Three adult rhesus monkeys were subjected to 2 and 24 hours of polymethylmethacrylate (PMMA) contact lens wear. The induced corneal changes were examined with the electron microscope. Mild epithelial oedema as well as early degenerative cell changes was present already after 2 hours' wear. Rigid lens wear for 24 hours produced more severe oedema and cell alterations together with premature cell loss and ultimately, in areas of lens bearing, corneal denuding. Intra and extra capsular cataract surgery are discussed with regard to advantages and problems. Particular consideration is given to modern microsurgical extracapsular technique. Analysis of corneal complications, cystoid macular oedema and retinal detachment suggest that the present evidence does not justify a change to extracapsular technique, other than to give support to a posterior chamber intraocular lens. It is pointed out that for many surgeons, such a change of technique may cause more problems than it prevents.

Central corneal thickness changes during the wearing of hydrogel lenses under closed-eye conditions were measured for two subjects. Each wore an investigational ultrathin Bausch & Lomb Soflens contact lens (series X-176) on one eye only under closed-eye conditions for periods ranging from 30 min to 6 hr. The other eye served as a control. Corneal thickness of the control eyes increased gradually and reached a maximum increase of about 4% at the end of the 4-hr wearing trial. Epithelial cysts in the anterior chamber are treated only if they obstruct the pupil or cause other serious complications. A closed-eye method is used in aphakic eyes to collapse the cyst and devitalize the epithelial cells using cryotherapy without incising the cyst wall. In phakic eyes, the cyst is dissected away from the cornea and the partially excised through an open-sky approach followed by transscleral cryotherapy to devitalize epithelial remnants. We conducted a retrospective longitudinal cohort study of 82 penetrating keratoplasties performed to treat herpes simplex keratitis. In 53 cases (65%) there was extensive corneal neovascularization or stromal ulceration and active intraocular inflammation, making the prognosis in these cases poor. Only 32 grafts (39%) were clear at the time of our analysis. Patients with active inflammation, active ulceration, perforation, or extensive neovascularization had particularly poor success rates. Exposure to ammonia gas causes severe tissue damage to skin, lungs, and eyes. Toxic effects, both immediate and delayed, can alter the response to other injuries. We report a 14-year-old boy who suffered multiple trauma in a vehicular accident and at the same time was exposed to anhydrous ammonia. He exhibited severe pulmonary and ocular damage in addition to other severe injuries, and despite aggressive treatment, died of respiratory failure.

Four patients with the congenital rubella syndrome had keratoconus and evidence of acute or previous corneal hydrops. All four of the patients vigorously rubbed and poked their eyes. The keratoconus and acute corneal hydrops in these patients probably resulted from chronic traumatizing mannerisms common in other patients with mental retardation and are not specific for the congenital rubella syndrome.

Transfixing keratoplasty was performed in a patient seven years after a lens extraction, following the appearance of corneal edema with hypertony and proliferation of a retrocorneal veil. Subjects wore hydrogel lenses of two different thicknesses (0.07 mm and 0.18 mm) on one eye and no lens on the other eye during 3-hour periods of eye closure. They developed more corneal edema (12.7%) with the thicker lenses than with the thinner lenses (7.9%) and more corneal edema with the thinner lenses than with no lens (0.9%). The results suggest that hydrogel lens thickness would have to be less than 20 nm to reduce corneal edema to levels that develop when the eyes are closed without a contact lens.

Six cases are reported of detachment of Descemet's membrane as a complication of the extraction of 1253 cataracts--a frequency of 0.5%. Corneal edema was found in 24 of 43 eyes with anterior segment complications of intraocular lenses. Keratoplasty was performed in 20 eyes. Three eyes required other surgical therapy for edema while medical treatment of edema was done in one case. Thirty-two lenses were removed because of pain, inflammation, bleeding or glaucoma. Nineteen of these were removed at the time of keratoplasty. Three eyes were enucleated because of absolute glaucoma, retinal detachment and endophthalmitis.

Three eyes of three patients are reported with ocular hypotension and mature, white cataracts secondary to other ocular disease. Ocular pressures rapidly rose to and remained at normal levels with recovery of excellent vision in all three eyes following cataract extraction, with follow-up periods of 16 to 28 months. Thus a hypotensive eye with a mature, white cataract secondary to other eye disease is not necessarily hopeless, and hypotony is not a contraindication to cataract extraction in such patients.

A review of 47 repeat penetrating keratoplasties in 29 eyes showed the most frequent indications to be Fuchs' dystrophy, bullous keratopathy, and keratoconus. The most common postoperative complications were edema, glaucoma, and vascularization. Ninety other surgical procedures were performed in 26 of the 29 eyes. Final vision was no worse than 20/300 in 26% of the group. It is suggested that repeat grafting should be continued in the absence of contraindications.

The complications of therapeutic hydrophilic contact lens wear are systematically outlined. Hypoxia is the major cause of initial patient failure as well as the long-term problems of corneal edema, neovascularization, and susceptibility to infection. The common difficulties with lens breakage, lens deposits, poor lens retention, and other entities are discussed. Suggestions for avoiding and treating various complications are given, including updated indications and contraindications for fitting these lenses. Careful patient selection and meticulous follow-up are essential.
