[Retinal tear in retinochoroiditis toxoplasmotica].

PATIENT: A 29-year-old white female presented with an episode of recurrent focal toxoplasmic retinochoroiditis in her right eye. Apart from a white active lesion between the temporal vascular arcades, marked vitreous opacification was present. Treatment with oral pyrimethamine and sulfadiazine led to resolution of retinochoroiditis activity including vitreous clearing within 10 weeks. Another 3 weeks later, a fresh peripheral retinal tear was noted on routine fundus examination of the right eye. While the left eye showed no signs of vitreoretinal pathology, biomicroscopy revealed a posterior v

[Late vision loss after focal hemorrhagic chorioretinopathy].

Prospective clinical studies about photocoagulation of extrafoveolar choroidal neovascularizations in focal hemorrhagic chorioretinopathy (CR) have demonstrated that the risk of visual loss years after successful treatment is related to the development of retinal pigment epithelium (RPE) atrophy around the laser scar. The reason for this event was thought to be late damage of RPE cells due to the laser treatment. However, because RPE atrophy can also be seen in untreated patients, a prospective study was started to test this pathogenetic hypothesis and to analyze the pathogenetic factors and p

[The clinico-immunological validation of a differentiated approach to the use of corticosteroids in treating S-dependent uveoretinitis].

A total of 630 patients with uveitis of different etiology aged 5 to 70 were examined. Signs of autoimmunization with retinal S-antigens (mol. weight 48 kD) associated with retinal involvement were detected in more than half of them. A moderate level of S-antigen in the tears was found to promote a favorable outcome of uveoretinitis. Deficit or a drastic increase of S-antigen titers in the tears, accumulation of S-antibodies and circulating S-antigen-antibody complexes in the serum, and blood leukocyte migration inhibition in response to S-antigen were observed in chronic and complicated cases

Microwave thermoradiotherapy for intraocular melanoma.

We evaluated the use of microwave hyperthermia as an adjuvant to ophthalmic plaque radiotherapy for patients with intraocular tumors. Forty-eight patients were offered combined microwave plaque thermoradiotherapy (TRT) as a primary treatment for their uveal melanomas. A dish-shaped microwave antenna was placed on the sclera beneath the tumor at the time of plaque brachytherapy. While temperatures were measured at the sclera, the tumor's apex was targeted to receive a minimum of 42 degrees C for a 45-min duration. Patients were also given full or reduced doses of plaque radiotherapy (125I or 10

[Toxoplasmosis-chorioretinitis: clinical course and treatment of seven patients].

Toxoplasmosis is a major and preventable cause of severe visual loss and blindness in young people. Ocular toxoplasmosis is the leading cause of posterior uveitis and in most cases it represents a late manifestation of a congenital infection. The clinical picture and anti-Toxoplasma therapy of seven patients referred to the Department of Infectious Diseases, Hvidovre Hospital is described. All patients had clinical ocular toxoplasmosis at initial examination with unilateral focal necrotizing retinitis associated with typical old, pigmented scars. All patients had anti-toxoplasmosis IgG antibod

Hyperlipofuscinosis and subretinal fibrosis in Stargardt's disease.

BACKGROUND: The fundus aspect of Stargardt's disease is characterized by progressive atrophic macular changes often associated with fundus flavimaculatus flecks. Other fundus changes have been described such as pallor of the optic disc, attenuated retinal vessels, cicatricial chorioretinitis, retinal pigment hyperplasia, subretinal neovascularization, or subretinal fibrosis. Three cases of Stargardt's disease and presumed subretinal hyperlipofuscinosis or subretinal fibrosis are presented. METHODS: The clinical history, and the ophthalmoscopic and fluoroangiographic characteristics of these t

TORCH syndrome.

The original TORCH complex described clinically similar congenital infections caused by Toxoplasma gondii, rubella virus, cytomegalovirus, and herpes simplex virus, types 1 and 2. Cutaneous manifestations, including petechiae, purpura, jaundice, and dermal erythropoiesis, are commonly seen in toxoplasmosis, rubella, and cytomegalovirus infections. In herpes simplex virus infections, 80% of symptomatic infants show single or grouped cutaneous vesicles, oral ulcers, or conjunctivitis. Extracutaneous signs and symptoms are variable and can be severe. Significant clinical signs in congenital toxop

Microperimetry in patients with central serous retinopathy.

In patients with acute central serous retinopathy (CSR), evaluation of visual acuity alone may not represent visual function. In patients with acute CSR, visual function may be disturbed by localized scotomas, distortion, and waviness. For the assessment of localized light sensitivity and stability of fixation, patients with CSR were evaluated by fundus perimetry with a scanning laser ophthalmoscope (SLO 101, Rodenstock Instruments). In all, 21 patients with acute CSR and 19 healthy volunteers were included in the study. Diagnosis of CSR was established by ophthalmoscopy and digital video fluo

Ocular Halicephalobus (syn. Micronema) deletrix in a horse.

Ocular contents from a horse with a 4-week history of severe unilateral uveitis were submitted for histopathologic examination. A severe unilateral granulomatous chorioretinitis with intralesional Halicephalobus deletrix was diagnosed. The horse developed progressive neurologic signs several days following the surgery to remove ocular contents and implant a prosthesis and was subsequently euthanatized. A severe multifocal granulomatous encephalitis with intralesional H. deletrix, localized primarily to the optic chiasm, thalamus, and brain stem, was diagnosed from tissues acquired at necropsy.

[Retinochoroiditis as a diagnostic indication of acute systemic toxoplasmosis in an immunocompetent patient].

HISTORY AND GENERAL INVESTIGATIONS: In February 1993 a 53-year-old immunocompetent man presented at our department with blurred vision on the right eye for 6 weeks. Following a journey to Guatemala in November 1992 he had developed undulating fever up to 40 degrees C (later subfebrile temperature) with loss of weight (15 kg), dysesthesia mainly in the feet and general weakness. He was hospitalized at a general hospital and treated with different antibiotics. Various examinations showed normal results, like cranial computer tomography, and serological tests for virus, bacteria or malaria. Only

[Ocular toxoplasmosis: value of immunoblotting for the determination of an intra-ocular synthesis of antibodies].

The aim of this study was to demonstrate the intraocular synthesis of specific antibodies in case of Toxoplasma retinochoroiditis on the basis of immunoblot analysis. Vitreous or aqueous and serum were analyzed by immunoblot in four cases of toxoplasmic chorioretinitis. The same assays were performed on samples from 16 control subjects. Immunoblot analysis of serum and ocular fluid from patients with toxoplasmic chorioretinitis showed IgG and IgA specific antibodies. Recognition patterns were different and demonstrated neo-antibodies in ocular fluid. In control samples only IgG were found and

[Focal hemorrhagic chorioretinopathy. Clinical differentiation and visual prognosis].

Macular choroidal neovascularization in young adults without any known underlying diseases is referred to under the general term of focal hemorrhagic chorioretinopathy. In endemic areas of the USA an infection with Histoplasma capsulatum is thought to be the causative agent, but in Europe the pathogenesis of this condition is unknown. With the aim of finding how European patients with this disease might be detected by clinical examination and to estimate the prognosis for sight in the affected eye and the fellow eye, a follow-up examination (follow up 1-24 years, mean 7 years) of 88 patients (

Intraocular candidiasis in patients with candidemia. Clinical implications derived from a prospective multicenter study.

PURPOSE: Intraocular infection caused by Candida species can have devastating visual consequences. With the emergence of Candida as a major nosocomial pathogen, the authors investigated the prevalence of ocular lesions in patients with candidemia and evaluated risk factors for eye involvement. METHODS: This study is a prospective, multicentered, observational design. One hundred eighteen patients with candidemia were evaluated by the infectious disease service and received indirect ophthalmologic examination within 72 hours of a reported positive blood culture. Ocular findings were classified

[Ocular murine cytomegalovirus infection in immunocompromised hosts].

Cytomegalovirus (CMV) chorioretinitis is one of the commonest complications in immunocompromised hosts. However, the entire scope of ocular involvement during generalized CMV infection in immunocompromised hosts is yet to be elucidated. Balb/c nude mice were inoculated intraperitoneally with Smith strain of salivary gland passaged murine cytomegalovirus. Eight days after infection, they were sacrificed and the pathological features of virus spread were studied by immunofluorescence method. Subconjunctival tissues, corneal stroma, outer ocular muscles and occasional choroidal tissue are the sit

[Pathogenesis and therapy of central serous retinopathy].

UNLABELLED: There is no consensus in the literature on the etiology and therapy of central serous retinopathy. We conducted a retrospective study to find out whether patients treated with argon laser coagulation (ALK) experienced long-term visual acuity improvement. We were also concerned to find whether choroidal perfusion was involved in the etiology. PATIENTS: In all, 28 patients (4 women/24 men) with RCS confirmed by angiography were analyzed. All patients were followed up from 1988 to 1992. Patients with chronic recurrent RCS were not considered. Angiography was performed to examine the

[Resorbable implant materials in retinal detachment surgery. Initial animal experiment studies].

In detachment surgery after a mechanically stabile chorioretinal scar is at present full scleral indentation no longer mandatory. Following these considerations absorbable scleral implants have been developed and tested in an animal model. Cylindrical pieces of absorbable composite-material with a diameter of 5 mm were sutured as radial scleral explants in 18 rabbits. The composite material consisted of a polyglactin-polydioxanon ratio 7:1. The absorption time for polyglactin is 60 days, for polydioxanon 180 days. The explants produced initially in ultrasonographically measured buckle height b

Detection of Toxoplasma gondii in aqueous humour by the polymerase chain reaction.

Ocular toxoplasmosis is the most frequent infectious cause of chorioretinal inflammation in immunocompetent patients. Nowadays, the biological diagnosis of ocular toxoplasmosis requires serological tests and anterior chamber puncture to detect the local production of specific antibodies. A new technique is described to detect Toxoplasma in aqueous humour by a polymerase chain reaction in which the target is a specific ribosomal DNA segment. Sixty eight patients (71 eyes) were included; 59 (83%) eyes were suspected of having ocular toxoplasmosis. Of these 59 eyes, 15 (25.4%) had characteristic

These two tests were found frequently positive especially during acute inflammation for the LST. However HBDT was positive in all the patients before relapse of acute inflammation which is in favor of its predictive value and its involvement in the triggering of inflammation at the basophiles or at t
