A case of cryptococcal retinochoroiditis associated with acquired immunodeficiency syndrome (AIDS) is reported. The patient was a 22-year-old man with hemophilia A. He was found seropositive for human immunodeficiency virus three years ago. He was admitted with complaints of fever and cough. CD4/CD8 ratio was 0.01 and CD4 lymphocyte count was 10/mm3. Two weeks later, ophthalmological examination revealed some yellowish white focal lesions at the posterior fundus. More detailed examinations were impossible because of his poor general condition. He died one month later. The autopsy showed dissem

AD169, a well-characterized laboratory strain of human cytomegalovirus (HCMV), was used to establish an animal model of progressive HCMV chorioretinal disease by injection of 10(5) pfu into the rabbit vitreous. Chorioretinal, vitreous, and pulmonary disease were monitored by HCMV recovery, clinical observation, antigen localization, and histopathology. Vitritis and focal areas of immune cellular infiltrates were seen in inner retinal layers on days 2-4 after inoculation. Disease progressed with more severe vitritis and to involve the outer retinal layers in areas of mixed monocytic cellular in

The decision to use therapy in toxoplasma retinochorioiditis depends on the location of the active lesion and the presence of vitreous activity. In eyes with dense vitreous clouding it can be difficult to see whether the macular region is involved or not. In theory the localisation of a lesion can be estimated on the basis of the flash ERG. The standard flash electroretinogram was recorded in 23 patients with inactive toxoplasma retinochorioiditis lesions in the retina. In 17 cases a lesion was present within the central 12 degrees of the visual field, 8 of these had a reduced photopic ERG. In

Retinochoroiditis is a frequent ocular lesion in toxoplasmosis. It may be severe, and even lead to blindness. Sixteen cases of toxoplasmic retinochoroiditis verified through IFA were presented in this paper. Among these cases, 1 was of the multifocal type, its appearance being similar to that of the Coats disease; while the other 15 were of the focal type resembling to the central exudative retinitis (Rieger disease) or retinochoroidal scar. Serological finding of all the cases showed a positive IFA titer of > 1:80. A combination of pyrimethamine and sulfadiazine as well as spiramycin in addit

The position of visceral toxoplasmosis in HIV infection has changed in the late 1980's. The strong prevalence of toxoplasmosis in the French population and the regression of pneumocystosis due to generalization of primary prophylaxis have made cerebral toxoplasmosis the initial manifestation of AIDS in about 20% of the cases. At the same time, a better management of AIDS patients has made it possible to hope for a longer survival, even in patients with very deep immunodeficiency. Altogether, these various elements are in favour of developing a primary prophylaxis in patients at high risk for v

Acute ocular toxoplasmosis is characterized by a focal retinochoroidal infiltrate, often appearing in the immediate vicinity of an old retinochoroidal scar (satellite lesion). There are atypical forms of ocular toxoplasmosis, including retinal granulomas, intraretinal and subretinal neovascularization, preretinal gliosis and retinal detachment. In rare instances, associated immunological phenomena, i.e. concomitant vasculitis and perivasculitis, have been described. We report on a 28-year-old male patient presenting with signs of congenital toxoplasmosis with intracerebral calcifications and i

Varicella embryopathy is a rare entity afflicting infants born to mothers who have contracted varicella during the first 20 weeks of pregnancy. The teratogenicity of varicella has not been established from epidemiologic studies, but isolated case reports describe characteristic malformations following early maternal infection. We describe a male neonate delivered at 40 weeks' gestation to a 26-year-old grava 2, para 2 mother who developed varicella during the first trimester. The infant lived 7 days and died of bronchopneumonia. At postmortem examination there was growth retardation, multiple

Examinations of 35 patients hospitalized after long ineffective antituberculous therapy on an outpatient basis have revealed serous detachment of pigmented epithelium or neuroepithelium of the retina in 17, hemorrhagic detachment of pigmented epithelium and neuroepithelium in 12, and residual symptoms of these conditions with macular dystrophy signs in 6 of these patients. The absence of chorioretinal foci that are characteristic of tuberculous inflammation of ocular internal membranes, absence of focal reactions in tuberculin diagnosis and no therapeutic effect in antituberculous therapy evid

In intraocular parasitosis the immunologic reactions in the vitrectomy-fluids depend on the parasitic agent: there is a lymphocytical reaction without any IgE production in toxoplasmic chorioretinitis, whereas there is a strong hypersensitivity reaction with a local IgE production as well as the presence of eosinophils and an antigenic stimulation in fluids with toxocara parasitosis. The electrophoretic patterns are quite similar in both cases.

Purified retinal pigment epithelial cells of bovine eyes have been fractionated by a series of buffer and detergent extractions. The electropherogram of the buffer-insoluble, Triton X-100-soluble fraction (RPE-TS) exhibits a major polypeptide band of M(r) 65 kDa and a variety of minor components. Electrophoretically purified 65 kDa-band protein (PEP-65) is immunologically unrelated to the known uveitogenic photoreceptor proteins, to other neural retina proteins, and to PEP-X, the RPE-melanin-bound uveitogenic antigen. An immunocytochemical study of eye tissues suggests that it is exclusively l

For immunocompetent patients, ocular toxoplasmosis is the most frequent infectious cause of chorioretinal inflammation. Nowadays, the laboratory diagnosis of ocular toxoplasmosis requires serological tests and anterior chamber puncture to detect the local production of specific antibodies. The authors describe a new technique to detect toxoplasma in aqueous humor by a polymerase chain reaction in which the target is a 88 bp specific rDNA fragment. 31 patients were concerned (23 highly suspect of ocular toxoplasmosis and 8 controls). The presence of the parasite in aqueous humor was found in 7

A 28 years old butcher contracted a systemic trichinosis after fasting sausage. Twelve days after onset of systemic symptoms he developed a hemorrhagic maculopathy in his left eye. In addition, on both eyes there were focal atrophies of the retinal pigment epithelium in the mid-periphery. We believe a causal connection of the maculopathy with his serologically proven trichinosis as a retinal involvement is well known in trichinosis and histologically demonstrated by Herrenschwand (1928). To our knowledge this is the first case of hemorrhagic maculopathy in trichinosis.

A case is reported of a teenage girl, who presented with a profound loss of vision in the right eye, secondary to retinal vasculitis. During the preceding year, a gradual change in her personality had been noted, associated with a deterioration in her intellect. She developed an area of pigment epithelial disturbance in the macular region of the right eye, and subsequently, optic atrophy. One year after this, she had a similar, but more localized episode in the left eye. She later developed involuntary movements, and two years after initial presentation, a diagnosis of subacute sclerosing pane

In the years 1988 and 1989, routine ophthalmological examinations of dogs from the company-owned beagle colony revealed a clinically inapparent chorioretinitis in 7.4 and 10% of the animals, respectively, as it has previously been described by Weisse et al. (1981). The alterations were seen mainly in the non-tapetal fundus, and they appeared more frequently in both eyes than in just one eye. Infection tests as well as virologic, bacteriologic and histopathologic investigations were performed in order to clarify the origin. A direct evidence of virus particles from processed ocular material by

Accurate diagnosis of uveitis is of great importance since the treatment for the various uveitis entities may differ considerably. In a large number of cases the clinical picture is sufficient to make an adequate diagnosis. There are cases in which the diagnosis cannot be made on clinical grounds alone and support is needed from laboratory tests. Only a limited number of tests have been proven to be useful as a diagnostic or prognostic aid. These include HLA-B27 typing in patients presenting with anterior uveitis and testing for angiotensin converting enzyme and lysozyme in case of suspected s

Due to the difficulties encountered in the etiologie diagnosis of papillo uveitis along with the increasing occurrence of fungal infections (in paralleled with the number of AIDS affected patients), we found of interest to report a case of exsudative macular chorioretinitis with poor evolution under steroid therapy. In this patient infection by Candida Albicans was suspected and confirmed only after isolation and culture of the fungal from a vitrectomy specimen. A therapeutic apponch using Fluconazole (Triflucan allowed a dramatic improvement of visual acuity within a few months. Thus, vitrect

Nonpenetrating or blunt ocular trauma, orbital trauma and systemic trauma may cause a variety of posterior segment abnormalities. Blunt ocular trauma may cause damage to the retina (commotio retinae), retinal pigment epithelium (retinal pigment epithelial edema), choroid (choroidal rupture) and optic nerve (optic nerve evulsion) alone or in combination. Traumatic macular holes and retinal detachment or dialysis may also occur after blunt ocular trauma. Trauma to the orbital tissues adjacent to the globe can cause concussive forces with damage to multiple structures within the eye (chorioretini

The outcomes of argon laser coagulation (ALC) of active tuberculous chorioretinal foci (mainly of exudative and hemorrhagic nature, macular and paramacular localization) in 26 patients are illustrated. The coagulation was performed concurrently with a combined general and local antituberculosis therapy (with 48 patients as controls). The developed direct ALC technique was used together with antibacterial therapy of the total surface of the inflammation focus itself. The examination findings prove that the effectiveness of the above technique is high, since the disappearance of the clinical sig

In the rabbit, after oral ingestion of 20 mg/kg/day of cyclosporin A (CsA) a high level of the drug is found in the blood. This level increases steadily during the first 3 days, leveling off by days 5-7. Although the blood level of CsA was within the "therapeutic window" of 400-600 ng/ml, the drug was not detected within the ocular tissues. Local application using 2% CsA in olive oil induced a high concentration of the drug in the cornea and conjunctive but no detectable levels within the intraocular structures. When a significant intraocular inflammation is induced in one of the rabbit eyes,
