Development and internal validation of a day-28 clinical risk stratification model for retinopathy of prematurity in preterm infants: A retrospective cohort study.

ObjectiveThis study aimed to develop and internally validate a day-28 clinical risk stratification model for retinopathy of prematurity in preterm infants.MethodsThis single-center retrospective cohort study included preterm infants who underwent retinopathy of prematurity screening between January 2019 and January 2026. Postnatal day 28 was defined as a fixed analytic prediction landmark rather than the scheduled date of the first ophthalmic examination. Candidate predictors were restricted to routine perinatal and early neonatal variables available from birth through day 28. The primary outc

Reported Retinal Detachment in Anti-VEGF-Treated Retinopathy of Prematurity: A Systematic Review and Meta-Analysis.

Objectives: To estimate the incidence of reported retinal detachment (RD) after primary anti-vascular endothelial growth factor (anti-VEGF) treatment for retinopathy of prematurity (ROP) and to explore study-level factors associated with RD incidence. Methods: PubMed, EMBASE, Cochrane Library, and Web of Science were searched up to 24 May 2026, for English-language studies reporting RD outcomes after anti-VEGF monotherapy for ROP. Pooled incidence was estimated with a random-effects model using Freeman-Tukey double arcsine transformation. Heterogeneity, subgroup differences, publication bias,

Extracellular Vesicles in Neonatology: Have We Found the Magic Bullet?

Extracellular vesicles (EVs) represent nanoparticles released by numerous cell types and exerting distinct biological roles. Extracellular vesicles are enclosed in a lipid layer and contain proteins, lipids, nucleic acids, carbohydrates or even mitochondria. The fundamental utility of EVs is intercellular communication, while their implication in various biological processes, such as inflammation and hemostasis, is well-documented. Growing evidence indicates the significant usefulness of EVs as biomarkers in certain prematurity-related diseases, like bronchopulmonary dysplasia (BPD) and necrot

Combined central retinal and posterior ciliary artery occlusion in a young female with primary myelofibrosis and Sweet's syndrome: A case report.

Primary myelofibrosis (PMF) is a rare myeloproliferative neoplasm that typically affects older adults but may present aggressively in younger patients. Thromboembolic complications are well recognized, though arterial occlusions are uncommon in this age group. Ocular ischemia associated with systemic inflammatory conditions such as Sweet's syndrome is exceedingly rare. A 22-year-old female with PMF, diagnosed 18 months earlier and managed initially with ruxolitinib, presented with sudden, complete loss of vision in her left eye. She also had a history of Sweet's syndrome managed with oral cort

Predicting Retinopathy of Prematurity in Thailand: A Practical Model for Risk-Based Screening.

Background/Objectives: Retinopathy of prematurity (ROP) screening must balance timely detection with feasibility, particularly in resource-limited settings. We aimed to develop a pragmatic, locally derived prediction model to support prioritization of ophthalmologic assessment among Thai neonates already eligible for ROP screening. Methods: We conducted a retrospective cohort study of neonates undergoing their first planned ROP screening examination at Chiang Rai Prachanukroh Hospital (2011-2015) and Lampang Hospital (2021-2022). Prespecified predictors included gestational age, birth weight,

Explainability-informed patient-disjoint evaluation of deep learning for plus disease in retinopathy of prematurity with an ophthalmologist reader study.

BACKGROUND: Plus disease is a treatment-relevant sign of retinopathy of prematurity, but fundus images from the same infant may differ in image quality, posterior-pole coverage, and visible vascular severity. We evaluated deep-learning classification using a patient-disjoint design, within-infant variability, and an ophthalmologist reader study. METHODS: The primary analysis included 732 Phoenix ICON fundus images from 38 infants, assigned exclusively to a 634-image development pool or a 98-image hold-out set from eight previously unseen infants. Five pretrained convolutional neural networks

Gestational age references and trajectory of fetal haemoglobin in relation to outcome in neonates born before 30 weeks of gestation: a single-centre retrospective cohort study in The Netherlands.

BACKGROUND: Fetal haemoglobin (HbF) has a high oxygen affinity and predominates during fetal life. After birth, HbF is progressively replaced by adult haemoglobin (HbA). In preterm neonates, transfusion with HbA-containing red blood cells (RBCs) may accelerate this transition, increasing tissue oxygen exposure and oxidative stress, which have been associated with retinopathy of prematurity, bronchopulmonary dysplasia, and necrotizing enterocolitis. We aimed to establish gestational age (GA)-specific reference values and examine postnatal HbF trajectories in relation to transfusion exposure and

Dysregulated Retinal DPP10 Expression with Single Cell-based Next Genomic Sequencing Profiling in X-linked Juvenile Retinoschisis.

X-linked juvenile retinoschisis (XLRS) is a hereditary retinal dystrophy resulting from mutations in the retinoschisin (RS1) gene, which lead to progressive structural abnormalities in the retina and consequent visual impairment. Despite this, the molecular mechanisms through which RS1 mutations induce cell type-specific transcriptional remodeling during disease progression remain inadequately elucidated. To delineate transcriptional changes associated with RS1 mutations, we conducted single-cell RNA sequencing (scRNA-seq) on a CRISPR/Cas9-engineered Rs1emR209C knock-in mouse model. Integrativ

Chorioretinal Scars in Congenital Cytomegalovirus Infection Following Prenatal Valacyclovir Therapy.

BACKGROUND: Congenital cytomegalovirus infection is the most common congenital infection. Ocular abnormalities affect approximately 14% of symptomatic children, but fewer than 1% of asymptomatic children. Prenatal valacyclovir reduces vertical transmission, but ophthalmic outcomes in exposed infants are poorly described. METHODS: We describe 2 neonates with congenital cytomegalovirus confirmed by amniotic fluid and urine polymerase chain reaction following first-trimester maternal primary infection. Both mothers received high-dose valacyclovir (8 g daily) from 13 weeks' gestation. Infants und

Oral eplerenone in first-episode acute central serous chorioretinopathy: results of a double-masked randomized trial.

BACKGROUND: Central serous chorioretinopathy (CSCR) is a pachychoroid disorder characterized by serous retinal detachment driven by choroidal vascular dysfunction and mineralocorticoid receptor (MR) overactivation. Eplerenone, a selective MR antagonist, has been proposed as a therapeutic option, although clinical evidence remains inconsistent. METHODS: In a randomized, double-blind, placebo-controlled trial, 44 patients with first-episode acute CSCR received either oral eplerenone (25 mg twice daily) or placebo for three months. Outcomes included best-corrected visual acuity (BCVA), central m

Reduced serum Dickkopf-1 levels as a potential supportive biomarker for the development and severity of retinopathy of prematurity: a prospective observational clinical study.

INTRODUCTION: Retinopathy of prematurity (ROP) involves impaired retinal vascular development, in which the Wnt/β-catenin signaling pathway plays a pivotal role in retinal vasculogenesis and angiogenesis. Dickkopf-1 (DKK-1) is a key endogenous inhibitor of this pathway. This study aimed to evaluate serum DKK-1 levels in preterm infants and explore its potential as a supportive biomarker associated with ROP development and severity. METHODS: In this prospective observational clinical study, preterm infants with a gestational age ≤32 weeks or birth weight ≤1500 g were enrolled. Serum DKK-1 leve

Barriers to artificial intelligence applications in ophthalmology: beyond diagnostic accuracy toward patient outcomes, workflow integration, ethics, and trust.

Artificial intelligence (AI) has become a major area of ophthalmic innovation, with a rapidly expanding literature, yet routine clinical adoption remains limited. Diagnostic accuracy alone is an insufficient marker of clinical value because even technically strong systems may fail to improve referral, follow-up, treatment, clinician workload, or health-system value. This focused narrative review examines the barriers that separate ophthalmic AI performance from real-world implementation. We reviewed ophthalmic AI literature indexed in PubMed, Web of Science, and Google Scholar, with emphasis o
