A patient with Terrien's corneal marginal degeneration and a previous trabeculectomy had planned extracapsular cataract extraction with intraocular lens implantation. A corneal incision was made with a diamond blade on the degeneration furrow superiorly and was closed with a 10-0 nylon double-running suture with keratoscopic control.

BACKGROUND: Climatic or chronic actinic keratopathy is an important corneal degeneration occurring after prolonged climatic exposure. The advanced stages of disease are confined generally to tropical or arid localities (including the Arctic) with high levels of sunlight. The preliminary report presents the results of application of ozone preparation in form of ophthalmic drops in 89 patients (134 eyes). This preparation has been used in virus conjunctival and corneal diseases as well as in corneal degenerations after inflammatory conditions and chemical burns. One observed as acceleration of the regeneration processes in active inflammatory conditions of the anterior eye segment; instead the cases of leucoma and corneal opacities did not show any clinical effect.

Crescent-shaped, deep corneal scars were observed in seven (39%) of 18 patients with pellucid marginal corneal degeneration. The scars most likely represent a degenerative change related to the precipitous alteration in curvature of the cornea in that area.

Keratosis follicularis spinulosa decalvans (KFSD) is a rare X-chromosomal disorder. It consists of follicular hyperkeratosis of the skin, scarring alopecia of the scalp, absence of the eyebrows, and corneal degeneration. In a retrospective, clinico-histological study, 84 corneas from 73 patients with diagnosed Fuchs' dystrophy (FD) were examined after penetrating keratoplasty. Two patients with advanced pellucid marginal corneal degeneration (PMCD) received bilateral epikeratoplasty grafts ranging in diameter from 9 to 12 mm. Corneal astigmatism was greatly reduced in every case and the possibility of resuming contact lenses for daily wear, obtained after surgery, provided patients with a satisfactory best corrected visual acuity. A series of corneal degeneration similar to Labrador keratopathy occurring in the Mongol nationalities is reported. Epidemiological survey indicated the incidence to be 8.11%, mostly among people over 30 years, and to increase with age. The majority of cases were bilateral and asymptomatic, showing proteinoid guttate opacities in the superficial layers and lower quadrants of the cornea. The etiology was believed to be related to altitude, cold windy and dusty environment, and probably alcohol.

Fifteen eyes in 12 patients with Salzmann's nodular corneal degeneration developed symptoms of recurrent erosion, decreased vision, or both. A history or evidence of ocular surface disease was present in 14 of 15 eyes. Removal of the nodule and accompanying pannus or corneal epithelial basement membrane degeneration, provided symptomatic improvement in all patients.

Three keratoplasties were carried out on two patients because of nodular degeneration of the cornea. Over a 14-year period from 1974 to 1988, 12 eyes of 11 patients with pellucid marginal corneal degeneration underwent penetrating keratoplasty. Peripheral corneal thinning required a large eccentric graft in each case. Follow-up ranged from one to eight years (mean, three years). One graft failed because of a persistent epithelial defect with keratolysis of the wound. Although endothelial allograft rejection was common, occurring in seven of 11 (64%) clear grafts, no graft failed because of rejection.
