Clinical Features, Multimodal Imaging, and Neurological Differential Diagnoses in Recurrent Susac Syndrome.

BACKGROUND: Susac's syndrome (SuS) is a rare microangiopathy, often consisting of a classic, pathognomonic, clinical triad of sensorineural hearing impairment, encephalopathy, and retinal arterial occlusions. The variable presentation of SuS leads to underdiagnosis, despite the syndrome standing as an important differential in a variety of ophthalmic, neurological, and psychiatric presentations. Low case numbers and a fast progressive clinical course add to the difficulty of early recognition and treatment. METHODS: We report on a case of Susac's syndrome in a 40-year-old woman with the class

Comparison of Two AI Chatbots for Diagnosis and Providing Treatment Suggestions in Retinopathy of Prematurity: Retrospective Study.

BACKGROUND: Retinopathy of Prematurity (ROP) is a leading cause of preventable childhood blindness; yet, a global shortage of experienced pediatric ophthalmologists impedes timely diagnosis and treatment. While emerging AI chatbots are promising clinical decision-support tools in some ophthalmic diseases, their performance in ROP diagnosis and providing treatment suggestions remains uncertain. OBJECTIVE: This study aimed to compare the performance of Google's Gemini 2.5 Pro and OpenAI's ChatGPT o4-mini in ROP diagnosis and providing treatment suggestions against the gold standard of clinical

Effect of Prematurity on Foveal Development During Infancy: Increased Foveal Inner and Outer Retina Layer Thicknesses in Extremely Premature Infants.

PURPOSE: Foveation and inner retinal displacement are arrested in extremely premature infants through term equivalent age. We hypothesize that the effect of prematurity continues to impact the developing fovea in later stages of foveal development, well after term equivalent age. METHODS: Infants enrolled in the STudy of Eye imaging in Preterm infantS (BabySTEPS1; NCT02887157) were imaged at four and nine months' corrected age. For infants with gradable imaging, foveal and parafoveal inner (IRL) and outer (ORL) retinal layer thicknesses were measured, with parafoveal/foveal thickness (P/F) ra

Factors associated with loss to follow-up in retinopathy of prematurity screening referrals at a South African tertiary hospital: A retrospective cohort study.

BACKGROUND: Continuity of retinopathy of prematurity (ROP) screening follow-up is essential, as the disease progression can occur after the initial examination. Evidence on factors associated with loss to follow-up (LTFU) in South African ROP services is limited. METHODS: This retrospective cohort study reviewed 1698 infants referred for ROP screening at Inkosi Albert Luthuli Central Hospital from 2016 to 2021. Demographic and clinical data were extracted from referral records and the electronic medical records. Loss to follow-up was defined as failure to attend any scheduled follow-up after

Time-, Location-, and Layer-Specific Chorioretinal Biomarkers in a Translational Large-Animal Porcine Model of Chorioretinal Ischemia.

PURPOSE: Conditions associated with complete or partial ocular ischemia, such as central retinal artery occlusion (CRAO), are vision-threatening emergencies. Therapeutic interventions are highly time-dependent, yet the exact onset of ischemia is often uncertain. Optical coherence tomography (OCT) has emerged as a potential tool for objective staging; however, quantitative, layer-resolved OCT data within the earliest phase of retinal ischemia in clinically relevant large-animal models are lacking. METHODS: Ten eyes from 10 pigs were included. Physiological retinal perfusion was confirmed using

Human T-lymphotropic Virus Type 1 (HTLV-1)-Associated Uveitis Presenting With Frosted Retinal Vasculitis.

Retinal vasculitis is a manifestation observed in various forms of uveitis. However, frosted retinal vasculitis, a severe type of retinal vasculitis, is seen in only a limited number of cases, typically associated with severe infections or autoimmune inflammatory conditions. A 65-year-old woman presented to our hospital after noticing a decline in vision in both eyes over the preceding month. Examination revealed vitritis and frosted retinal vasculitis in both eyes. Fluorescein angiography showed marked leakage from the optic disc and retinal vessels. Optical coherence tomography (OCT) reveale

Scutellarin attenuates high glucose-induced edema and inflammation in retinal Müller cells via AQP4/Kir4.1 restoration and TLR4/MyD88/NF-κB inhibition.

BACKGROUND: Diabetic retinopathy (DR) is a metabolic eye disease closely linked to inflammation and oxidative stress, potentially causing vision decline or blindness. Scutellarin (SCU), a major flavonoid extracted from Erigeron breviscapus, possesses anti-inflammatory and antioxidant properties; however, its protective mechanisms against retinal Müller cell (RMC) injury in DR remain unclear. OBJECTIVE: To investigate the protective effects and underlying mechanisms of SCU against high-glucose(HG)-induced edema and inflammation in retinal Müller cells (RMCs). MATERIALS AND METHODS: Primary RM

Spontaneous Head Turn in Central Retinal Artery Occlusion With Nasal Cilioretinal Artery Sparing: A Case Report.

BACKGROUND Central retinal artery occlusion (CRAO) is an ophthalmic emergency and a retinal ischemic stroke. Cilioretinal arteries may remain perfused during CRAO because they arise from the posterior ciliary circulation. Most reported cilioretinal sparing involves papillomacular or foveal preservation; nasal sparing is atypical. We report CRAO with nasal retinal sparing and spontaneous head-turn behavior during screening. CASE REPORT A 55-year-old man presented with a 2-day history of sudden, painless vision loss in the right eye. He had newly diagnosed hypertension and was an active smoker.

Reactivation of retinopathy of prematurity after anti-VEGF treatment: a review.

Anti-vascular endothelial growth factor (anti-VEGF) agents have demonstrated efficacy and short-term safety in the treatment of retinopathy of prematurity (ROP), establishing them as the preferred option for zone I and zone II posterior disease. Despite their advantages, the effects of anti-VEGF agents are relatively temporary. Consequently, ROP may reactivate or recur if retinal vascularisation has not sufficiently progressed and significant areas of avascular anterior retina remain while the anti-VEGF agents are cleared from the vitreous cavity. Extreme prematurity, early onset of ROP and se

Role of stem cells in ocular diseases: Progress and challenges.

Ocular diseases, including corneal disease, glaucoma, age-related macular degeneration (AMD), diabetic retinopathy, and retinopathy of prematurity, can significantly impair vision and reduce quality of life. Because degenerated cells in these diseases are unable to regenerate, treatments for these conditions have limited efficacy. Stem cell therapies are revolutionizing the treatment of degenerative eye conditions, enabling structural and functional restoration through mechanisms such as cell replacement and paracrine signaling. This review examines advances in stem cell therapy for ocular dis

LncRNA DANCR Activates p38/mTOR-Mediated Autophagy via ANXA2 to Exacerbate Oxygen-Induced Retinal Neovascularization in Mice.

The mouse model of oxygen-induced retinopathy (OIR) recapitulates key pathological features observed in human retinal neovascular diseases, including retinopathy of prematurity (ROP). Recent research has highlighted the involvement of long noncoding RNAs (lncRNAs) in retinal neovascular diseases, but their specific roles remain unclear. Using human retinal microvascular endothelial cells (ECs) and OIR mice, we found upregulation of lncRNA DANCR in ROP models. DANCR overexpression enhanced EC proliferation, migration, and angiogenesis, while suppressing apoptosis via autophagy activation. RNA p
