Macular oscillatory potentials in humans. Macular OPs.

Studies of the focal macular electroretinogram (ERG) have been made with special reference to oscillatory potentials (OPs) by using a fundus monitoring system in humans. Human macular OPs consist of 3 to 4 wavelets (mean peak interval, approximately 6.5 msec). The distribution of OPs in relation to those in a- and b-waves was studied. The amplitudes of a-waves, b-waves, and OPs of the upper macula were significantly larger than those of the lower macula. The distribution of OPs is relatively sparse in the fovea, becoming more dense than the a- and b-waves from the fovea toward the parafovea, a

Laboratory tests in uveitis. New developments in the analysis of local antibody production.

Analysis of local intraocular antibody production is a valuable tool with which to confirm a suspected clinical diagnosis in uveitis. We have analysed paired serum and aqueous samples for the presence of specific antibodies against toxoplasma, cytomegalovirus, herpes simplex virus and varicella zoster virus. Of the patients retrospectively diagnosed as having toxoplasma chorioretinitis 75% had a positive antibody coefficient indicating specific antibody production in the eye. Local antibody production in the eye directed against CMV confirmed the suspected diagnosis of CMV retinitis in 50% of

[Experience in the differential diagnosis of peripapillary "geographic" choroid disease and tuberculous chorioretinitis].

The etiology of peripapillary choriopathy (so-called geographic serpiginous choroiditis) is still unknown. This condition should be differentiated from tuberculous chorioretinitis because both abnormalities are characterized by a prolonged chronic course with involvement of the choroid and retina in young and middle-aged patients. The present study analyzes the results of a follow-up of 32 patients with peripapillary choriopathy; before these patients were admitted to hospital, ocular tuberculosis was diagnosed in 19 and suspected in 13 of them by local specialists. The authors came to a concl

Multifocal posterior uveitis: clinical and pathological findings.

A pathological study was performed on the necropsy eyes of a 59-year old-woman who had suffered for nine years from multifocal posterior uveitis. The disease had been controlled by steroid therapy with good preservation of visual function. Extensive investigation did not reveal the aetiology. On macroscopic examination numerous focal lesions with various degrees of pigmentation were observed scattered across the fundi. These lesions were studied by light and electron microscopy and immunohistochemistry. There was ongoing chorioretinal inflammation in the foci, producing destruction of Bruch's

[HIV patient and eyes].

A large percentage of patients in stage IV of HIV infection (CDC classification) show changes in the ocular fundus. Most frequent are functionally unimportant cotton-wool spots resulting from a HIV-associated microvasculopathy. Infectious retinitis due to opportunistic organisms is in most cases caused by cytomegalovirus (CMV). Untreated patients may become blind. In case of general or local treatment of cytomegalovirus retinitis with ganciclovir, sight may be preserved on a long-term basis. The ophthalmoscopic appearance of the typical changes and their histological substrate are presented, a

[Classical immunosuppressive agents].

Immunodepressive or immunoregulatory drugs might be indicated among the therapeutic possibilities in uveitis and associated diseases, especially in those cases, which resist to specific treatment and steroids, and in those in which disorders of the immune system play a pathogenetic role. Classical cytostatic drugs mainly act by a reduction of the amount of circulating lymphocytes. The most useful seem to be the alkylating drugs (chlorambucil, cyclophosphamide) and antimitotics (procarbazine, colchicine), or antifolic (methotrexate), while the antipurines (azathioprine) seem to us less effectiv

[Interstitial tubulo-nephritis and uveitis (Nitu syndrome). Apropos of a case].

Acute renal failure due to tubulo-interstitial nephritis developed in a 12 year old boy. It was accompanied by an inflammatory syndrome consisting of a markedly increased erythrocyte sedimentation rate and high levels of C. reactive protein and fibrin. The association of these anomalies with an uveitis are typical of the Tinu syndrome (tubulo-interstitial nephritis uveitis) first described by Dobrin and al. in 1975. The interest of this case lies in the observation of focal chorioretinitis, as yet never described. The possibility of toxoplasmosis is discussed; it could be a direct etiologic fa

[The significance of the phenomenon of dynamic disadaptation scotoma in the early diagnosis of macular dystrophies].

Examinations of 202 patients (337 eyes) with different forms of macular dystrophies, such as idiopathic flat detachment of the retina in the macular area, central sclerotic dystrophy of the retina, tapetoretinal macular degeneration, outcome of local inflammation of pigmented epithelium, post-traumatic central chorioretinitis, etc., allowed to receive data confirming high information value of a method based on the phenomenon of dynamic scotoma of disadaptation as compared with examinations on the Amsler's grid and campimetry. Thus, metamorphopsias were revealed on the Amsler's grid in 193 eyes

[Experimental Candida albicans chorioretinitis].

The long term (12 months) clinical and histopathological features of experimental chorioretinitis induced by intravenous injection of Candida albicans (5-10 x 10(6) spores) in pigmented rabbits were reported. Nine of 26 animals developed bilateral and 11 of 26 animals unilateral chorioretinal lesions. The lesions were round, yellow-white spots initially growing with a fluffy border, and breaking into the vitreous body. Finally, the lesions became chorioretinal scars with bridging strands in the vitreous body. Histologically, the initial lesions were subretinal microabscesses of pseudoeosinophi

Late development of chorioretinal lesions in birdshot retinochoroidopathy.

The clinical manifestations of diffuse retinochoroidopathy after recurrent episodes of vitriitis led to the diagnosis of birdshot retinochoroidopathy in two patients in whom the initial lesions were cystoid macular edema and papillophlebitis. Although the onset and clinical characteristics of the early lesions varied, both patients had manifestations of chronic retinal vasculitis. We observed the late appearance and evolution of the characteristic cream-colored depigmented spots which were not present in the early stages of the disease. In addition the second case also developed choroidal neov

Peripheral retinal neovascularization in birdshot retinochoroidopathy.

A 69-year-old black woman had ocular findings consistent with birdshot retinochoroidopathy; these included cream-colored depigmented spots at the level of the retinal pigment epithelium and vitreitis. She also had peripheral retinal neovascularization with localized vitreous hemorrhage. There were no laboratory or systemic findings consistent with other disease entities.

Aqueous humor analysis as a diagnostic tool in toxoplasma uveitis.

Analysis of local toxoplasma antibody production to confirm a suspected clinical diagnosis of toxoplasma chorioretinitis is a valuable diagnostic tool. Determination of toxoplasma antibodies in the blood of the patient is of limited use. When blood toxoplasma tests are negative this indicates that toxoplasma as a causative organisms in the pathogenesis of uveitis is unlikely. A positive blood test is a sensitive test (100% patients positive) but not a specific test since so many healthy individuals already have undergone subclinical infection and have acquired humoral immunity against the para

[Diagnosis of ocular toxoplasmosis by the ELISA method applied to the determination of immunoglobulins of the aqueous humor].

The toxoplasmosic origin of focal chorioretinitis can be detected on the basis of certain clinical signs in the fundus. However, this must be biologically confirmed by studying the local production of specific antibodies. For that purpose, the rate of intraocular specific Immunoglobulins is compared to the serum rate. The ELISA method is well adapted to quantitative determination of microamounts in aqueous humors. The authors report a study of 103 patients of whom, 28 were suffering from uveitis, and have studied the correlation between clinical lesions and results of the ELISA test. There was

Rhodopsin-induced experimental autoimmune uveoretinitis: dose-dependent clinicopathological features.

We have studied the clinicopathological features of experimental autoimmune uveoretinitis (EAU) induced in Lewis rats by injection of different doses of rhodopsin and its illuminated form opsin. Rhodopsin consistently appears to be more pathogenic than opsin. Injected in Freund's complete adjuvant and pertussis adjuvant 50 micrograms of rhodopsin induces a frequency of severe EAU similar to 250 micrograms of opsin. Intensity, frequency and location of ocular inflammation are markedly dose dependent. At high dose (100-250 micrograms), rhodopsin induces severe bilateral uveoretinitis in all anim

Experimental autoimmune uveitis (EAU): immunophenotypic analysis of inflammatory cells in chorio retinal lesions.

Using a panel of monoclonal antibodies to guinea pig leukocyte antigens we have examined the phenotype of inflammatory cells present in focal chorio-retinal lesions developing in guinea pigs during S-antigen induced experimental autoimmune uveitis (EAU). The earliest choroidal infiltrates of T cells appeared to be T helper (Th) cells but in choroidal foci of inflammatory cells and in early retinal lesions, where rod outer segment lysis had occurred, the presence of a Pan T negative, T cytotoxic/suppressor (T c/s) positive population of effector cells was identified. In areas of more extensive

[Local production of specific antibodies in the aqueous humor in experimental Candida endophthalmia in rabbits].

An increasing frequency of ocular lesions due to Candida albicans has been reported, but clinical detection of lesions, which may cause permanent blindness, remains difficult. Hematogenous endophthalmitis caused by Candida albicans in the rabbit is a useful model to study the pathogenesis of the disease, and perhaps to find new methods of biological diagnosis. A suspension of Candida albicans was injected in 15 rabbits, according to the method described by Edwards et al. In 1975, causing uni or bilateral chorioretinitis in 12 of them. Unaffected eyes were chosen as controls. After aspiration o

Posterior segment manifestations of orbital trauma.

The posterior segment of the eye can be affected by blunt orbital trauma by either direct or indirect mechanisms or by equatorial expansion. Commotio retinae can occur either at the site of direct or indirect injury. Retinal detachments can be precipitated at the site of the direct injury by a necrotic tear, at the site of the indirect injury with a macular hole, or just posterior to the vitreous base if equatorial expansion plays a role. Contusion effects on the choroid can manifest as a localized hemorrhage, a hemorrhagic choroidal detachment, or a choroidal rupture. Chorioretinitis sclopeta

The causes of tissue damage in toxoplasmic retinochoroiditis.

The pathogenetic mechanisms of tissue damage in toxoplasmic retinochoroiditis are complex and incompletely understood. They include invasion and parasitisation of retinal tissues by the Toxoplasma parasite and localised tissue damage by a complex associated immune response. Clinical and experimental studies which shed light upon our understanding of the disease process are reviewed and discussed with regard to current policies in treatment.
