Pediatric uveitis in southern Turkey.

Uveitis in the pediatric population is not so common, and the etiology is different from adults. In the present study the charts of the patients with onset of uveitis at 16 years of age or younger were reviewed in order to analyze the etiology in pediatric patients with uveitis, and to compare the results with other studies carried out on different populations. The charts of 90 cases, followed at the Uvea-Immunology Clinic of the Cukurova University Medical Faculty Department of Ophthalmology, between January 1987 and February 1996 were reviewed retrospectively. There were 47 girls, 43 boys, a

Ophthalmologic manifestation of Behçet's disease.

Loss of vision in Behçet's disease is one of the most common, as well as one of the most serious, of its varied manifestations. Total blindness may often be the eventual outcome. The underlying problem in the eye, as well as in other organ systems, is an occlusive vasculitis. Although the most common ocular symptom is that of iridocyclitis, the presence of retinovascular lesions, especially necrotizing retinitis, is well known but often obscured by the severity of the anterior reaction, which precludes a good view of the retina. While Behçet's disease is characterized by spontaneous remissions

Lesions associated with infectious keratoconjunctivitis in alpine ibex.

Following a severe infectious keratoconjunctivitis (IKC) epizootic in free-ranging alpine ibex (Capra ibex ibex) in Switzerland in 1993, 19 animals were examined from six different populations. Mucopurulent exudates, reddened conjunctiva and mononuclear inflammatory cell infiltration in the conjunctiva and the limbic area were observed in mild cases. In more severe cases, lesions ranged from perilimbic neovascularization to corneal edema, erosion, ulceration and neovascularization accompanied by infiltration of neutrophils. Sometimes an iridocyclitis was observed. In the most advanced stages,

Herpes zoster sine herpete. A potential cause of iridoplegic granulomatous iridocyclitis.

OBJECTIVE: Herpes zoster ophthalmicus (HZO) is a recurrence of varicella zoster virus involving cranial nerve V-1, but does not always have skin manifestations. The objective of this work is to study iridoplegic granulomatous iridocyclitis as an acute, fulminant iridocyclitis that probably is caused by the recurrence of varicella zoster virus without skin eruptions. PARTICIPANTS: The author reports 15 cases of iridoplegia granulomatous iridocyclitis with involvement of the anterior uveal tract without known skin eruptions. RESULTS: All patients have had a clinical course of iridocyclitis clo

[The ocular manifestations in Felty's syndrome].

The Felty syndromme is a rare clinical form of the rheumatoid poliarthritis, which involves, besides the articular manifestations specific to the latter, other manifestations such as: splenomegalia with hypersplenism, poliadenopathy, ulcerations at the low leg level and skin pigmentation. In some cases, the Felty syndromme may be associated with ocular modifications: episcleritis, nodular scleritis, corneal ulcer, Sicca syndromme, iridocyclitis, retineal vasculite. A clinical observation of Felty syndromme associated with ocular modifications is presented. Some aspects regarding the nosologic

Endocrine aspects of paediatric rheumatic diseases.

Compared to the now numerous studies on the endocrinology of rheumatic diseases in adults, only a small number of studies has been published on children with rheumatic diseases. Prolactin has been most extensively investigated, showing interesting parallels with the findings in adults with rheumatological diseases. Thus, analogous to adult RA most forms of JRA or JCA (with the exception of ANA-positive JRA with uveitis) appear to show, if anything, low to normal levels of prolactin. Since the prolactin levels in adult RA depend on the inflammatory activity, and the physiological prolactin secr

[5-fluorouracil in the treatment of postoperative glaucoma].

INTRODUCTION: Standard filtering surgery in postoperative glaucoma in aphakic and pseudophakic eyes and eyes after failed filtering surgery is successful in 50% cases. The main cause of failure filtering surgery is scarring of the filtering bleb-fibroblastic proliferation at the subconjunctival episcleral interface [1, 2]. The use of antimetabolites such as 5-fluorouracil and Mitomycin C inhibits fibroblast proliferation and increases the success rate of filtering surgery, especially in postoperative glaucoma [3-6]. The exact dosage and frequency of administration to maximize success and minim

Juvenile rheumatoid arthritis--Madras experience.

In a prospective study of 1,053 consecutive children who attended the Rheumatic Care Centre, Government General Hospital, Madras from 1991 to 1995, 331 children fulfilled the criteria proposed by the American Rheumatism Association as modified by Cassidy et al for the diagnosis of Juvenile Rheumatoid Arthritis. These children were thoroughly examined and investigated and classified into 3 onset types which was then sub-classified into early entry and late entry groups based on the duration of illness. Other arthritic conditions were excluded. There were 44 cases belonging to Systemic onset, 17

Ocular complications of adult rheumatoid arthritis.

In a prospective study of 325 patients with adult rheumatoid arthritis, ocular complications were seen in 73 patients (22.4%). Keratoconjunctivitis sicca was the most common ocular lesion. Other lesions were episcleritis, scleritis, marginal thinning of the cornea with keratolysis, stromal corneal opacities with peripheral vascularisation, and iridocyclitis. The mean duration of the arthritis and the mean duration of seropositivity were found to be significantly higher in patients with ocular complications. Physicians should include ophthalmic examination as a routine in their protocol for pat

Cataract surgery using heparin surface-modified intraocular lenses in Fuchs' heterochromic uveitis.

I prospectively studied cataract extraction with implantation of heparin surface-modified intraocular lenses (IOLs) in 20 patients with Fuchs' heterochromic uveitis. The mean follow up was 14.5 months. Seven episodes of postoperative uveitis occurred, including two with fibrin deposition, but substantial giant-cell adhesion to the IOL was seen in only four cases. A visual acuity of 20/40 or better was achieved in 95% of the eyes despite other causes of visual loss such as vitreous opacities. The use of heparin surface-modified IOLs is suggested for patients with heterochromic uveitis.

Amsler's sign associated significantly with Fuch's heterochromic cyclitis (FHC).

To investigate the frequency and diagnostic significance of Amsler's sign in patients with Fuch's heterochromic cyclitis (FHC), we performed a diagnostic AC puncture of the eyes of 31 patients with this disease and looked for the same sign in 104 patients with intermediate uveitis and 373 patients with various other types of uveitis who underwent the same procedure. AS was significantly more frequent in the FHC patients than in either of the other two groups of patients, and occurred infrequently in all other types of chronic uveitis. We therefore consider its occurrence to have important diff

Waardenburg syndrome type II: phenotypic findings and diagnostic criteria.

The Waardenburg syndrome (WS) consists of at least two distinct autosomal dominant hereditary disorders. WS Type I has been mapped to the distal part of chromosome 2q and the gene identified as PAX3. Other gene(s) are responsible for WS Type II. Mapping WS Type II requires accurate diagnosis within affected families. To establish diagnostic criteria for WS Type II, 81 individuals from 21 families with Type II WS were personally studied, and compared with 60 personally studied patients from 8 families with Type I and 253 cases of WS (Type I or II) from the literature. Blindness in leprosy patients of Kaduna State, Northern Nigeria.

We examined the eyes of 311 leprosy patients to determine the prevalence of blindness and impaired vision, and the causes of blindness in leprosy patients in Northern Nigeria. There was impaired vision or blindness in 74 eyes. Both eyes were affected in 5.1% of patients while 2.9% were totally blind. Leprosy alone was responsible for total blindness in 1.3% of patients, while other ocular diseases contributed to blindness in 1.6%. Exposure keratitis (21.3%), corneal opacities (13.5%) and chronic uveitis (10.1%) were the commonest leprotic cause of blindness. Primary care and early intervention

Association to HLA-DRB1*08, HLA-DPB1*0301 and homozygosity for an HLA-linked proteasome gene in juvenile ankylosing spondylitis.

To assess the role of HLA genes other than those encoding B27 in predisposing to JAS and AAS, we analyzed the distribution of B*4001, as well as the DRB1, DPB1, and LMP2 alleles, using PCR-based techniques in 63 JAS and 44 AAS patients (all B27 positive). The NBMDR (N = 4724) provided a source of controls matched with the patients for B27 (or other markers when necessary). We found an increase of the B*4001, DRB1*08, and DPB1*0301 alleles, as well as the LMP2 b/b genotype (the latter was most pronounced among patients with acute iridocyclitis), in JAS compared to B27-positive controls. [Differential diagnosis of tuberculous and herpetic uveitis].

A total of 158 inpatients with ophthalmic tuberculosis or with this condition suspected were examined at the St. Petersburg Research Institute of Phthisiopulmonology. Tuberculous etiology of iridocyclitis and chorioretinitis was confirmed in 39 patients, herpesvirus etiology was recognized in 35 patients. In the rest patients endogenous uveitis or dystrophic involvement of the eye of other origin were diagnosed. An algorithm of differential diagnosis tuberculous and herpetic uveitis of different localization has been developed, which is based on the clinical findings and results of tuberculin

A genetic association between juvenile rheumatoid arthritis and a novel interleukin-1 alpha polymorphism.

OBJECTIVE: The genetic factors that predispose to the development of juvenile rheumatoid arthritis (JRA) and its complications are not completely understood. The cytokine interleukin-1 (IL-1) has been implicated in the pathogenesis of JRA and other inflammatory diseases. This study was performed to test whether polymorphisms of the IL-1 alpha gene might be associated with JRA. METHODS: We sequenced the 5' regulatory region (containing the promoter) of the human IL-1 alpha gene in 18 normal subjects. This revealed a C (IL-1A1) to T (IL-1A2) transition polymorphism at position -889. We studied

Fuch's heterochromic cyclitis and HLA histocompatibility antigens.

Genetic typing of class I major histocompatibility complex (MHC) antigens (HLA-A, -B and -C) was performed in 22 patients with Fuch's heterochromic cyclitis (FHC) and in a control group of 339 healthy subjects in order to ascertain whether there are any genetic factors that protect against or increase the risk of developing this disease. The frequency of only one HLA-antigen was found to differ significantly in the patients with FHC as compared with the normal subjects: HLA-A2 had a negative association with FHC (9.09% in FHC verus 47.47% in the control group; p < 0.01, Fisher's exact test wit

HLA and juvenile rheumatoid arthritis.

HLA class II analysis in a group of 153 Czech children with juvenile rheumatoid arthritis by PCR and oligonucleotide hybridization demonstrated associations with several alleles. DRB1*0801 (RR = 5.3, p < 0.005) and DRB1 * 11 (RR = 2.2, p < 0.01) including all subtypes were shown to be increased in the rheumatoid factor-negative group (N = 137). The same results were observed in Italy, England and Norway. In patients with the pauciarticular onset with conversion to polyarticular within 3 years, a statistically significant increase in DR2 (RR = 10.1, p < 0.00005), mostly due to DRB1*1501, was fo

Subtypes of HLA-DRB1*03, *08, *11, *12, *13 and *14 in early onset pauciarticular juvenile chronic arthritis (EOPA) with and without iridocyclitis.

A set of 200 patients with early onset pauciarticular juvenile chronic arthritis (EOPA-JCA) from Munich (165) and Prague (35) was investigated for the subtypes of HLA-DRB1*03, *08, *11, *12, *13 and *14. In addition, the relationship of DRB1, DQA1, DQB1 and DPB1 alleles with iridocyclitis in patients with EOPA-JCA was investigated. Subtyping for DRB1*03 was not informative, as all DR3 positive patients and all except one of the controls possessed DRB1*0301. Thus, the role of DRB1*0302 could not be assessed. The subtypes for DRB1*12, *13, and *14 did not reveal any statistically significant dif
