Ocular syphilis presenting as panuveitis: a management approach for the non-uveitis specialist.

We describe the challenging clinical scenario of an adult male presenting with acute, uniocular, sight-threatening panuveitis to a tertiary eye emergency department. Clinically, this was characterised by painless anterior chamber inflammation and significant vitritis, precluding clear fundal view.We outline the initial approach to establishing an appropriate working diagnosis, the laboratory investigations, multimodal imaging undertaken and empirical treatment administered in the first instance for imminently sight-threatening infectious causes. The principles described may be applied to other

Species-specific risk of ophthalmic involvement in candidaemia: a propensity-matched competing risk analysis.

OBJECTIVES: Given the limited data on species-specific ocular involvement in candidaemia, we aimed to assess the differential risk of ocular involvement according to Candida species in patients with candidaemia. METHODS: A retrospective review was conducted of patients with candidaemia who underwent funduscopic examination at a tertiary centre in Seoul, Republic of Korea between January 2014 and December 2023. Ocular involvement, defined as endophthalmitis, was determined by two retinal specialists. Propensity score matching and competing risk analyses were performed to adjust for pre-specifi

Primary Tubercular Chorioretinitis Without a Pulmonary Focus: A Case Report.

Ocular tuberculosis (TB) can affect various eye structures and may manifest independently of systemic TB. Typically, it arises from hematogenous dissemination from a primary focus; however, in exceptional instances, it may originate as a primary infection after epithelial injury. Diagnosing TB in an extrapulmonary site presents a significant clinical challenge. We present the case of a 33-year-old Bangladeshi female who presented with a deteriorating loss of vision in her left eye. A thorough neurologic examination and serological tests, the tuberculin skin test, a CT scan of the chest, ocular

Disseminated Mycobacterium chimaera infection successfully treated with a clofazimine-containing regimen and long-term follow-up after discontinuing treatment.

Mycobacterium chimaera is a slow-growing member of the Mycobacterium avium complex. It can contaminate tap water and has been detected in water tanks of heater-cooler devices used during open-heart surgery. Herein we report a case of a 67-year-old- male with disseminated M. chimaera infection who presented with fevers, significant weight loss and was found to have bacteremia with prosthetic valve endocarditis, chorioretinitis, bone marrow and splenic granulomas two years after an open-heart surgery. He developed multiple drug adverse events over the course of treatment but was successfully tre

A case report of Penicillium chorioretinitis in a Border Collie dog.

Systemic fungal pathogens typically gain entry through a single portal and disseminate to affect multiple body systems. Infections caused by fungal Penicillium species are rare in dogs, and these cases have a poor prognosis. An unknown Penicillium species was isolated from the right eye of a middle-aged Border Collie without evidence of systemic disease. Due to financial limitations, an extensive diagnostic evaluation could not be performed. Histopathology alone failed to identify the specific causative agent; however, fungal culture on aqueous humor provided the definitive diagnosis. This cas

Disseminated protothecosis caused by Prototheca bovis in two dogs.

Protothecosis is an infectious disease caused by unicellular algae of the genus Prototheca, which are mainly known for causing mastitis in farm animals. However, there are reports of protothecosis in humans and other animals, such as dogs and cats, which develop cutaneous lesions or systemic lesions. Dogs with the systemic form generally develop bloody diarrhoea and blindness and neurological signs may also occur. The condition generally leads to the animal being euthanized due to the unfavourable prognosis. The objective of this work was to report two cases of disseminated protothecosis, one

Long-term follow-up of a bilateral acute posterior multifocal placoid pigment epitheliopathy following COVID-19 infection: a case report.

BACKGROUND: Acute posterior multifocal placoid pigment epitheliopathy (APMPPE) is a rare inflammatory eye disorder that is characterized by the presence of multiple placoid lesions in the posterior pole of the eye. Relentless placoid chorioretinitis (RPC) is an inflammatory chorioretinopathy that combines clinical features of APMPPE and serpiginous chorioretinitis, which is a progressive condition with a high risk of visual disability. Patients with COVID-19 can develop various ocular manifestations, however, there have been limited reports of APMPPE and RPC associated with the infection. We r

Retinal ischemic diseases and promising therapeutic molecular targets.

Retinal ischemia is a fundamental pathologic condition associated with retinal vascular occlusion, glaucoma, diabetic retinopathy, age-related macular degeneration, and other eye diseases. Extensive inflammation, redox imbalance, apoptosis, and abnormal vascular formation in retinal ischemia could lead to visual impairments. Developing or finding effective treatments is urgently needed to protect the eye against retinal ischemia and related damage. To address the demand, we have searched for promising therapeutic molecular targets in the eye (e.g., hypoxia-inducible factor [HIF], peroxisome pr

Increased oxidative stress biomarkers in central serous chorioretinopathy.

Current data suggest that oxidative stress may play an important role in the occurrence of acute central serous chorioretinopathy (CSC), as chorioretinal integrity may be affected by disruption of the patient's metabolic redox balance, indicating the need for biomarkers. In addition to oxidative stress, high-density lipoprotein (HDL) dysfunction due to dyslipidemia can also lead to many types of physical discomfort. However, little is known about the pathophysiology of the disease resulting from oxidative stress and HDL dysfunction in CSC. The aim of this study was to investigate whether serum

Optical coherence tomography features in vitreoretinal lymphoma compared with non-infectious uveitis.

BACKGROUND: Vitreoretinal lymphoma (VRL) is a rare intraocular malignancy that poses a diagnostic challenge due to the non-specific clinical presentation that resembles uveitis. The use of spectral domain optical coherence tomography (SD-OCT) has emerged as a valuable imaging tool to characterize VRL. Therefore, we sought to determine the specific OCT features in VRL compared to the uveitides. METHODS: Retrospective chart review of patients who were seen at Mayo Clinic from January 1, 2010 through December 31, 2022. The medical records and SD-OCT images at time of initial presentation were re

Dose-Dependent Progression of Chorioretinal Atrophy at the Injection Site After Subretinal Injection of rAAV2/8 in Nonhuman Primates.

OBJECTIVE: Progressive retinal atrophy has been described after subretinal gene therapy utilizing the adeno-associated virus (AAV) vector platform. To elucidate whether this atrophy is a consequence of inherent properties of AAV, or if it is related to the surgical trauma of subretinal delivery, we analyzed data from an Investigational New Drug-enabling study for PDE6A gene therapy in nonhuman primates. DESIGN: Animal study (nonhuman primates), retrospective data analysis. SUBJECTS: Forty eyes of 30 healthy nonhuman primates (macaca fascicularis) were included in the analysis. Two AAV dose l

Systemic and ocular outcomes in TB-immunoreactive patients receiving immunomodulatory therapy for non-infectious uveitis: a case-control study.

BACKGROUND: Tuberculosis (TB)-immunoreactivity, measured in vivo (tuberculin skin test (TST)) or in vitro (interferon gamma release assay (IGRA)), can be found in latent, active or even following clearance of TB infection. In this case-control study, we compared the systemic and ocular outcomes between patients with or without TB-immunoreactivity, who received immunomodulatory therapy (IMT) for non-infectious uveitis. METHODS: We retrospectively reviewed charts of patients with (cases) or without (controls) TB-immunoreactivity (TST±IGRA), who received conventional IMT for ≥6 months, for the t

Macular neovascularization.

Neovascularization of the macula, a common complication of many chorioretinal diseases such as neovascular age-related macular degeneration, polypoidal choroidal vasculopathy, and pathologic myopia, results from increased synthesis of vascular endothelial growth factor (VEGF) by the retinal pigment epithelium and/or Müller cells because of localized ischemia and inflammation. The Consensus on Neovascular AMD Nomenclature (CONAN) study group acknowledged that these vessels may originate from either the choriocapillaris or the retinal microvasculature, prompting them to propose the term 'macular
