Autoimmune multiorgan involvement in elderly men is it SLE?

Systemic lupus erythematosus (SLE) in males and especially in the elderly is infrequent. We describe two male patients over 65 years old, who presented with anaemia, cytopenia, nephritis, and serositis, suggesting that they suffered from an autoimmune disease. Following an interval of 6 and 9 years respectively, the diagnosis of SLE was made. In both patients, hypothyroidism, and eye involvement (uveitis and iridocyclitis) were present indicating autoimmune activity or even a new autoimmune syndrome. One patient was treated with corticosteroids and azathioprine upon which he developed many sid

Coccidioidomycosis iridocyclitis.

BACKGROUND: Coccidioidomycosis has reached epidemic proportions in the southwest region of the United States. Despite the greater numbers of cases, isolated anterior segment ocular coccidioidomycosis in the absence of systemic infection continues to be rare, although its incidence may be increasing. METHODS: Two patients without clinical evidence of systemic disease and one patient with previously treated pulmonary coccidioidomycosis had granulomatous iridocyclitis and iris nodules that were unresponsive to corticosteroid therapy. All three patients underwent iris biopsy, anterior chamber tap

Reduction of macular oedema by acetazolamide in patients with chronic iridocyclitis: a randomised prospective crossover study.

Thirty patients with cystoid macular oedema secondary to chronic iridocyclitis were enrolled in a two period, prospective, randomised, double masked, crossover study that compared sustained release acetazolamide (500 mg twice a day) with a placebo to measure the effects on the reduction of cystoid macular oedema and improvement of visual acuity. All patients were treated for 1 month with either acetazolamide or placebo, received no treatment for 1 month, and were then treated for 1 month with the other medication. Statistically significant improvement in visual acuity was seen at 14 and 28 day

Fuchs' heterochromic cyclitis and retinal vascular abnormalities in progressive hemifacial atrophy.

We report a case of progressive hemifacial atrophy with a combination of ipsilateral Fuchs' heterochromic cyclitis and retinal vascular abnormalities. Recently, evidence was found for a neurovascular defect in hemifacial atrophy. Our case not only supports the (clinical) association between Fuchs' heterochromic cyclitis and hemifacial atrophy, but the retinal vascular abnormalities found in this patient add further support to the existence of a neurovascular defect. These findings and our short review of the literature point to the hypothesis of a common sympathetic defect, implicated in the a

[Functional results of cataract surgery with implantation in Fuchs' heterochromic cyclitis].

Cataract removal with implantation of an intraocular lens (IOL) was performed in 66 cases of Fuchs' heterochromic cyclitis. After an average postoperative follow-up of 40 months (6 months to 10 years, 81.6% of cases obtained visual acuity superior or equal to 5/10 (16.6% with a final visual acuity of 10/10). Thirty cases were analysed, 27 had satisfactory binocular vision. Two complications explained this limited visual performance: vitreal disorders observed in 45.5% of cases were responsible for visual acuity inferior or equal to 5/10 in 23% of cases, glaucoma observed in 12.1% of cases (3 c

Classification of 750 consecutive uveitis patients in the Rotterdam Eye Hospital.

The data of 750 consecutive uveitis patients who visited the uveitis department of the Rotterdam Eye Hospital were analysed by computer. Anterior uveitis was diagnosed most frequently (52%), mostly the acute form. We found a high percentage of Fuchs' heterochromic iridocyclitis and HLA-B27+ iridocyclitis. In the posterior uveitis group appeared the more recent clinical entities such as acute retinal necrosis, Birdshot retinochoroidopathy, CMV-retinitis, AMPPE and ocular candidiasis. Ocular Toxoplasmosis was still the leading cause in this group. These data will be compared with other studies o

[Ocular manifestations of rheumatic diseases. Cooperation between internist/ophthalmologist].

A red or painful eye may be the clue to a systemic condition, many of which are of a rheumatological or immunological nature. Conjunctivitis may occur in Sjögren's Syndrome, Reiter's Syndrome (and other sero negative spondyloarthropathies) and with infections such as chlamydia and viruses. 70% of cases of episcleritis are idiopathic, the other 30% being associated with rheumatoid arthritis or other connective tissue diseases or herpes zoster infection. Scleritis may be seen with connective tissue diseases or auto immune conditions (rheumatoid arthritis, Wegener granulomatosis, polyarteritis no

Does autoimmunity to S-antigen play a role in Fuchs' heterochromic cyclitis?

Autoimmunity directed against retinal or choroidal antigens has been suggested to play a role in the chorioretinal lesions observed in patients with Fuchs' heterochromic cyclitis. This hypothesis was addressed and patients with Fuchs' heterochromic cyclitis were tested for cellular immunity (migration inhibitory factor assay) against human retinal S-antigen. A significantly higher percentage of patients with Fuchs' heterochromic cyclitis had a positive cellular autoimmune response to S-antigen than healthy controls and other patients with anterior uveitis. This finding is remarkable since Fuch

[Cerebral venous thrombosis disclosing Behçet disease].

BACKGROUND: Behçet's disease is very occasionally revealed by thrombophlebitis in children. Dural sinus thrombosis can be one of its complication. CASE REPORT: A 14 year-old boy of Mediterranean origin was admitted for acute meningitis with temperature of 40 degrees C. His CSF contained 24 cells/ml and 0.33 g/l protein; it was sterile. The RBC sedimentation rate was 84 mm. Other investigations, including brain scan, were negative. The condition became worse, with a deterioration of visual acuity, attack of aphthous stomatitis and skin lesions at points of puncture. There was papilledema with

Quantitative analysis of iris translucency in Fuchs' heterochromic cyclitis.

PURPOSE: To measure intraocular stray light and to quantify translucency of the iris and the surrounding ocular wall in both eyes of patients with Fuchs' heterochromic cyclitis; to investigate whether differences in paired eyes could be measured in patients without heterochromia or with a minimal degree of iris atrophy. METHODS: Intraocular stray light was measured by means of the direct compensation technique. A modification of this technique was used to quantify translucency of the iris and the surrounding ocular wall. RESULTS: Intraocular stray light proved to be significantly higher in a

Reactivation in children of juvenile chronic arthritis and chronic iridocyclitis associated with human herpesvirus-6 infection.

The human herpesvirus-6 (HHV-6) causes exanthem subitum. Moreover it can provoke a large scale of different other clinical symptoms. The life-long persistence of human herpes viruses is well known (HSV 1 and 2, VZV, CMV, EBV). For the HHV-6 we have described here for the first time the onset/reactivation of juvenile chronic arthritis (jcA) and chronic iridocyclitis as chronically active and persistent infection in two children of school age.

Iris crystals in chronic iridocyclitis.

Iris crystals in three patients with idiopathic iridocyclitis, Fuchs' heterochromic iridocyclitis, and iridocyclitis associated with systemic pseudolymphoma are described. Two patients had iris crystals in one eye, and the other patient had bilateral involvement. The number of iris crystals in each eye ranged from one to multiple crystals. Iris crystals were minute and refractile and glistened with light; they have previously been reported to be Russell bodies and to be associated with hypergammaglobulinaemia. However, two of the patients, who underwent serum protein electrophoresis, did not d

A murine model of experimental autoimmune lens-induced uveitis using Klebsiella O3 lipopolysaccharide as a potent immunological adjuvant.

Experimental autoimmune uveitis and finally panophthalmitis could be produced in mice by repeated immunization of syngeneic eyeball extract mixed with Klebsiella O3 lipopolysaccharide (KO3 LPS) as a powerful immunological adjuvant. No ocular lesions were produced in mice given eyeball extract emulsified in complete Freund's adjuvant (CFA), KO3 LPS alone or eyeball extract alone. Histopathological changes in the ocular lesions at the early stage after the second or tertiary immunization were characterized by infiltration with inflammatory cells in the ciliary body and iris. The iridocyclitis wa

The place of corticosteroid therapy in juvenile chronic arthritis in 1992.

Juvenile chronic arthritis (JCA) is a heterogeneous disease, treated with systemic or local corticosteroid therapy. Systemic corticosteroids are indicated in systemic JCA only when nonsteroidal antiinflammatory drugs have failed or cause side effects or if there is evidence of severe pericarditis. Intravenous pulses of methylprednisolone produce little benefit. Corticosteroid therapy must be tapered slowly, and it may take a long time to reach an alternate day regimen. Side effects are the most worrying problem with daily therapy, particularly growth arrest. Some cases of polyarticular JCA wit

Eye lesions in pet birds.

Amongst eye lesions in birds that died in quarantine, cataracts were the most common disorders (37/241, 15.4%), being prevalent in the annular pads of cockatiels (Nymphicus hollandicus), Amazon parrots (Amazona aestiva aestiva) and budgerigars (Melopsittacus undulatus). The incidence in male birds was more than twice that in females. Deposition of crystals, mostly in the cornea, was the second most frequent lesion (21/293, 8.7%), mainly found in cockatiels, parakeets (Psittacula krameri manillensis), Amazon parrots (Amazona aestiva aestiva), budgerigars and finches (Poephila gouldiae gouldiae)

Results of extracapsular cataract extraction with intraocular lens implantation in eyes with uveitis and Fuchs' heterochromic iridocyclitis.

A total of 63 cataractous eyes of 48 patients (group 1) with uveitis and 12 eyes of 12 patients (group 2) with Fuchs' heterochromic iridocyclitis underwent extracapsular cataract extraction (ECCE) with capsular bag fixation of a posterior-chamber intraocular lens (PC OIL). A perioperative regimen of systemic dexamethasone (0.8-1.5 mg/kg) was initiated. This dose was tapered by 20 mg/day, reaching zero within 4-8 days. Intraoperatively, synechiolysis was required in 51 of 63 eyes in group 1 and a sector iridectomy was necessary in 42 eyes. A fibrinous reaction occurred in 7 eyes. During a mean

Analysis of aqueous humour in uveitis by high performance liquid chromatography and sodium dodecyl sulphate-polyacrylamide gel electrophoresis.

Aqueous humour from patients with Fuchs' heterochromic cyclitis (FHC) and other types of uveitis was analysed by high performance liquid chromatography (HPLC) and sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE). Using HPLC, the number of peaks and their respective elution times were similar for the FHC, uveitis and control groups. SDS-PAGE and silver staining showed immunoglobulin G migrating as two to four distinct bands in all non-reduced samples. In the reduced state, 1-5 unidentified low MW bands (10-19 kD) were seen. Two bands at 19 and 10 kD appeared to be specific

Immune deposits in iris biopsy specimens from patients with Fuchs' heterochromic iridocyclitis.

To investigate whether Fuchs' heterochromic iridocyclitis may be an immune complex vasculitis, we used an immunofluorescence technique to detect immunoglobulins and complement in iris biopsy specimens from nine patients with Fuchs' heterochromic iridocyclitis, 12 patients with other types of uveitis, and nine patients with glaucoma but without uveitis. No specific immune deposits were observed in the irises of the patients with Fuchs' heterochromic iridocyclitis. Immunoglobulin G, IgA, IgM, and complement were detected in patients with Fuchs' heterochromic iridocyclitis and patients with uveit

Cataract extraction in uveitis: comparison of aphakia and posterior chamber lens implantation.

We reviewed the charts of 39 patients (45 eyes) with uveitis who underwent cataract extraction (intracapsular or extracapsular) with or without implantation of a posterior chamber intraocular lens (IOL) at the Royal Victoria Hospital, Montreal, between 1981 and 1990. The nine eyes with Fuchs' iridocyclitis in which an IOL was implanted had good visual results and few postoperative complications. The 18 eyes with uveitis other than Fuchs' iridocyclitis that received an IOL did not show a greater risk of postoperative complications than those left aphakic (n = 17), and the postoperative visual a

Fuchs' heterochromic iridocyclitis is not associated with ocular toxoplasmosis.

To analyze the association between Fuchs' heterochromic iridocyclitis (FHI) and toxoplasmosis, we performed ocular examinations and used various specific laboratory tests to establish a role for Toxoplasma gondii in the pathogenesis of FHI. Results were compared with those for other types of uveitis and healthy controls. Of the 88 patients with FHI, nine (10.2%) had toxoplasmosislike scars, but an association could not be proved by the indirect immunofluorescence antibody test or enzyme-linked immunosorbent assay, or by a test for cellular immunity to Toxoplasma antigen.
