Thirty patients requiring cataract extraction for improved vision, and having concurrent primary open-angle glaucoma controlled with varying success by medication, had combined intracapsular cataract extraction and cyclodialysis performed by one surgeon. The average duration of follow-up was 22 months. Visual acuity deteriorated in one case in whom vision was limited to finger counting preoperatively. Steroids that inhibit glucose-6-phosphate dehydrogenase (G6PD) were used to examine the correlation between the loss of GSSG-reducing activity and G6PD deficiency in the lens. The correlation was found to be nonlinear. In senile cataracts, which had lost 36% of NADPH-generating activity as compared to clear lenses, the estimated loss of GSSG reduction was only 20%. I used the laser interferometer to test 137 eyes to determine whether preoperative interference visual acuity tests could accurately predict postoperative Snellen visual acuities. Fifty-seven eyes of 49 patients with senile retinoschisis were treated by argon laser photocoagulation and followed up for 3 to 6 years. In most of the treated eyes the retinoschisis either completely or partially collapsed and in none did the retinoschisis or loss of visual field progress further. One hundred sixty-seven patients aged 50 years or more had cataracts extracted between Jan 1, 1979, and Dec 31, 1979, at the New England Medical Center, Boston. In comparison with 824 patients who elected one of six other surgical procedures, when data were stratified according to age and sex, patients undergoing cataract extraction had nearly twice the mortality of the reference patients. Analysis indicated diabetic patients did not significantly alter the relative mortality. 282 cataract patients were operated and a Worst Medallion 2-loop pseudophakos was implanted. The material consisted of 154 males, 128 females, mean age 58.8 years, range 3-88 years. 200 had senile, 35 pre-senile, 31 traumatic and 16 some other type of cataract. 247 were intracapsular, 35 extracapsular extractions. 87.6% of all cases had a vision of greater than or equal to 0.5; if 8 cases with non-implant dependent complications are excluded, the rate is 90.1%. The following complications were observed: one bullous keratopathy (0.4%) and 5 local corneopathies (1.8%), 9 cases of uveitis (3.2%)

Free amino groups were analyzed in the water-insoluble fraction (WIF) of cataractous human lenses of the nuclear sclerosis, pigmented type. Two modified versions of the trinitrobenzene sulfonic acid (TNBS) procedure for the determination of free amino groups in proteins (AFSA Habeeb, Anal Biochem 14:328-336, 1966) were used for this purpose. Intra and extra capsular cataract surgery are discussed with regard to advantages and problems. Particular consideration is given to modern microsurgical extracapsular technique. Analysis of corneal complications, cystoid macular oedema and retinal detachment suggest that the present evidence does not justify a change to extracapsular technique, other than to give support to a posterior chamber intraocular lens. It is pointed out that for many surgeons, such a change of technique may cause more problems than it prevents.
