Genetic differences in drug metabolism associated with ocular toxicity.

The tissue localization and subcellular distribution of drug-metabolizing enzymes in the eye are described. With the use of inbred strains of mice, the [Ah] complex is shown to be an important experimental system for probing genetic differences in drug metabolism and related drug toxicities. Although the genetic system described in detail here involves mice, there is ample evidence that the same system operates in man. Genetic differences in acetaminophen- and naphthalene-induced cataract formation and and other ocular degeneration are shown to be related to the [Ah] complex. Because this toxi

Pars plana lensectomy by ultrasonic fragmentation. I. Versatile and simple technique for cataract removal.

A comparison has been made between pars plana lansectomy and intracapsular cataract extraction Pars plana lensectomy has been shown to have many advantages. It is applicable to any age, while intracapsular cataract extraction is restricted to patients over 30 years old. It can be used in any type of cataract, whereas intracapsular cataract extraction is restricted to senile cataracts. It requires less patient preparation, less instrumentation, and less surgical dexterity than does intracapsular cataract extraction. It allows intraocular examination at the time of surgery. It can be combined wi

Pars plana lensectomy by ultrasonic fragmentation. II. A poor procedure for routine cataract extraction.

Removal of the lens through a pars plana incision may be carried out with an aspiration-cutting device in young soft lenses, and with ultrasonic fragmentation, combined with an aspiration-cutting probe, in older, harder lenses. Extremely hard lenses cannot be efficiently removed through small incisions. These techniques are most useful when combined with a pars plana vitrectomy so that the same sized incision may be used for both procedures. In a few other selected situations, lensectomy through a pars plana incision is also advantageous. Adoption of this incision site for routine senile catar

Evaluation of superoxide dismutase (orgotein) in medical treatment of canine cataract.

Orgotein (Palosein) is a veterinary product with marked superoxide dismutase activity that has been used intracamerally to treat senile cataracts in the dog. Fourteen old dogs with bilateral senile cataracts were injected twice in one eye with orgotein and twice in the other eye with isotonic saline under masked conditions. Observations were made over an average of a 12-week period. No clearing of lenses could be detected and there was no observed subjective improvement in the dogs' visual behavior.

The Emory mouse cataract: an animal model for human senile cataract.

The Emory mouse cataract is a late-appearing lens opacity which may serve as an animal model for some human senile cataracts. It is inherited as an autosomal dominant trait and has a typical course of development. Lens opacities may become readily apparent as early as 6-8 months in mice having a familial history of early cataracto-genesis. Many gross morphologic and microscopic features resemble findings in human senile cataract. As an animal model it has many desirable characteristics. Its slow development permits studies of the lens at the pre-cataractous stage and makes it a good assay syst

Senile lens changes and diabetes in two population studies.

Clinical and epidemiologic studies of the association between senile cataract and diabetes have produced what initially appears to be conflicting evidence. Three studies of extracted cataract have found a positive association in persons less than 70 years old. Conversely, two studies of senile lens changes (including aphakia) reported no association with diabetes. These apparently disparate findings led to the hypothesis that the diseases are in fact not associated, but that diabetics are more likely to be referred to ophthalmologists and therefore to have cataracts removed. We tested this hyp

Clinical experience with continuous-wear hydrophilic contact lenses in aphakia.

Twenty-two aphakic eyes in seven females and thirteen males were fitted with a new type of soft contact lens designed for continuous wear. Nine patients were operated on for unilateral traumatic cataract, and eleven patients were operated on for senile cataract. Sixteen patients terminated the study. In all cases the visual acuity with lens was almost equal to the acuity obtained with spectacles. The lenses were well tolerated without any special wearing schedule. No serious complications occurred. However, about one third of the patients developed conjunctivitis during the investigation perio

Day-case cataract surgery.

This is a report of prospective trial to compare day-case and inpatient surgery for senile cataract. In this district, cataract extraction is usually delayed until bilateral surgery is warranted. Both eyes are then operated upon a week or so apart. Such patients are ideal for comparing the results of day-case and in-patient management, since if one eye is done by each method, there is a perfect match for age, sex, general medical health, and domestic circumstances. During a period of 27 months, 138 patients aged 50 years or over were included in this trial. Five more patients were dropped from

Histological comparison of senile posterior subcapsular cortical cataract and experimental posterior subcapsular cortical cataract induced by vincristine sulfate.

The similarity between vincristine sulfate-induced experimental cataract and posterior subcapsular cortical cataract in man is the grossly visible pathological processes of cataracts in the posterior subcapsular cortical region. In histological observations, these processes are described as degeneration and necrosis of the crystalline lens fibers immediately below the anterior capsular epithelial cells, as well as degeneration and necrosis of the cellular nuclei and irregularity of the cellular arrangement in the bow area, and degeneration and vesicle formation of the lens fibers immediately b

Palmar main line polymorphisms in cataracts.

The palm prints of patients with different types of cataract (total, nuclear, zonular, cortical and complicated) were analysed for main line terminations and compared with those of a random population. The cases were also grouped into congenital, juvenile and senile types for broader comparison. The results showed significant variations for all the 4 main line terminations (D, C, B and A) in senile and cortical cataracts, while among other types of cataract, congenital cases differed significantly in D and A line, zonular in C and A line and nuclear in B and A line terminations compared with c

Variations of lens thickness in relation to biomicroscopic types of human senile cataract.

The results are reported of 38 ultrasonographic in vivo mensurations of intraindividual differences in axial thickness between a cataractous lens in one eye and a biomicroscopically clear or slightly cataractous lens (incipient deep cortical opacity) in the other. Obviously intumescent cataractous lenses were excluded. In general, the cataractous lens was thinner than the contralateral clear or slightly cataractous lens. Large decreases in lens thickness appeared in lenses with the capsule-near opacities of posterior subcapsular cataract (PSC) + anterior capsular/subcapsular opacity (ACSCO). P

[On the proof of late sequelae of bulbal and orbital contusion (author's transl)].

Contusion cataracts may develop long time after injury, without visible lens changes in the interval. The lens opacities appear like those known in senile cataracts if the eye was injured after the 45th year of age. Demonstration of other traumatic changes becomes then very important for the evaluation of the role of the accident as a cataract cause. As late sequelae of eye (and orbit) contusion, enophthalmos and supersensitivity of the pupil to adrenergic drugs can be found. Ultrasound exophthalmometry proved to be more accurate for checking the former than Hertel's optical exophthalmometry a

Lindner posterior vitreous fistula with senile cataract extraction for quick reformation of anterior chamber: indications, technique, and results.

Pars plana partial vitrectomy using Lindner posterior vitreous fistula was prophylactically used with senile cataract extraction in cases known to be liable for nonreformation of anterior chamber and gelly vitreous with perfect results as regards postoperative maintenance of anterior chamber aphakic depth. The combined operation is indicated in cases (1) if the other eye was lost through nonreformation of anterior chamber after a perfect senile cataract extraction; and (2) in very old patients with very shallow anterior chamber especially when there are other senile ocular changes such as flat

Prophylaxis of aphakic cystoid macular edema using topical indomethacin.

The purpose of the present investigation was to define clinically the effect of topical indomethacin in preventing CME. The study was conducted on 50 patients who underwent bilateral intracapsular extraction of senile cataracts. One eye was used as the experimental subject and the other as the control. Postoperative alterations in the permeability of the blood-aqueous barrier and the chronological relationship of the changes in the retina and the blood-aqueous barrier were also studied. The mechanism underlying these postoperative ocular changes is discussed. In cases treated with indomethacin

Radiographic abnormalities in eyes with retinoblastoma and other disorders.

The importance of radiographic examinations of pathological ocular material is stressed. Characteristic ocular radiodensities are observed with retinoblastomas, calcified cataracts, senile scleral plaques, intraocular ossification, and a variety of radiodense foreign bodies. Radiographs supplement other techniques for the documentation of ocular abnormalities and in certain instances may be the ideal method. They also permit the precise localisation of radiodense foreign bodies that need to be removed prior to the processing of tissue for microscopic examination. In certain situations valuable

Phacoemulsification--why?

When a new operation replaces an old, it must be better, not just equally good. The Kelman method of cataract extraction has given impressive results in patients under the age of 30 years. However, for senile cataracts it is not better, the hospital stay is not shortened, and the recovery time is directly proportional to the age of the patient and to the preoperative condition of the eye, not to the type of extraction. Phacoemulsification is a cataract extraction and, therefore, should receive exactly the same monetary reward. No comparative series of cases done by the same surgeon or surgeons

[Indications and contraindications of the combined operation (fistulation and cataract extraction) in open- and closed-angle glaucoma (author's transl)].

The respective indications are briefly described for: a) trabeculectomy through a scleral flap, b) simple cataract extraction, c) combined operation of lens extraction and fistulation in open-angle glaucoma on the one hand and close-angle glaucoma on the other. The choice of operation is dictated by many considerations. The main considerations are as follows. A Lens extraction alone has no lasting effect on the pressure in open-angle glaucoma, but is a good operation against early closed-angle glaucoma. B. Any fistulising operation predisposes to cataract when performed on old patients whose l

Between the years 1962 and 1976 85 eyes which had undergone cataract surgery were accessioned to the Ophthalmic Pathology Laboratory of the Helsinki University Eye Hospital. The specimens were submitted from the various eye departments of the country. Of these 8k eyes nine had been enucleated within 2 months after surgery while in 64 cases the enucleation had been performed more than 12 months after surgery. 40 eyes had had an attempted operation for senile cataract, 30 eyes for traumatic cataract while the remaining cases were congenital cataract cases or cataracts in pre-existing glaucomatou
