BACKGROUND: Pupillary capture can cause complications after intrascleral intraocular lens (IOL) fixation; however, no method has been established to definitively prevent pupillary capture. Therefore, we aimed to examine the differences in the incidence of pupillary capture in patients who underwent intrascleral IOL fixation and had intraoperative lens capsule preservation or lens capsule loss.
METHODS: This single-center, retrospective study, conducted at a University Hospital, included 83 eyes from 83 patients. The eyes were allocated to the capsule and no-capsule groups based on the presenc

PURPOSE: We present a novel office-based technique using a 25-gauge needle to reopen closed peripheral iridotomies in aphakic eyes filled with silicone oil.
METHODS: We use a 25-gauge needle to enter the clear cornea and reopen the closed peripheral iridotomy, allowing for aqueous humor and silicone oil to escape and relieve elevated intraocular pressure.
RESULTS: This minimally invasive office-based approach provides immediate relief of elevated intraocular pressure due to silicone oil-induced pupillary block, avoiding the need for more invasive surgical intervention.
CONCLUSION: Our techn

Congenital cataract is a lens opacification that disrupts normal visual development, requiring early surgical intervention to prevent amblyopia. In children, timely surgery, followed by optical correction and vision rehabilitation, is crucial for achieving binocular vision with foveal fixation. The recommended surgical timing is within 8 weeks for unilateral cases and by 4 months for bilateral cases to minimize long-term visual impairment. Despite advancements in intraocular lens (IOL) technology and ophthalmic microsurgery, accurate IOL power selection in pediatric patients remains a challeng

PURPOSE: To present the long-term visual outcomes and factors affecting treatment success in children with congenital cataract surgery in infancy followed by optical correction by contact lens (CL).
METHODS: We retrospectively analyzed data from 30 eyes of 26 pediatric aphakic patients followed by visual habilitation primarily by CL between 2004 and 2023. The patients were evaluated for factors affecting final best-corrected visual acuity (VA), need for additional surgery, and adherence to CL and occlusion therapy.
RESULTS: The study included 16 eyes with idiopathic congenital cataract, 12 e

DESIGN: Prospective, Interventional series.
SETTING: Private practice.
METHODS: 11 eyes of 6 patients (3 males, 3 females) with microcornea underwent glued IOL scaffold procedure for aphakia associated with Soemmering ring. All patients had undergone initial cataract surgery at age less than 10 years and were rendered aphakic.

Intraocular lens (IOL) exchange is performed to treat severe IOL-related complications. There is little published data on the impact of this procedure on the refractive outcomes and intra- and postoperative complications, especially in otherwise healthy eyes. We investigated the refractive outcome of IOL exchange surgery, including corneal aberrations; additionally, we assessed the influence of different IOL characteristics on intraoperative and postoperative complications. This prospective clinical study included 35 eyes with homogenous IOL calcification without other ocular pathologies. Usin

Variants in gap junction protein alpha 8 (GJA8), the gene encoding connexin 50 (Cx50), are primarily associated with developmental cataract, although some are associated with severe structural eye anomalies, such as aphakia (absent lens), microphthalmia (small eyes), and sclerocornea. To further define the relationship of GJA8 variants to ocular developmental disorders, we screened four large international cohorts with structural eye anomalies, including anophthalmia, microphthalmia, and coloboma (AMC) or cataracts. We identified 15 new families carrying 14 different heterozygous GJA8 variants

Congenital aphakia is a rare eye condition in which the lens fails to form properly. It is typically caused by pathogenic variants within the FOXE3 or HCCS genes; however, it can also be associated with GJA8 pathogenic variants. GJA8 should be included in the genetic testing of patients with this condition.

INTRODUCTION: The retropupillary iris-claw intraocular lens (RP-IOL) offers a sutureless solution to complications like aphakia, intraocular lens (IOL) dislocation, and opacification post-cataract surgery. Unlike time-consuming, complication-prone traditional methods, RP-IOL potentially reduces surgical time and complications. This study evaluates RP-IOL's clinical outcomes to assess its efficacy and safety.
METHODS: This single-center retrospective case series reviewed medical records of 68 eyes from 68 patients who underwent RP-IOL implantation between January 2017 and May 2023. Preoperativ

IMPORTANCE: The optimal timing for secondary intraocular lens (IOL) implantation in children with bilateral aphakia remains uncertain, with important implications for long-term visual outcomes and the risk of complications such as glaucoma. Determining when to implant to achieve the best visual outcomes while minimizing complications is critical for improving clinical decision-making in pediatric cataract management.
OBJECTIVE: To report the visual acuity (VA) and complications in children with bilateral aphakia after lensectomy for pediatric cataracts, undergoing secondary IOL implantation a

PURPOSE: To evaluate long-term refractive changes following pediatric cataract surgery.
DESIGN: Retrospective case series.
METHODS: A total of 451 eyes from 273 patients who had undergone cataract surgery at age 6 years or younger (mean 1.8 ± 1.8 years) were analyzed, with a mean follow-up of 15.0 ± 2.5 years (range: 12-18 years). The cohort included 120 cases with bilateral aphakia, 58 cases with bilateral pseudophakia, 40 cases with unilateral aphakia, and 55 cases with unilateral pseudophakia.
RESULTS: In eyes that underwent surgery at or before age 2, the median myopic shift was -8.2 D

Background/Objectives: Different surgical techniques are available in cases of missing or insufficient capsular bag support. Next to the anterior chamber or iris-fixated intraocular lenses (IOL), the implantation of the Carlevale IOL provides a sutureless and scleral fixated treatment method. Methods: In a retrospective single-center study, the perioperative data of 100 patients who consecutively received a scleral fixated Carlevale IOL combined with a 25 gauge (G) pars plana vitrectomy between September 2021 and June 2024 were investigated. The intraoperative and postoperative results were an
