Influence of inflammatory and metabolic factors on keratoconus risk: a causal inference analysis.

BACKGROUND: Keratoconus is a complex disease of the cornea, in part caused by environmental exposures whose nature is not fully understood. METHODS: This study relied on previously published results from genome-wide association studies of European ancestry. Summary statistics of available genome-wide association studies on atopy, chronic inflammatory disease and serum glucose levels; results were paired with genomic association results for keratoconus into a well-powered and hypothesis-driven Mendelian Randomisation to statistically test the causal role that may have over keratoconus risk. Sectoral and Layer-Specific Corneal Thinning in Thyroid Eye Disease: A Cross-Sectional Study.

PURPOSE: Corneal thickness in patients with thyroid eye disease (TED) and the layer or specific region of the cornea primarily affected by corneal thickness changes remain unclear. This study aimed to examine and compare the differences in corneal thickness across various layers in patients with TED and a control group, focusing on identifying the specific corneal region where these changes are most pronounced. METHODS: This cross-sectional retrospective study included 57 eyes from 29 patients with TED and 18 eyes from 9 control participants. Basic eye examinations and corneal topography meas

Early-stage keratoconus in a case of oculodentodigital dysplasia.

Oculodentodigital dysplasia (ODDD) is a rare genetic disorder caused by mutations in the GJA1 gene, which encodes connexin 43 (Cx43). The condition presents with a broad phenotypic spectrum including ocular, dental, craniofacial, neurological, and skeletal abnormalities. We report the case of a 24-year-old female patient with genetically confirmed ODDD, carrying the c.226C > T (p.Arg76Cys) variant in the GJA1 gene, who presented with compatible clinical signs. Ophthalmologic evaluation revealed epicanthal folds, microcorneas, nystagmus, hyperprolate corneal profiles, and corneal topography con

Epithelial thickness and tomographic characteristics in central keratoconus: A comparative analysis of cross-linked and untreated eyes.

PurposeTo evaluate epithelial thickness (ET) and corneal tomographic parameters in early-stage keratoconus (KC) patients with central cones, comparing eyes with and without a history of corneal collagen cross-linking (CXL).MethodsThis retrospective cross-sectional study included patients with early-stage keratoconus (Grade 1-2) and central cone localization diagnosed between 2020 and 2024. Central cones were defined by a maximum keratometry value within the central 3 mm zone, and one eye per patient was randomly selected. Eyes were classified as cross-linked (CXL+) or untreated (CXL-), with al

Hormonal Modulation of Keratoconus: A Systematic Review and Screening Strategy for At-Risk Populations.

TOPIC: Keratoconus (KCN) is a progressive corneal ectasia with poorly understood etiology and risk factors leading to inadequate screening methods for at-risk populations. One potentially vulnerable population includes individuals experiencing fluctuations in hypothalamic-pituitary-gonadal (HPG) axis hormones. This systematic review aims to evaluate the association between HPG-axis hormonal fluctuations and the development or progression of KCN. CLINICAL RELEVANCE: Elucidating the relationship between HPG-axis hormone fluctuations and KCN may enhance screening strategies, facilitate early det

Comparison of corneal thickness measured using the new Revo FC optical coherence tomographer and the pentacam Scheimpflug-based imaging system.

PURPOSE: Corneal thickness is a key measurement in ophthalmology, playing a significant role in diagnosing and monitoring conditions such as keratoconus, corneal dystrophies, and glaucoma, as well as in the preoperative evaluation for refractive surgery and keratoplasty. This study aimed to compare corneal thickness measurements obtained using a Scheimpflug-based imaging system with a new optical coherence tomographer. METHODS: This prospective study enrolled Polish patients with healthy eyes. Measurements were performed using the two devices, the Oculus Pentacam and Optopol Revo FC, in a rand

Sex Hormones and Keratoconus: In Search of the Link.

Background: Keratoconus (KC) is the most common ectatic corneal disorder, causing progressive corneal deformation, visual impairment, and reduced quality of life. Although KC pathogenesis is multifactorial, the contribution of systemic factors, including hormonal regulation, remains incompletely understood. This study aimed to investigate the role of sex hormones and gonadotropins in KC in a predominantly Greek population. Methods: We recruited 105 KC patients and 71 healthy controls (HC). Plasma levels of luteinizing hormone (LH), follicle-stimulating hormone (FSH), estradiol (E2), prolactin

Sexual dimorphism in keratoconus: transcriptomic and hormonal mechanisms underlying stromal remodelling.

BACKGROUND: Keratoconus (KC) is a vision-threatening condition with a higher prevalence and earlier onset in males than in females. The study aims to investigate sex-associated transcriptomic features of KC and assess whether sex hormones modulate stromal remodelling. METHODS: We performed whole-transcriptome sequencing of human corneal tissues from patients with KC and matched controls (n = 20; five males and five females per group), followed by weighted gene co-expression network analysis, Gene Ontology, Kyoto Encyclopaedia of Genes and Genomes enrichment, and competitive endogenous RNA (ce

The Critical Role of Oxygen Supplementation in Epithelium-On Corneal Cross-Linking: A Narrative Review.

Keratoconus is a progressive corneal disorder characterized by thinning and steepening of the cornea, leading to visual impairment and reduced quality of life. Although epithelium-off corneal cross-linking has emerged as the gold-standard therapy, it can be associated with postoperative pain, delayed healing, and increased risk of complications due to removal of the corneal epithelium. To overcome these limitations, epithelium-on cross-linking was developed as a less invasive alternative. In epithelium-on cross-linking, three essential components must be present to maximize the photochemical r

Clinical burden of dry eye disease in patients with ocular graft versus host disease.

BACKGROUND: Ocular graft-versus-host disease (oGVHD) is a frequent complication after hematopoietic stem cell transplant, with dry eye being the most common presentation. Patient experience with dry eye from oGVHD may negatively affect quality of life. The purpose of this study was to evaluate patients' perspectives on the symptoms, treatments, and care burdens associated with dry eye from oGVHD. MATERIALS AND METHODS: An electronic questionnaire was sent to patients with dry eye asking about their diagnoses, symptoms, treatments, and experiences associated with dry eye. RESULTS: Of the 639

PURPOSE: To report a pediatric case of post-corneal cross-linking (CXL) infectious keratitis successfully managed with early initiation of topical losartan to prevent corneal haze and stromal fibrosis. CASE REPORT: A 14-year-old boy developed infectious keratitis in the right eye two days after undergoing bilateral epithelium-off CXL for advanced keratoconus. Slit-lamp examination revealed a mid-peripheral ring-shaped stromal infiltrate with overlying epithelial defect, edema, and posterior fibrin. Intensive empirical antimicrobial therapy was initiated.
