell loss on the side with previous edema as compared to the other side without corneal involvement. The development of endothelial damage is assumed to be caused by ischemia of the endothelial cells during the acute phase, which, in turn, caused the corneal edema.
