Ultrasonic chorioretinopathy: a chloroquine vs control study.

High frequency ultrasound was used to produce chorioretinal lesions in two groups of pigmented rabbits. The control group received no medications. The other group was treated with subretinotoxic doses of chloroquine. Our experiment showed that the untreated group developed focal chorioretinal lesions and ricochet lesions at lower energies than did the chloroquinated group. We postulated that chloroquine, a melanin binding drug, altered melanin's ability to process ultrasonic energy by sonic-thermal conversion. This work suggests that chloroquine, even in subretinotoxic doses, may still exert a

Demonstration of intraocular synthesis of immunoglobulin G toxoplasma antibodies for specific diagnosis of toxoplasmic chorioretinitis by enzyme immunoassay.

By using a modified enzyme immunoassay we have conducted a detailed study on the intraocular synthesis of toxoplasma antibodies during ocular toxoplasmosis. In control patients operated on for cataracts, the ratio of immunoglobulin G (IgG) toxoplasma antibodies in serum to those in aqueous humor was more than 100. In eight of the nine patients with clinically diagnosed toxoplasmic chorioretinitis, the ratio ranged between 6 and 56. In five patients with some other type of uveitis, the ratio was comparable with that in controls. Parallel determinations of mumps IgG antibody ratios were carried

Subacute sclerosing panencephalitis: fundus changes and histopathologic correlations.

This paper describes the fundus changes in 2 patients with subacute sclerosing panencephalitis (SSPE). In both cases the ocular involvement preceded the neurological symptoms by several months. In one patient a localized serous detachment in the macular region was associated with retinal infiltrates and in the second patient an atypical unilateral macular chorioretinitis was first diagnosed. The diagnosis of SSPE was suspected when neurological signs appeared and it was confirmed by the typical EEG changes and especially by the high titers of measles antibodies in serum and CSF. The affected l

Atypical histopathologic features in sympathetic ophthalmia. A study of a hundred cases.

One hundred cases of clinically acceptable sympathetic ophthalmia (SO) were evaluated histologically to determine the incidence of atypical features and correlate them with the severity of choroidal inflammation. The inflammatory response in the choroid varied from a focal non-granulomatous to a diffuse non-necrotizing granulomatous process. The choriocapillaris was focally involved rather frequently (in 40% of the cases). This was associated with severe inflammation. Chorioretinal adhesions were noted in 7% of the cases. Retinal detachment was observed in 50% of the cases, and this was associ

Fuchs' heterochromic cyclitis and ocular toxoplasmosis.

Ocular toxoplasmosis and Fuchs' heterochromic cyclitis are well-defined diseases that have completely different clinical courses. We studied 13 patients (seven men and six women, ranging in age from 18 to 73 years) who had Fuchs' heterochromic cyclitis and the focal necrotizing chorioretinal lesions characteristic of ocular toxoplasmosis. The reason for this association is still unknown, as is the pathogenesis of Fuchs' heterochromic cyclitis. None of the patients had ciliary injection or posterior synechiae, but 11 of the 13 (84.6%) had keratic precipitates, anterior chamber reaction, and cat

[Relapsing corticodependent posterior pole serous retinal detachment (author's transl)].

A case of serous retinal detachment and episcleritis in a 26 years old woman is reported. The detachment, always, unilateral, disappeared in a few days with corticotherapy, but relapsed eight times in a year when steroids were discontinued (4 times in R.E. and 4 in L.E.). No systemic pathology was found. The detiology probably involved a localized auto immune process. Based on the angiographic findings typical of a central serous chorioretinopathy) and the evolution (similar to Harada's disease) this case supports the hypothesis that relates both diseases as belonging in to the same nosologic

Antibodies against Toxoplasma gondii in the aqueous humour of patients with active retinochoroiditis.

In active cases of ocular toxoplasmosis, antibodies against the parasite are produced locally in the eye. This antibody production may be revealed by examination of the aqueous humour, and comparing the antibody titer with that of serum. We examined the dye test titer as well as the total IgG concentration of the aqueous humour and serum in 10 patients with active retinochoroiditis. Six of the 10 patients had a significantly higher antibody titer in the aqueous humour than in serum, in relation to their contents of total IgG, proving that the retinochoroiditis was caused by infection with Toxo

Ocular coccidioidomycosis.

Coccidioidomycosis has been recognized as a common systemic disease since the late 1930's. The occurrence of ocular lesions associated with the systemic infection is uncommon. The anterior segment manifestations appear to be a mild hypersensitivity response; the disease may present as a chronic granulomatous iridocyclitis. In the posterior eye, the manifestations may range from asymptomatic focal chorioretinitis (somewhat similar to the peripheral atrophic lesions of the ocular histoplasmosis syndrome) to a fulminating granulomatous process involving the entire eye. The ocular lesions can occu

Late cerebral relapse of congenital toxoplasmosis.

Focal encephalitis occurred in a girl with activation of chorioretinitis which on clinical and serological grounds was taken to be caused by toxoplasma. The encephalitis and acute ocular inflammation resolved with treatment with pyrimethamine and sulphadiazine. This is presumably an example of reactivation of congenital cerebral toxoplasmosis.

[Branch artery occlusion due to focal necrotizing retinitis -- probably caused by toxoplasmosis (author's transl)].

A case of necrotizing retinitis, probably due to toxoplasma gondii, is described in a 21-year-old man. Transient occlusion of the arteriole lying over the lesions was confirmed angiographically. The presence of inflammatory chorioretinal foci in the vicinity of retinal vessels was documented photographically in three other cases. The fundus changes and the field defects in these patients suggested that retinal perfusion had been temporarily impaired. The customary administration of steroids combined with antibiotics appears justified in order to minimise the occurrence of retinal vascular dama

Chorioretinitis sclopetaria.

Chorioretinitis sclopetaria occurs as a result of missiles penetrating into the orbit and grazing, but not perforating, the sclera. Localized destruction of both the retina and choroid develops adjacent to the impact site and these structures are subsequently replaced by a connective tissue response. Extensive damage is produced in the retinal circulation, which is not confined to the zones containing connective tissue plaques. Residual field defects are found to relate more closely to areas of retinal capillary non-filling than to the while scarred areas.

[Differential diagnosis of blurred optic disks by fluorescein angiography (author's transl)].

Fluorescein angiography has been underrated as an exploratory tool in the diagnosis of blurred optic disks. The ten cases presented here had blurred disks of diverse etiology; fluorescein angiography helped in the differential diagnosis and therapeutic approach. Irrigation of the disk, type, magnitude and location of dye leakage, as well as objectivation of peripapillary and retinal vascular characteristics all help to explain the pathogenic mechanisms leading to different types of optic disk edema. Although it is known tha papilledema due to increased intracranial pressure can not be differen

Vascular obstructive alterations related to chorio-retinitis.

Different stages of retino-choroiditis, many of toxoplasmic origin, may develop vascular alterations. One of these pictures has been observed in relation to old scars of retino-choroiditis, where focal vasculitis may affect the venous return circulation sometimes leading to intraocular hemorrhages and retinal holes of variable severity. The natural history of this process is described, as well as the clinical characteristics and the results of treatment. Generally, the disease has a favorable course and a good prognosis. 18 cases are reported.

Multifocal retinopathy in Borna disease virus infected rabbits.

Experimental infection of rabbits with Borna disease virus led in all cases to a multifocal retinopathy that paralleled the clinical neurologic symptoms. The retinal changes always became evident first in the lower anterior quadrant of the eye. Infectious virus and antigen were detected in altered and unaltered regions of the retina. Individual chorioretinal lesions showed destruction of the pigment epithelium and the photoreceptors and perivascular inflammation close to small choroidal veins. Because of maximal antigen accumulation and the focal destruction of the retinal pigment epithelium w

Pigmentary paravenous chorioretinal degeneration: fundus appearance and retinal functions.

A case of pigmentary paravenous chorioretinal degeneration initially diagnosed as retinitis pigmentosa is presented. The focal macular visual evoked response (VER) was normal. Both rod and cone components in the electroretinogram showed reduced amplitude with normal peak time. The psychophysical threshold in the dark adaptation was nearly normal in some parts of the retina and extremely elevated in other parts of the retina. Fundus appearance and retinal function studies suggest that the impaired area in the retina and choroid is localized in this disease.

Two intravitreal Taenia cysts were removed intact by pars plana vitrectomy from a 59-year-old woman who had never left the continental United States. The intraocular course of the cysts progressed from an initial chorioretinal location, accompanied by an intense focal inflammatory reaction, to free floating in the vitreous cavity within two months; thereafter, there was only a low-grade vitritis for an additional five months until removal. Light and electron microscopic studies suggested Cysticercus cellulosae as the infecting agent, although mature hooklets were absent. Local pork products we
