Structures within the eye bulb diminishing visual acuity can be cut by means of neodymium-YAG-laser without surgical opening of the globe. The First Eye Department has had the first instrument of this kind in Vienna at its disposal since the summer of 1984. Up to now it has been used for posterior capsulotomy, iridotomy, opening pupillary membranes and for cutting vitreous membranes in 84 operations altogether. The authors report on the neodymium-YAG laser used after extracapsular cataract extraction with the implantation of a posterior chamber lens in 1726 patients. The most common indication was capsular fibrosis necessitating posterior capsulotomy. The alternative to the YAG laser would be a bulbus-opening procedure via pars plana using a Sato knife. The advantage of the YAG laser in comparison with capsulotomy with a Sato knife is that there is no danger of intraocular infection or bleeding (which may result from tension on the fibers with the first method). Anterior segment surgery via the pars plana was done on 114 eyes with the Klöti vitreous stripper between January, 1977 and December 31, 1982. In every case the posterior capsule was removed when present and anterior or total vitrectomy was performed. None of the 114 eyes had a history of retinal detachment prior to nor at the time of anterior segment surgery. There were 37 cases of trauma in this series. Follow-up varied from 1 to 66 months with 58% of cases having a follow-up of greater than one year. There was a 10.50% post-operative retinal detachment rate (12/114 eyes). Seven detachments

We have used the neodymium-yttrium-aluminum-garnet (YAG) laser effectively to cut posterior capsules and pupillary membranes, but the procedure was difficult. Using the Abraham contact lens facilitated the procedure, but may be related to the development of retinal damage.

Two infants, aged three days and six weeks, respectively, presented with dense extensive persistent pupillary membranes as sporadic isolated findings. A clear central pupillary aperture was unable to be achieved with mydriatic agents. The patients were treated by surgical removal of the membranes at age five days and six months respectively. Various ocular complications following cataract surgery were treated using vitrectomy instruments by the pars plana approach. A total of 11 eyes were operated. Eight of these eyes had various anterior segment problems: Secondary cataract (2 eyes), cyclitic pupillary membranes (2 eyes), up-drawn pupils (3 eyes), and occluding pupillary membrane following haemorrhage (1 eye). We used closed-eye endosurgery to treat 58 patients with anterior segment problems. These included nonsenile cataract, residual lens material, pupillary membranes, vitreocorneal touch, aphakic pupillary block glaucoma, intraocular lens complications, epithelial ingrowth, and total hyphema. The results of intraocular lens implantation in eight cases of cataract secondary to heterochromic uveitis are presented. Follow-up was from 16 months to two years (mean 22 months). Visual acuity was restored to 6/9 or better in six cases and 6/18 in one, but remained hand movements in the remaining case. Postoperative complications included the development of inflammatory pupillary membranes, vitreous haze, and increased intraocular pressure.

Observations based on clinical and histopathologic data from 200 consecutive cases of retrocorneal pigmentation by melanin-containing cells (RCP) and 25 cases of pigmented pupillary membranes (PPM) are reported. A new instrument, the membrane peeler cutter (MPC), has a solenoid-activated right-angled microscissors (diameter, 0.9 mm) that can be used as a hooked needle as well. The eyes of 160 Collies were examined for Collie Eye Anomaly, Progressive Retinal Atrophy, and any other eye abnormality present. The incidence of CEA in 160 Collies was 40.6 per cent. One hundred and eight pupillary membranes treated with the vitrophage through a pars plicata or pars plana approach were evaluated for long-term results. The average follow-up was 3.86 years, with a range of 1 to 7 years. No major operative complications were encountered. Anatomical success was achieved in all cases. Vision improved in 67.5% of eyes. 9 patients are presented on whom anterior vitrectomy was performed. The indications included congenital rubella cataract with microphthalmos, traumatic cataract, persistent anterior and posterior hyperplastic primary vitreous, postcataract pupillary membranes, and postcataract vitreous prolapse with cystoid mascular edema. All patients to date have responded satisfactorily to the procedure. There has been no incidence of complicating retinal detachment.
