The authors present material collected in the course of 10 years of applying the surgical technique of cataract extraction by means of a Cavitron-Kelman model 8000 V. The age of the patients oscillated between 4 months and 80 years. Phacoaspiration was performed in 4416 eyes and phacoemulsification in 2195 eyes. A successful corneal wedge resection was performed to correct the visual impairment in the left eye of a 30-year-old male who suffered from bilateral pellucid marginal degeneration. The patient had visual impairment in both eyes; it was worse in the left. Best corrected visual acuity in the left eye before surgery was 20/800 with 15.5 diopters (D) of corneal astigmatism. Sixty-two cases of corneal amyloidosis were studied. The median age of the 48 men and 14 women was 66 years. Corneal calcification can be broken down into two distinctly different types: calcific band keratopathy and calcareous corneal degeneration. Both have been described in patients with ocular inflammatory disorders, with elevated serum calcium, or following ocular surgical procedures. This is the first case report of corneal calcification in a donor corneal button following penetrating keratoplasty, and it closely resembled calcareous corneal degeneration.

Report on a 47-year-old woman patient in whom an eccentric tumorlike corneal change was found in the left eye. Only after penetrating keratoplasty was the exact diagnosis--a primary isolated corneal amyloidosis--established, by light microscopic examination. The graft remained clear. Familial and systemic amyloidosis were ruled out by further examinations. The prognosis is favorable.

We examined biochemically the lysosomal enzyme activities in tear fluids from patients with mild myopia, senile cataract, and Terrien's marginal corneal degeneration. Tear acid phosphatase activities in Terrien's degeneration were almost the same as those in mild myopia and senile cataract, while those of N-acetyl-beta-D-glucosaminidase in Terrien's degeneration were higher. The high activity of tear N-acetyl-beta-D-glucosaminidase may be derived from the lacrimal gland and infiltrate histiocytelike cells in Terrien's marginal corneal degeneration.

Labrador keratopathy (LK) is an acquired corneal degeneration thought to be caused by chronic exposure to solar irradiation. Reports so far suggest that it is a progressive or at least a stationary condition. There are no detailed reports on recommended therapy. A 15-year-old female showed classical signs of Terrien's corneal degeneration including peripheral corneal thinning, vascularization, lipid deposition, and ectasia. She also showed episodes of conjunctival inflammation and perforation following minor trauma, all in the presence of an intact epithelium.
