BACKGROUND/AIMS: Pediatric cataracts are considered a leading cause of visual impairment. While intraocular lens (IOL) implantation is mostly performed for children >2 years, its use in younger children remains contentious. This study assessed primary IOL in a unique pediatric population characterized by limited adherence to postoperative care, where primary IOL implantation was favored to minimize the risk of prolonged uncorrected aphakia.
METHODS: This retrospective study examined primary IOL implantation outcomes in children across three age brackets: below 6 months, 6-12 months, and 1-18

INTRODUCTION: One highly discussed topic in managing pediatric cataracts is determining when is it appropriate to implant an intraocular lens (IOL) in infants and young children who require cataract surgery. IOLs are an artificial replacement to the natural lens that may offer great visual benefit, but there are many factors to consider in the infant and young child population.
AREAS COVERED: This review covers the evidence on primary IOL implantation in infants and young children with a special focus on visual benefit and complications to consider in the timing of IOL implantation. PubMed, G

INTRODUCTION: The FIL SSF intraocular lens (IOL) (Carlevale, Soleko, Italy) is a sutureless scleral-fixated lens designed for cases of aphakia with insufficient capsular support. While generally stable, intraoperative complications such as haptic rupture may occur. We present a novel rescue technique for scleral fixation of FIL SSF IOL following rupture of the trans-scleral plug during combined penetrating keratoplasty, three-port pars plana vitrectomy, and IOL implantation.
CASE PRESENTATION: During the combined procedure, rupture of one FIL SSF IOL plug was identified. The compromised hapti

The reported prevalence of zonular weakness in the general population ranges between 0.46% and 2.6%. Several etiological risk factors are associated with zonular weakness, including increased age, pseudoexfoliation syndrome, Marfan syndrome, homocystinuria, retinitis pigmentosa mature cataracts, high axial myopia, and previous ocular trauma or surgery. Careful preoperative assessment or intraoperative recognition of the condition in the early steps of cataract surgery can aid in the prevention of complications and facilitate the management of the case. In this article, we summarized the availa

To present a case of a 20-year-old female who underwent a novel surgical technique for post-traumatic aphakia, aniridia, and astigmatism. Prior to surgery, the artificial iris (AI) (Customflex Artificial Iris, HumanOptics) was sutured to a toric intraocular lens (IOL) (Carlevale, Soleko), folded, and then implanted in the sulcus through a small corneal incision. The procedure was complication-free and resulted in favorable functional and cosmetic outcomes. The Carlevale IOL provides stable support for the AI, effectively addressing post-traumatic aphakia, aniridia, and astigmatism. This techni

Background: Scleral fixation of intraocular lenses (IOLs) is a valuable option in cases of aphakia or inadequate capsular support, yet conventional sutured and sutureless approaches can pose technical challenges and complication risks. The needle-guided scleral fixation technique offers a simplified, single-suture solution that enhances safety and reproducibility. Methods: In this retrospective interventional case series, 30 eyes with insufficient capsular support underwent IOL implantation using Meduri's needle-guided single-suture technique at the G. Martino University Hospital, Messina. The

We prefer using posterior capsular rent is a significant intraoperative complication of cataract surgery, often leading to vision-threatening sequelae if not promptly recognized and appropriately managed. PCR can result from various factors, weak zonules, posterior polar cataracts, including improper phacoemulsification techniques, and excessive instrument manipulation. Early recognition is critical, and swept-source optical coherence tomography and intraoperative assessments can enhance early detection. Prevention strategies focus on meticulous surgical planning, careful hydrodissection, judi
