Anterior surgical approaches to cataracts in juvenile rheumatoid arthritis (JRA) patients are compromised by chronic anterior and posterior uveitis, miotic pupils, and posterior synechiae. The authors retrospectively reviewed the results of pars plana lensectomy and vitrectomy in ten eyes from seven JRA patients with cataracts associated with chronic iridocyclitis and vitritis. The adjunctive use of nonsteroidal anti-inflammatory drugs (NSAIDs) for the treatment of chronic iridocyclitis was evaluated in 14 patients, eight with juvenile rheumatoid arthritis and six with idiopathic iridocyclitis. In all patients, the activity of the iridocyclitis improved with the addition of NSAIDs to their treatment regimens, permitting reduction in the dose of corticosteroid drugs. These data suggest that NSAID therapy may have an adjunctive role in the treatment of chronic iridocyclitis in childhood.

Type I pauciarticular juvenile rheumatoid arthritis (PA-JRA) is characterized by a female predominance, chronic iridocyclitis and a positive antinuclear antibody (ANA) test. In contrast, patients with type II PA-JRA are usually male, have a negative ANA test, but are HLA-B27 positive. One hundred and fifty children with unsuspected juvenile chronic arthritis (JCA) and a disease duration of 3-6 months entered a prospective study. Diagnosis of JCA could be verified in 107 patients according to the WHO-EULAR criteria. This study of children with chronic arthritis confirms that patients at greatest risk for chronic iridocyclitis have a young age of onset and positive antinuclear antibodies (ANA). Children who are ANA negative have a low risk of iridocyclitis. When it occurs it is at a significantly older age. We studied six cases of chronic, indolent intraocular inflammation that occurred after extracapsular cataract extraction and posterior chamber intraocular lens implantation. The inflammation was characterized by a delayed onset, and in three cases had the clinical appearance of a granulomatous iridocyclitis. A 72-year-old white man who had undergone surgical trabeculectomy and extracapsular cataract extraction with a posterior-chamber lens implantation in the left eye suffered from chronic iridocyclitis for eight months. He subsequently presented with acute hypopyon and vitritis. Anterior-chamber and vitreous cultures were positive for Pseudomonas cepacia. The infection was successfully treated with subconjunctival piperacillin, intravitreal cefotaxime, and intravenous piperacillin and gentamicin. To our knowledge, this is the first documented case of Pseudomonas cepacia endophthalmitis.

Juvenile rheumatoid arthritis (JRA) is best defined as the condition of chronic synovitis in children. Such chronic childhood arthritis probably includes several distinct disease processes. HLA-DR5 is associated with a chronic iridocyclitis and juvenile rheumatoid arthritis with onset in early childhood. Previously published data provided indirect evidence for selective linkage between two HLA-B alleles and HLA-DR5. Treatment of uveitis depends on the inhibition of the inflammatory reaction by corticosteroids or immunosuppressive agents. "Climatotherapy" remains a controversial subject and is generally not accepted. To test its efficacy a pilot study was undertaken. Eighty-six patients with chronic anterior uveitis were followed up for an average of five years after cataract surgery. In most cases the course of the uveitis was favorably influenced: inflammatory signs were reduced in 70% and the frequency of recurrence in 94%. With regard to the postoperative course, there was no significant difference between the eyes in which an iris suture had been performed and those in which it had not. One should therefore not hesitate to remove cataracts in patients with chronic uveitis.

Several different sets of diagnostic criteria of rheumatoid arthritis in children are in practical use. The criteria most widely applied are discussed here, especially with regard to clinical studies. Reasons are presented for the selection of criteria used in the present controlled clinical studies on rheumatic children in Oslo. Three modifications were made in the North-American criteria: 1) disease onset before 16 years of age; 2) persistent arthritis for more than 3 months; 3) biopsy is preferable in monarticular cases, except in patients with chronic iridocyclitis.

Of 38 cases of uveitis examined from 1970 to 1980 in which a tuberculous cause was considered, we classified 18 as presumed tuberculous on the basis of a positive response to isoniazid. We compared these patients to a control group who had a positive skin test but a negative response to the isoniazid therapeutic test.
