A 43-year-old man was diagnosed as a case of near-mature senile cataract, chronic simple glaucoma, and pterygium both eyes. Pterygium shaving and extracapsular cataract extraction were done together. Thick aftercataract was diagnosed with moderate iridocyclitis on fourth postoperative day. Treatment was started with atropine and steroids, but without any relief. A second operation of curette evacuation of aftercataract was done to rule out and treat the presumed lens induced uveitis, but the condition further deteriorated. In next four days the eye had to be evacuated. On culture material, Sta

Coccidioidomycosis has been recognized as a common systemic disease since the late 1930's. The occurrence of ocular lesions associated with the systemic infection is uncommon. The anterior segment manifestations appear to be a mild hypersensitivity response; the disease may present as a chronic granulomatous iridocyclitis. In the posterior eye, the manifestations may range from asymptomatic focal chorioretinitis (somewhat similar to the peripheral atrophic lesions of the ocular histoplasmosis syndrome) to a fulminating granulomatous process involving the entire eye. The ocular lesions can occu

Seven out of 22 children with monarticular juvenile rheumatoid arthritis (MJRA) developed involvement of other joints between six months and three and a half years from the onset. In the other 15 patients the disease has remained monarticular for between one and 16 years (mean six years). Chronic iridocyclitis was seen in three of the five boys, two with antinuclear antibodies. Children with MJRA and antinuclear antibodies should have periodic ophthalmic assessment. Synovial biopsy was of value primarily in excluding other cases of arthritis, but there was only limited correlation between the

The problems of Behçet's disease in Japan have been reviewed with particular emphasis on the ophthalmologic aspects: the historical background for the Japanese works, diagnostic critieria, epidemiology, some statistics, ocular symptomatology, ocular histopathology, etiology, pathophysiology, and treatment. Behçet's disease is the most frequent entity in endogenous uveitis in Japan. Patients are found throughout the country, and the prevalence rate averages seven to eight per 100,000 population: the rate is higher in the northern than in the southern districts. The diagnosis is made on the basi

Two calves were treated with 15 micrograms/kg body weight of 1alpha-hydroxycholecalciferol by intramuscular injection on four occasions at seven-day intervals. Anorexia and reduced water consumption persisted for 48 h after each treatment. No clinical signs of iridocyclitis or any other lesions of the eyes were present at any time either macroscopically or microscopically. After the first treatment serum GOT and GD activities increased, serum AP activity fell, serum concentrations of calcium and inorganic phosphate increased, and magnesium concentrations decreased. The reduced serum magnesium

We determined the incidence of immune complex increases, the nature of immune complexes, and the correlation between the immune complex level and clinical uveitis status in patients with chronic iridocyclitis and chronic diffuse uveitis. Ten of 16 patients with chronic iridocyclitis had increased levels of immune complexes, and this correlated with disease activity. Immune complexes were detected in ten of 12 patients with chronic diffuse uveitis. These complexes have a molecular weight of approximately 2 X 10(6) daltons. The antibody component of immune complex is IgG; however; the antigenic

A study was made, in co-operation with several gastroenterology and rheumatology centres, of the clinical and genetic characteristics (HLA B27) of 50 patients suffering from both inflammatory bowel disease (38 Crohn's disease (CD), 12 ulcerated colitis (UC)) and ankylosing spondylitis (AS), the latter diagnosis being established according to the New York criteria. 20 CD (52.6%) and 8 UC (66.7%) patients were HLA B27 positive. The presence of HLA B27 was studied in relation to clinical parameters, such as first occurrence of symptoms of AS or inflammatory bowel disease (IBD), a history of perip

Since 1975 the operation of implantation of a scleral strip (10 mm long and 2 mm wide) has been performed on 75 glaucomatous patients. The following variants of surgical technique were used: (1) implantation of an autoscleral strip into the supraciliary space, (2) implantation of an autoscleral strip combined with cyclodialysis and (3) implantation of a homoscleral (cadaver) strip combined with cyclodialysis. The first technique was employed in 12 cases. The intraocular pressure was controlled by surgery alone in only four eyes, though a decrease in the level of the intraocular pressure and an

A diagnosis of Mixed Connective Tissue Disease (MCTD) had been made in five juveniles in the past two years. All have antibodies to the ribonucleoprotein (RNP) component of extractable fluorescence pattern. No child has had life-threatening complications, though three have required steroids to control their symptoms. Antibodies to ENA were looked for in the other sub-groups of Juvenile Chronic Arthritis (JCA); only two of 75 patients with chronic iridocyclitis and antinuclear antibodies had weakly positive tests.

Juvenile rheumatoid arthritis is a diverse group of diseases that includes systemic-onset, polyarticular, and pauciarticular types. Appreciation of the different types and their hallmarks is particularly important to accurate diagnosis, which is determined by exclusion of other known disease entities in children with chronic arthritis (more than three months' duration). Therapy should be directed at the arthritis per se (synovitis), at the extra-articular manifestations, and at the whole child. Salicylates provide the most satisfactory control of the arthritis per se and of the systemic manife

Careful study of inflammatory and immunologic parameters in JRA and other connective tissue diseases of childhood has contributed to our current understanding of these diseases. Examination of serum for acute phase reactants, and antibodies to immunoglobulin antigens and nuclear antigens, combined with radiologic evaluation of symptomatic joints and, when indicated, examination of joint fluid or synovial membrane, may confirm the diagnosis of JRA. Changes in inflammatory indicators can be used as an index of adequacy of treatment. An ANA positivity in the child with pauciarticular arthritis sh

A patient with psoriasis and anterior uveitis is described. In her family the two entities of iridocyclitis and psoriasis occurred separately in two generations and appeared associated in the third generation. Tissue typing of the family revealed HL-A 27 antigen in two members and HL-A 17 in the other two members. The possible relevance of HL-A antigens to the two clinical entities is discussed.

HLA antigen frequencies and antinuclear antibody titres were studied in 43 patients with juvenile chronic iridocyclitis. We did not detect an increased incidence of HLA-B27 in patients having chronic iridocyclitis associated with juvenile rheumatoid arthritis or in patients with "chronic iridocyclitis in young girls". Both groups of patients had a statistically significant increased incidence of antinuclear antibodies. However, these antibodies were detected more frequently in patients with chronic iridocyclitis associated with JRA than in patients with "chronic iridocyclitis in young girls".

We have studied the antigen frequencies in a group of adult patients with acute and chronic iridocyclitis and chronic, cyclitis. In patients with iridocyclitis a statistically significant increased incidence of HLA-B27 was noted, even in those patients without joint or systemic disease. Patients with chronic iridocyclitis also were noted to have an increased incidence of B27; however, those patients with chronic cyclitis did not. On the basis of HLA-B27 studies there appears to be a fundamental difference between adult and juvenile iridocyclitis.

Corticosteroids, although potent suppressors of symptoms and signs of disease, are not effective in curing rheumatoid arthritis in adults or children. Prolonged use of corticosteroids is attended with multiple serious side effects. In JRA, the outlook for most children is good; potentially dangerous drugs should be avoided if at all possible. The indications for corticosteroid usage in JRA might be summarized as: Severe systemic disease unresponsive to salicylates Chronic iridocyclitis unresponsive to topical steroids. Only very rarely for severe arthritis per se; and then with the realization

In the present study we have reconfirmed an earlier observation of the association of iridocyclitis with limited joint involvement in children with JRA. Children who develop iridocyclitis are predominantly girls whose sera are positive for antinuclear antibodies. Iridocyclitis was found to be asymptomatic at onset. Only periodic slit lamp examinations will detect the majority of cases. Iridocyclitis began a median of 2 years after onset of arthritis. Although most episodes occurred within 7 years after onset of arthritis, the risk of this development may never be absent in these children, even

A retrospective study comparing 14 patients with juvenile rheumatoid arthritis (JRA) complicated by chronic iridocyclitis (Group I) with an equal number of JRA patients without this complication (Group II) was conducted to determine which factors might aid in the earlier diagnosis of chronic iridocyclitis. Eleven patients in Group I had oligoarticular disease compared to 5 in Group II. Antinuclear antibody was found in 13 patients in Group I compared to only 2 in Group II. Only one of nine patients tested in Group I for the histocompatibility antigen W-27 was positive. The patient with oligoar

Clinically, idiopathic uveitis may be associated with chronic active hepatitis B. In searching for a possible cause of the uveitis in 6 patients having concurrent chronic iridocyclitis and chronic active hepatitis with serum Australia antigen (AA), the aqueous humor from each patient was analyzed for AA, passed through 220-mmu filters and inoculated directly into the livers of mice. The animals were observed for spontaneous mortality for 12 months, at which time the remaining animals were sacrificed. The livers of all animals were examined for hepatitis and AA. Although the aqueous humor from
