[Retinal Artery Occlusions].

Retinal artery occlusions are acute vascular diseases. Very often they are caused by an embolisation. Rarely a thrombosis is caused by an arteriitis. In the acute phase diagnosis in most of the cases can be made by ophthalmoscopy due to the whitish retinal edema and in central retinal artery occlusion the cherry red spot. Difficulties in making the diagnosis can occur in the chronic stage, especially after reperfusion of the retinal vessels, when the fundus colour returned to normal again. Several diagnostic procedures help to make the diagnosis. Unfortunately there is no generally accepted th

Current treatment of central retinal artery occlusion: a national survey.

BACKGROUND: Central retinal artery occlusion (CRAO) is an ophthalmological emergency, the retinal analog of a stroke. To date there is no consensus or national guidelines on how this disorder should be managed. As academic neurologists and ophthalmologists treat CRAO frequently, we set out to understand how these clinicians approach patients with CRAO with a national survey. METHODS: We identified university-associated teaching hospitals offering vascular neurology, neuro-ophthalmology and/or retina fellowships in the US and asked the directors of the programs to respond to questions in an op

Severe bilateral optic nerve and retinal hypoperfusion in a patient with acute respiratory distress syndrome and septic shock.

PURPOSE: To report a case of bilateral central retinal artery occlusion with both anterior and posterior ischemic optic neuropathy. OBSERVATIONS: A 65-year-old Caucasian woman presented with acute respiratory distress syndrome and septic shock. After treatment with vasopressors and prolonged prone positioning, she was noted to be bilaterally completely blind on hospitalization day 12. Evaluation revealed evidence of bilateral central retinal artery occlusion and bilateral ischemic optic neuropathy. Magnetic resonance imaging of the orbits demonstrated severe restricted diffusion of both optic

Multimodal Images of Acute Central Retinal Artery Occlusion.

Two illustrative cases of acute central retinal artery occlusion (CRAO) are presented with multimodal imaging, including fluorescein angiography (FA) and commercially available optical coherence tomography angiography (OCT-A). In both patients, retinal ischemia was imaged well using both FA and OCT-A, and the two imaging studies provided comparable pictures. OCT-A provides useful information for the diagnosis and management of patients with acute CRAO, without the need for dye injection.

Preoperative aqueous humour flare values do not predict proliferative vitreoretinopathy in patients with rhegmatogenous retinal detachment.

BACKGROUND/AIMS: Patients with rhegmatogenous retinal detachment (RRD) who develop postoperative proliferative vitreoretinopathy (PVR) have been found to have higher preoperative laser flare values than patients with RRD who do not develop this complication. Measurement of laser flare has therefore been proposed as an objective, rapid and non-invasive method for identifying high-risk patients. The purpose of our study was to validate the use of preoperative flare values as a predictor of PVR risk in two additional patient cohorts, and to confirm the sensitivity and specificity of this method f

Vitrectomy for Epiretinal Membranes and Macular Holes in Uveitis Patients.

PURPOSE: Macular pathology, including macular holes (MHs), epiretinal membranes (ERMs), and macular edema, is a cause of irreversible vision loss in the setting of uveitis. Medical management involves corticosteroids, immunomodulatory medications, and biologics to control inflammation. Pars plana vitrectomy (PPV) has been suggested as a therapy for uveitis, with reports of reduced inflammation and decreased dependence on medication postoperatively. Currently, PPV is reserved for retinal detachments, vitreous opacities, and ERMs, causing vitreomacular traction. However, little is known about th

[Multimodal Approaches for the Analysis of Retinal Functional Disorders―Focusing on Retinal Detachment].

Microstructural and functional restoration of the retina after reattaching a retinal detachment were assessed by electroretinography (ERG), optical coherence tomography (OCT), adaptive optics (AO) fundus imaging, and laser speckle flowgraphy (LSFG). As a representative study, an intentional retinal detachment created during macular translocation surgery by a 360° retinotomy, reducing the cone ERGs by 12% was compared to the preoperation values. Six months after successful reattachment surgery, the amplitudes of the a- and b-waves of the focal macular ERGs (FMERGs) elicited by a 15° spot recove

[Understanding the Pathology and Current Treatment Strategy of Highly Myopic Complications].

The population of high myopia in Japan is large and increasing. It is a high risk factor not only for its specific macular complications but also as a precursor for cataracts and glaucoma. Myopic foveoschisis and the consequent macular hole with retinal detachment (MHRD) are caused by inner retinal traction which may include the vitreous cortex, epiretinal membrane, internal limiting membrane and posterior staphyloma formation. Vitrectomy is the 1st choice of treatment, and releasing the retina from traction is the goal. MHRD has poor anatomical success rates, and, therefore, inverted internal
