The pseudo plateau iris: A review.

Pseudo-plateau iris configuration (PPIC) is an under-recognized cause of persistent angle narrowing in the young population, characterized by anterior displacement of the peripheral iris secondary to iridociliary cysts. Although quite common in the general population, occurring in up to half of otherwise normal eyes, only a small subset of ciliary body (CB) cysts produce clinically significant angle compromise. The condition often remains undetected because routine anterior-segment evaluation lacks the ability to visualize the CB, and ultrasound biomicroscopy (UBM) is not universally employed.

Tapioca iris melanoma masquerading as unilateral glaucoma.

A woman in her mid-70s developed uncontrolled intraocular pressure (IOP) in her right eye despite trabeculectomy, Ahmed valve implantation and ongoing topical therapy. Left IOP was normal without treatment. Examination revealed fluffy anterior chamber deposits and vitreous debris. Ultrasound biomicroscopy (UBM) showed diffuse thickening of the iris and ciliary body. Iris biopsy and anterior chamber aspiration confirmed melanoma, and enucleation demonstrated extensive iris and ciliary body involvement with extrascleral extension (stage pT4). This case underscores the diagnostic challenge of lig

A case report: iridociliary melanocytoma associated with secondary glaucoma.

BACKGROUND: Distinguishing between benign iridociliary melanocytoma and malignant melanoma presents a diagnostic challenge, particularly given the potential overlap in tumor growth patterns and clinical manifestations, especially when patients present with secondary glaucoma. Misdiagnosis may induce severe clinical consequences, including enucleation. Therefore, the judicious selection of biopsy or surgical techniques is crucial in both diagnosing and managing the condition. CASE PRESENTATION: A 44-year-old female presented with uncontrolled elevated intraocular pressure (IOP) and a heavily p

Corneoscleral plaque brachytherapy using 'BARC I-125 Ocu-Prosta seeds' in the management of iridociliary melanoma.

We present the case of a woman in her late 40s who presented with an iris lesion extending to the ciliary body and mature white cataract with a visual acuity of perception of light in her right eye. The tumour was managed with Bhabha Atomic Research Centre Iodine-125 (BARC I-125) Ocu-Prosta seeds plaque therapy with the amniotic membrane buffer technique. No corneal complications or secondary glaucoma were seen over a 6-month follow-up, and a reduction in tumour size was observed on MRI. Ten months post corneoscleral plaque brachytherapy (PB), the patient underwent phacoemulsification in the s

Extensive intraocular melanoma with secondary glaucoma in a 15-month-old Thoroughbred filly.

A 15-month-old, grey, Thoroughbred filly presented for investigation of a 6-week history of corneal oedema and blepharospasm on the right eye (OD). The filly was otherwise healthy. Following ophthalmic examination, glaucoma on the OD was diagnosed. A space occupying mass within the anterior chamber was documented on transpalpebral ultrasonographic examination. This mass obliterated most of the anterior intraocular structures on the peripheral nasal side (corneal endothelium and drainage angle), leading to secondary glaucoma. After systemic and topical treatment addressing secondary glaucoma, t

Peering into the eye: A comprehensive look at ultrasound biomicroscopy (UBM) and its diagnostic value in anterior segment disorders.

BACKGROUND: Ultrasound biomicroscopy (UBM) is a noninvasive imaging modality that enables in-vivo visualization of the structures of the anterior segment of the eye. Unlike routine ophthalmic diagnostic ultrasound which uses frequencies of 5-10 MHz, UBM utilizes ultrasound frequencies in the range of 50-100 MHz. The high-frequency probes in UBM allows for higher resolution and better visualization of subsurface ocular structures, even in the presence of anatomic or pathological obscuration. UBM has qualitative as well as quantitative applications in various disorders affecting the anterior seg

Multifocal choroidal melanoma: a clinicohistopathological correlation.

A woman in her late 50s presented with on-and-off redness and diminution of vision in her left eye for 6 months. Her best corrected visual acuity was 20/40 in the right eye and hand motion in the left eye. Anterior segment examination revealed a greyish-white lesion extending from 3 to 6 o'clock hours posterior to the iris and protruding into the anterior chamber. Left eye B-scan ultrasonography showed a multifocal choroidal lesion, a smaller one involving the posterior pole, and a larger lesion involving the complete nasal quadrant and anteriorly extending to the ciliary body and iris. Correlation of clinical and radiological predictors of retinoblastoma with high-risk histopathological features.

INTRODUCTION: Retinoblastoma (RB) tumors having high-risk histopathologic features (HRFs) have an increased risk of metastasis and disease relapse. However, RB has not been studied widely in Pakistan. Therefore, we evaluated the association of clinical, histopathologic, and radiological findings with HRFs in patients with RB who were treated at the Indus Health & Hospital Network in Karachi, Pakistan. METHODS: We enrolled treatment-naïve patients with RB who received upfront enucleation from September 2017 to February 2021. We evaluated enucleated eyes with the Intraocular Classification of R

Recurrent Conjunctival Squamous Cell Carcinoma and Intraocular Tumor Extension after Topical Erythropoietin: A Case Report.

Topical erythropoietin has been recently introduced for the treatment of avascular conjunctival and scleral lesions. Before this treatment can be routinely used, however, its safety profile and contraindications should be determined. Herein, we report a case of recurrent conjunctival squamous cell carcinoma (SCC) and intraocular tumor extension after treatment with topical erythropoietin for excisional biopsy-induced scleral necrosis. An 87-year-old man underwent excisional biopsy for a conjunctival leukoplakic mass. Histological examination showed a well-differentiated SCC on the postoperativ

The mounted alloimmunity of the iris-ciliary body devotes a hotbed of immune cells for corneal transplantation rejection.

Graft rejection is still the major obstacle causing corneal transplantation failure. However, the underlying pathogenesis remains largely unclear. The iris-ciliary body (I-C) is enriched with blood vessels and various immune cell populations, presumably predisposed to be involved in corneal transplantation rejection. After penetrating keratoplasty, compared to the normal (Nor) and syngeneic (Syn) groups, I-C tissues in the allogeneic (Allo) group displayed stronger alloimmune responses, with more infiltrations of CD45+ inflammatory cells and CD3+ lymphocytes, increased transcriptional levels o

[Echography].

Echography (also ultrasound) is a clinical, non-invasive imaging module that is used for the measurement of the axial length of the eye and for the investigation of pathologic entities of the chamber angle, the iris and the ciliary body. Furthermore, its role in the management of vitreoretinal pathologies and the differentiation of intraocular tumors is indisputable. Echography remains the first-choice imaging tool in case of insufficient visualization of the posterior segment due to opacity or obstruction of the optical media of the eye. In addition, it can contribute to a more precise diagno

Clinical 3D Imaging of the Anterior Segment With Ultrasound Biomicroscopy.

PURPOSE: Ultrasound biomicroscopy (UBM) is an important ophthalmic imaging modality due to its ability to see behind pigmented iris and to visualize anterior chamber when the eye's transparency is compromised. We created a three-dimensional UBM (3D-UBM) system and acquired example images to illustrate its potential. METHODS: A commercial 50-MHz two-dimensional UBM (2D-UBM) system was attached to a precision translation stage and translated across the eye to acquire an image volume. The stage was mounted on a surgical microscope, which enabled safe, stable positioning. Image processing steps i
