[Eye injuries in 'paintball'; a modern 'war injury'].

Paintball shooting is played with increasing frequency in the Netherlands. A bullet, filled with paint, shot at 60 m/sec can cause serious damage to the eye. All five patients seen in two years showed a hyphaema. Four out of five patients showed damage of the posterior pole, including vitreous haemorrhage, choroidal rupture, retinal detachment, and sub- or intraretinal haemorrhage. Three out of five patients incurred permanent visual loss, varying from 3/60 to 25/100. Adequate instruction of the participants in the game and proper use of the eye protector will be of help in preventing eye inju

Retinal lesions experimentally produced by intravitreal ultrasound.

Currently, vitreous surgical techniques combined with phakoemulsification are an established procedure for the removal of luxated natural lenses or dislocated lenticular material. This has proven to be a safe procedure, but occasionally retinal lesions have been inadvertently created. We describe the ophthalmoscopic and morphologic features of ultrasonically induced retinal lesions in rabbit eyes using a similar technique. The retina was treated directly using a conventional ultrasound tip for pars plana lensectomy (Fragmatome). Ophthalmoscopically slight lesions corresponded to an area of ret

A case of distal anterior choroidal artery aneurysm: case report and review of the literature.

We describe a very rare case of distal anterior choroidal artery aneurysm. A 34-year-old woman was admitted to our hospital because of headache and vomiting. Computed tomographic scans revealed a diffuse intraventricular hematoma and a thin subarachnoid hemorrhage. Cerebral angiography disclosed a saccular aneurysm in the peripheral portion of the right anterior choroidal artery, which was located in the temporal horn of the right lateral ventricle. We approached the temporal horn through a small inferior temporal incision, and the aneurysm was resected without any neurological damage. A histo

Closed-system phacoemulsification and posterior chamber implant combined with penetrating keratoplasty.

The main intraoperative difficulties of performing a procedure combining open-sky extracapsular cataract extraction, implantation of posterior chamber intraocular lens (PC-IOL), and penetrating keratoplasty ("triple procedure"), most frequently caused by the uncompensated posterior pressure created when the cornea is open, include incomplete capsulorhexis, incomplete aspiration-irrigation of the cortex, uncertain placing of the IOL, posterior capsule rupture, choroidal effusion, and even expulsive hemorrhage. We recommend a two-step procedure that eliminates these problems: The first step begi

[Resorption of post-traumatic cilio-choroid (supra-choroid, sub-scleral) hematoma. A histopathologic study].

The healing process of ciliochoroidal (suprachoroidal, subscleral) hematomas was studied histopathologically in 75 eyes enucleated between 1 and 42 days following a traumatic incident. The ciliochoroidal hematomas occur from ruptured vortex veins or ciliary blood vessels located in the suprachoroidal (subscleral) space. Detached ciliary blood vessels and nerves form a network within the hematoma. In early stages there is little fibrin formation and no signs of coagulation are visible. Then hyperemia of uveal blood vessels develops, followed by a perivascular accumulation of polymorphonuclear l

Changes in the fundus caused by blunt ocular trauma.

Generally encountered during adolescence, blunt ocular trauma results in a variety of changes in the fundus. Our study investigated the distribution of trauma-induced changes in the fundus on the basis of the incidence and the latent interval between the trauma and the occurrence of the symptoms. In addition, we studied the distribution of the cases in regard to patient age and sex and the cause of the trauma. Included in this study were the 445 eyes, whose fundi could be evaluated, of 435 patients, who applied to the Eye Clinic of the Faculty of Medicine, Ankara University, because of blunt e

Outpatient treatment of microscopic and rim hyphemas in children with tranexamic acid.

Twenty-one patients aged 4 to 15 years with microscopic or rim hyphemas were treated as out-patients with systemic tranexamic acid therapy. The protocol included limited activity at home, topical steroid therapy, no dilating drops, patching for comfort or surface abrasions only, and follow-up every 24 to 48 hours. With this regimen no patient had a secondary hemorrhage. The incidence of associated injuries was surprisingly high, including four patients with angle recession, two with choroidal ruptures and four with commotio retinae, two of whom manifested retinal holes, requiring cryotherapy.

Anomalous arrangement of the origins of the anterior choroidal and posterior communicating arteries. Case report.

A case of anomalous origin of the anterior choroidal and posterior communicating arteries in a 36-year-old woman with aneurysmal subarachnoid hemorrhage is presented. Preoperative four-vessel angiography revealed a reverse relationship of these vessels, wherein the anterior choroidal artery origin was proximal to the origin of the posterior communicating artery. This arrangement is extremely rare; the only other reported case was in a patient undergoing angiography prior to acoustic neurinoma resection. It is believed that this is the first case reported in a patient with subarachnoid hemorrha

Subretinal fibrosis after laser photocoagulation for diabetic macular edema.

Seven eyes had subretinal fibrosis after grid laser photocoagulation for diabetic macular edema. The fibrosis caused persistent loss in visual acuity, and in six of the seven eyes, was not associated with detectable laser-induced Bruch's membrane rupture or subretinal hemorrhage. Choroidal neovascularization was detected in only one patient, who was notably younger (27 years) than the median age of 70 years in this series. The median preoperative visual acuity was 20/80 (range, 20/40 to 20/400); the median postoperative visual acuity was 20/400 (range, 20/80 to counting fingers). The subretina

[A ruptured aneurysm at the peripheral collateral circulation of the anterior choroidal artery in a patient with moyamoya disease: a case report].

This 42-year-old man experienced a sudden onset of occipital headache. Neurological examination revealed a moderately disturbed consciousness and a moderate left hemiparesis. CT scan disclosed a hugh hematoma in the right temporo parietal lobe without intraventricular hemorrhage. A cerebral angiography demonstrated typical findings of moyamoya disease and a small saccular aneurysm at the peripheral portion of the right anterior choroidal artery, which was dilated at the collateral circulation to the parietal lobe. The hematoma was removed at once by a craniotomy. He became alert but mild hemip

Comparative histopathological study of free-running mode and continuous wave Nd:YAG laser photocoagulation of monkey retina and choroid.

The effects of free-running mode (FRM) or continuous wave (CW) Nd:YAG laser application on the retina and choroid of 12 monkeys were examined periodically up to 24 months by light and electron microscopes. FRM Nd:YAG laser produced severe choroidal hemorrhage even in mild burns, because of the extensive coagulation of melanocytes at the center of the burned lesion. The rupture of Bruch's membrane did not depend on the power of the laser. On the other hand, CW Nd:YAG laser rarely produced subretinal hemorrhage through the rupture of Bruch's membrane in the medium-grade and severe burns. However

Ocular injuries caused by elastic cords.

Five patients with ocular trauma due to elastic cords with attached metal or plastic hooks were evaluated over a 13-month period. Injuries included hyphema, angle recession, lens subluxation, choroidal rupture, vitreous hemorrhage, retinal detachment, and scleral rupture. Two patients suffered penetrating ocular injuries, and three patients had final visual acuities of 20/200 or less. This series emphasizes the potential severity of ocular injury due to these commonly used devices and reiterates a modification in design that might decrease the likelihood of accidental trauma from elastic cords

[Generalized choreic movement associated with subarachnoid hemorrhage].

A case of generalized choreic movement associated with subarachnoid hemorrhage is reported. A 71 year-old hypertensive woman suddenly developed severe headache 14 days before admission. Consciousness disturbance and involuntary movement involving the face and upper extremities appeared about 8 days after onset. The involuntary motion was diagnosed as generalized choreic movement. CT scans showed subarachnoid hemorrhage with ventricular dilatation and periventricular lucency involving bilateral caudate nuclei. On admission the patient was stuporous with Hunt & Kosnik Grade 4. She showed involun

Management of traumatic rupture of the globe in aphakic patients.

We report the successful treatment of three cases of traumatic ruptures of the globe complicated by massive choroidal hemorrhage, uveal prolapse and retinal detachment. All three of the eyes were aphakic prior to injury and all patients were age 64 or older. The presenting visual acuity in all patients was light perception. The blunt injury in each case caused a wound dehiscence at the site of previous cataract extraction. All injuries were associated with uveal prolapse. Secondary surgical intervention was performed when the hemorrhagic choroidal detachments had decreased as demonstrated by e

Transpupillary irradiation of the rabbit retina with the cw-Nd:YAG laser. I. Acute morphologic effects produced using two different pulse forms.

The retinas of six pigmented rabbits (12 eyes) were irradiated with a cw-Nd:YAG laser using two different pulse forms, in conjunction with a constant pulse duration of 200 msec and a retinal focus diameter of 200 microns. The first pulse form (a) was characterized by an initial transient, lasting 15-20 msec, with a peak power twice that of the ensuing plateau phase of 180-185 msec. The second pulse form (b) was almost rectangular in shape. Although the average energies (and powers) for each pulse form were identical, almost 20% of this entity was delivered within the first 20 msec for the firs

Posttraumatic yellow maculopathy.

Blunt trauma to the eye may cause several types of macular abnormalities, including commotio retinae, macular hole, intraretinal and subretinal hemorrhage, and choroidal rupture. In most cases the diagnosis is readily apparent; however, the complex appearance of simultaneous hemorrhage, pigmentary change, and ruptured tissue planes may be confusing. The authors present two cases of blunt trauma in young males that resulted in a subretinal hemorrhage that became yellow and was initially misinterpreted as a vitelliform lesion. A history of trauma should be sought in any patient with the clinical

Choroid plexus arteriovenous malformation in a full-term neonate. Case report.

A full-term newborn infant with intracranial hemorrhage secondary to an arteriovenous malformation (AVM) of the choroid plexus is presented. Although angiography was not undertaken before surgery, a vascular nodule containing an AVM nidus was found in the hematoma wall. The authors stress that an AVM should be considered as a cause of intracranial hemorrhage and that the hematoma wall be searched thoroughly in neonates without predisposing factors.

Sudden death from choroid plexus vascular malformation hemorrhage: case report and review of the literature.

A case of primary intraventricular hemorrhage in a previously healthy 13-year-old boy due to rupture of a choroid plexus vascular malformation is described. Review of the literature reveals that individuals with choroid plexus malformations usually present under age 40 with serious neurological symptoms. Sudden death, however, is an unusual outcome. This case and literature review illustrate the importance of early clinical recognition and surgical intervention. Choroid plexus should be microscopically examined at autopsy in any individual in which the cause of intracerebral hemorrhage is not

Factors prognostic of visual outcome in patients with subretinal hemorrhage.

We reviewed the charts of 29 patients with large subretinal hemorrhages involving the center of the fovea to evaluate factors that might be prognostic of visual outcome. The average final visual acuity was 20/480 with a mean follow-up of three years. Patients with thick hemorrhages (causing an obvious elevation of the fovea) had worse final visual acuity than patients with thin hemorrhages (P = .02). The diameter of the hemorrhage was not a significant predictor of outcome. Patients with aging macular degeneration had poorer final visual acuity (mean, 20/1,700, P = .002), and patients with cho
